ADHD 증상과 우울을 보이는 대학(원)생을 위한 행동활성화 집단 프로그램의 효과성 예비연구
A Preliminary Study of a Group Behavioral Activation Program for University and Graduate Students with ADHD Symptoms and Depressive Symptoms
This study aimed to preliminarily examine the effectiveness of a group behavioral activation program for university and graduate students showing ADHD and depressive symptoms. Nineteen university and graduate students aged 18–34 who showed ADHD and depressive symptoms were selected based on a score of ≥60 on at least one subscale of the K-CAARS-26 and a total PHQ-9 score of ≥5. Three participants dropped out within the first two sessions, resulting in a final sample of 16 (8 male, 8 female) for analysis. The group received a behavioral activation program once a week for 70 minutes per session for 8 sessions. The main findings are as follows. First, participants showed statistically significant improvements in depressive symptoms (Cohen’s d = 1.56), ADHD symptoms (Cohen’s d = 0.90), goal-focused self-regulation (Cohen’s d = 1.05), and behavioral activation levels (Cohen’s d = 0.73) post-treatment compared to pre-treatment. Notably, ADHD symptom scores also decreased significantly following the BA program, suggesting the potential applicability of BA to behavioral difficulties associated with ADHD symptoms. Second, changes in depression scores were significantly and positively correlated with changes in ADHD symptom scores (rs = .64, p < .01). Third, based on the PHQ-9 cutoff score, 37.50% (6/16) of the participants moved to the non-clinical range after the program. Specifically, 60.00% of the mild group (5–9 points), 33.33% of the moderate group (10–14 points), and 25.00% of the moderately severe group (15–19 points) moved to the non-clinical range, whereas none of the participants in the severe group (20 points or higher; n = 1) did so. For ADHD symptoms, 50.00% (8/16) of the participants moved to the normal range based on the K-CAARS-26 criteria. Specifically, all participants in the low-risk group (100%) moved to the normal range, compared with 11.11% of those in the high-risk group. Fourth, subgroup analyses showed significant group-by-time interaction effects for depressive and ADHD symptom scores, with greater reductions observed in the low ADHD risk group than in the high-risk group. These findings preliminarily suggest the potential applicability of behavioral activation for university and graduate students showing ADHD and depressive symptoms. Clinical implications and future directions are discussed.

본 연구는 ADHD 증상과 우울을 보이는 대학(원)생을 대상으로 우울을 개선하는 데 있어서 행동활성화 집단 프로그램의 효과성을 예비적으로 검증하고자 수행되었다. Conners 성인 ADHD 평정척도 단축형(K-CAARS-26)에서 하나 이상의 하위 척도 T점수 60점 이상이면서 환자건강설문지(PHQ-9)에서 총점 5점 이상(가벼운 우울 이상)에 해당하는 만 18세 이상 35세 미만의 대학(원)생 19명을 선발하였으며, 이 중 3명이 2회기 이내에 중도 탈락하여 최종 16명(남 8명, 여 8명)의 데이터를 분석하였다. 행동활성화 프로그램은 주 1회, 회기당 70분씩 총 8회 실시되었다. 본 연구의 주요 결과는 다음과 같다. 첫째, 프로그램 참여 후 참여자의 우울 증상(Cohen’s d = 1.56), ADHD 증상(Cohen’s d = 0.90), 목표중심 자기조절(Cohen’s d = 1.05) 및 행동활성화 수준(Cohen’s d = 0.73)에서 통계적으로 유의한 개선이 나타났다. 둘째, 프로그램 참여 후 우울 변화량은 ADHD 증상 변화량과 유의한 정적 상관을 보였다. 셋째, 프로그램 참여 후 PHQ-9로 측정한 우울이 비임상 수준으로 이동한 비율을 분석한 결과, 전체 참여자의 37.5%가 비임상 수준으로 이동하였다(경도 우울군 60%, 중등도 우울군 33.33%, 중고도 우울군 25%, 고도우울군 0%). 또한 K-CAARS-26로 측정한 ADHD증상이 정상 범위로 이동한 참여자는 전체의 50.0%였다(고위험군 11.11%, 저위험군 100%). 넷째, ADHD 증상에 따른 고위험군과 저위험군 집단별 본 프로그램의