본 연구는 성인 남녀를 대상으로 긍정적 평가에 대한 두려움과 부정적 평가에 대한 두려움이 사회불안 증상과 관련되는 과정에서 긍정결과 가치절하 및 부정결과 파국화와 회피 안전행동이 순차적으로 매개하는지를 검증하고자 하였다. 이를 위해 20세 이상 49세 이하 성인 464명의 자료를 수집하여 구조방정식 모형을 통해 분석하였다. 분석 결과, 긍정적 평가에 대한 두려움과 부정적 평가에 대한 두려움은 모두 사회불안 증상과 직접적으로 관련되었다. 또한 두 평가 두려움은 각각 긍정결과 가치절하와 부정결과 파국화를 거쳐 회피 안전행동으로 이어지는 경로를 통해 사회불안 증상과 간접적으로도 관련되었다. 더 나아가 교차 경로 또한 유의한 것으로 확인되어, 사회불안이 높은 개인의 평가 두려움이 특정 해석편향에만 제한되지 않고, 긍정적 정보의 의미를 축소하고 부정적 결과를 과장하는 복합적 인지과정과 연결될 수 있음을 시사한다. 본 연구는 긍정적 및 부정적 평가 두려움, 사회적 사건에 대한 해석편향, 회피 안전행동을 통합적으로 검증함으로써 사회불안의 인지․행동적 유지기제를 이해하는 데 기여한다.

This study investigated whether fear of positive evaluation (FPE) and fear of negative evaluation (FNE) influence social anxiety through the sequential mediation of interpretation biases—specifically, disqualification of positive outcomes and catastrophizing of negative outcomes—and the use of avoidance-based safety behaviors in adults aged 20 to 49. Data from 464 participants were analyzed using structural equation modeling. The results indicated that both FPE and FNE had significant direct effects on social anxiety symptoms. In addition, both variables also had indirect effects on social anxiety symptoms through pathways involving disqualification of positive outcomes and catastrophizing of negative outcomes, followed by avoidance-based safety behaviors. Furthermore, cross-path effects were also significant, suggesting that FPE and FNE operate at a generally similar level in influencing social anxiety symptoms. These findings suggest that fear of evaluation may elicit specific cognitive interpretation biases and contribute to the maintenance and exacerbation of social anxiety through avoidance-based safety behaviors.

사회불안장애의 핵심 특징을 측정하기 위한 도구로서 긍정적 평가에 대한 두려움 척도(Fear of Positive Evaluation Scale; FPES)와 단축형 부정적 평가에 대한 두려움 척도(Brief Fear of Negative Evaluation Scale; BFNE)는 선행연구들을 통해 신뢰도와 타당도가 확인되었다. 그러나 두 척도는 별도로 사용되어 측정이 이루어지므로, 사회불안을 지닌 개인이 경험하는 평가에 대한 두려움을 통합적으로 설명하기 어렵다는 제한점이 있다. 이에 본 연구에서는 양가적 평가에 대한 두려움 척도(the Bivalent Fear of Evaluation Scale; BFOES)를 한국 성인 표본을 대상으로 타당화하는 것을 목적으로 하였다. 이를 위해 20~50대 성인 400명을 대상으로 신뢰도 및 타당도를 검증하였다. 분석 결과, 본 척도는 원척도와 동일하게 3개의 하위요인(긍정적 평가에 대한 두려움, 부정적 평가에 대한 두려움, 평가 전반에 대한 두려움)과 이를 설명하는 상위요인으로 구성된 위계적 고차요인 구조를 전반적으로 지지하였으며, 한국 성인 표본의 자료 특성을 반영하여 19번 문항의 요인 재배치와 잔차구조에 대한 제한적 수정이 이루어졌다. 본 척도는 사회적 상호작용 불안척도(K-SIAS)와 사회공포증 척도(K-SPS)와 상관을 보여 수렴 타당도가 확인되었다. 역학연구 우울척도 개정판(K-CESD-R)과의 상관을 통해 변별 타당도를 확인하였다. 내적 합치도와 검사-재검사 신뢰도 모두 양호하였다. ROC 분석 결과, 사회불안 관련 위험군을 구분하는 데 양호한 분류 성능을 보였으며, 최적 기준점은 42.5점으로 나타났다. 이러한 결과는 본 척도가 사회불안 관련 위험군의 선별에 활용될 가능성을 시사한다. 본 연구는 사회불안에서 평가에 대한 두려움의 다차원적 구조를 경험적으로 확인하고, 이를 측정하기 위한 척도의 타당화 근거를 제시하였다는 점에서 의의가 있다.

The Fear of Positive Evaluation Scale (FPES) and the Brief Fear of Negative Evaluation Scale (BFNE) as instruments for measuring the core features of social anxiety disorder, have demonstrated reliability and validity through previous studies. However, these two scales have a limitation in that they are administered separately, making it difficult to comprehensively account for the fear of evaluation experienced by individuals with social anxiety. Accordingly, this study aimed to validate the Bivalent Fear of Evaluation Scale (BFOES) in a sample of Korean adults. Confirmatory factor analysis (CFA) was conducted with 400 adults aged 20-59 to examine the reliability and validity of the scale. The results supported a higher-order three-factor structure consistent with the original scale, comprising Fear of Positive Evaluation (FPE), Fear of Negative Evaluation (FNE), and Inter-valent Fear of Evaluation (IVFE), and demonstrated an adequate model fit. Convergent validity was established through significant correlations with the Social Interaction Anxiety Scale (SIAS) and the Social Phobia Scale (SPS). Discriminant validity was supported by correlations with the Korean version of Center for Epidemiologic Studies Depression Scale-Revised (K-CESD-R). Both Cronbach’s  and test-retest reliability coefficients indicated good internal consistency and temporal stability. Furthermore, Receiver operating Characteristic Analysis revealed that the optimal cutoff score was 42.5, at which the sum of sensitivity and specificity was maximized. Therefore, this scale is expected to be a useful tool for screening individuals at risk for social anxiety. This study provides empirical support for the multidimensional structure of fear of evaluation in social anxiety and demonstrates the sound psychometric properties of the K-BFOES.

본 연구의 목적은 수용전념치료에 기반하여 심리적 유연성의 잠재집단을 유형화하고 그 유형에 따른 사회불안과 우울의 차이 및 잠재집단 분류의 예측요인을 탐색하는 것이었다. 이를 위해 대학생 287명의 자료로 잠재프로파일 분석을 수행하였다. 연구결과는 다음과 같다. 첫 째, 대학생의 심리적 유연성 유형은 경직형, 평균형, 유연형 3개의 유형으로 분류되었다. 둘째, 심리적 유연성 유형에 따른 사회불안과 우울의 차이를 분석한 결과 두 변인 모두 경직형, 평균형, 유연형 순으로 높게 나타났다. 셋째, 성별과 학년이 대학생의 심리적 유연성 유형에 미치는 영향을 분석하였다. 그 결과, 성별은 여학생이 남학생에 비해 유연형보다 경직형에 속할 가능성이 약 4.5배 높았고, 평균형에 속할 가능성은 약 2.6배 높게 나타났다. 이와 달리 학년은 모든 유형 간 분류 확률에 대해 통계적으로 유의한 영향을 미치지 않는 것으로 나타났 다. 본 연구를 통해 심리적 유연성의 하위 구성개념을 보다 명확하게 이해하고, 대학생의 심리적 유연성 유형별 특성을 구체적으로 확인할 수 있었다. 또한 각 유형의 특성을 고려한 차별화된 정신건강 상담 개입의 적용 가능성을 제시하였으며, 이를 바탕으로 대학상담 현장에 서의 시사점과 후속 연구의 방향성을 논의하였다.

This study aimed to identify latent profiles of psychological flexibility based on Acceptance and Commitment Therapy and to examine differences in social anxiety and depression across profiles and predictors of profile membership. Latent profile analysis was conducted using data from 287 university students. Three profiles were identified: inflexible, average, and flexible. Social anxiety and depression were highest in the inflexible profile, followed by the average and flexible profiles. Gender significantly predicted profile membership: female students were approximately 4.5 times more likely than male students to belong to the inflexible rather than the flexible profile and 2.6 times more likely to belong to the average rather than the flexible profile. Academic year did not significantly predict profile membership. These findings clarify the subconstructs of psychological flexibility and the characteristics of each profile and suggest the applicability of differentiated mental health counseling interventions tailored to each profile. Implications for university counseling practice and future research are discussed.

본 연구는 ADHD 증상과 우울을 보이는 대학(원)생을 대상으로 우울을 개선하는 데 있어서 행동활성화 집단 프로그램의 효과성을 예비적으로 검증하고자 수행되었다. Conners 성인 ADHD 평정척도 단축형(K-CAARS-26)에서 하나 이상의 하위 척도 T점수 60점 이상이면서 환자건강설문지(PHQ-9)에서 총점 5점 이상(가벼운 우울 이상)에 해당하는 만 18세 이상 35세 미만의 대학(원)생 19명을 선발하였으며, 이 중 3명이 2회기 이내에 중도 탈락하여 최종 16명(남 8명, 여 8명)의 데이터를 분석하였다. 행동활성화 프로그램은 주 1회, 회기당 70분씩 총 8회 실시되었다. 본 연구의 주요 결과는 다음과 같다. 첫째, 프로그램 참여 후 참여자의 우울 증상(Cohen’s d = 1.56), ADHD 증상(Cohen’s d = 0.90), 목표중심 자기조절(Cohen’s d = 1.05) 및 행동활성화 수준(Cohen’s d = 0.73)에서 통계적으로 유의한 개선이 나타났다. 둘째, 프로그램 참여 후 우울 변화량은 ADHD 증상 변화량과 유의한 정적 상관을 보였다. 셋째, 프로그램 참여 후 PHQ-9로 측정한 우울이 비임상 수준으로 이동한 비율을 분석한 결과, 전체 참여자의 37.5%가 비임상 수준으로 이동하였다(경도 우울군 60%, 중등도 우울군 33.33%, 중고도 우울군 25%, 고도우울군 0%). 또한 K-CAARS-26로 측정한 ADHD증상이 정상 범위로 이동한 참여자는 전체의 50.0%였다(고위험군 11.11%, 저위험군 100%). 넷째, ADHD 증상에 따른 고위험군과 저위험군 집단별 본 프로그램의 치료 효과성의 차이를 검증한 결과, ADHD 저위험군이 고위험군에 비해 BA 치료전후 ADHD와 우울 증상의 감소 폭이 상대적으로 큰 것으로 나타났다. 이러한 결과는 행동활성화가 ADHD 증상과 우울을 보이는 대학(원)생의 우울 개선에 활용될 가능성을 보여준다. 또한 행동활성화의 구체적인 행동 실행과 활동 계획, 강화 경험의 축적이 ADHD 증상 및 목표중심 자기조절과 관련된 어려움의 개선에 기여할 가능성도 더불어 시사한다. 본 연구는 ADHD 증상을 보이는 대학(원)생을 대상으로 행동활성화의 적용 가능성을 탐색한 예비연구라는 점에서 의의가 있으며, 이러한 연구 결과를 바탕으로 임상적 함의와 향후 ADHD 증상을 보이는 성인을 대상으로 한 행동활성화 개입 연구의 방향을 논의하였다.

This study aimed to preliminarily examine the effectiveness of a group behavioral activation program for university and graduate students showing ADHD and depressive symptoms. Nineteen university and graduate students aged 18–34 who showed ADHD and depressive symptoms were selected based on a score of ≥60 on at least one subscale of the K-CAARS-26 and a total PHQ-9 score of ≥5. Three participants dropped out within the first two sessions, resulting in a final sample of 16 (8 male, 8 female) for analysis. The group received a behavioral activation program once a week for 70 minutes per session for 8 sessions. The main findings are as follows. First, participants showed statistically significant improvements in depressive symptoms (Cohen’s d = 1.56), ADHD symptoms (Cohen’s d = 0.90), goal-focused self-regulation (Cohen’s d = 1.05), and behavioral activation levels (Cohen’s d = 0.73) post-treatment compared to pre-treatment. Notably, ADHD symptom scores also decreased significantly following the BA program, suggesting the potential applicability of BA to behavioral difficulties associated with ADHD symptoms. Second, changes in depression scores were significantly and positively correlated with changes in ADHD symptom scores (rs = .64, p < .01). Third, based on the PHQ-9 cutoff score, 37.50% (6/16) of the participants moved to the non-clinical range after the program. Specifically, 60.00% of the mild group (5–9 points), 33.33% of the moderate group (10–14 points), and 25.00% of the moderately severe group (15–19 points) moved to the non-clinical range, whereas none of the participants in the severe group (20 points or higher; n = 1) did so. For ADHD symptoms, 50.00% (8/16) of the participants moved to the normal range based on the K-CAARS-26 criteria. Specifically, all participants in the low-risk group (100%) moved to the normal range, compared with 11.11% of those in the high-risk group. Fourth, subgroup analyses showed significant group-by-time interaction effects for depressive and ADHD symptom scores, with greater reductions observed in the low ADHD risk group than in the high-risk group. These findings preliminarily suggest the potential applicability of behavioral activation for university and graduate students showing ADHD and depressive symptoms. Clinical implications and future directions are discussed.
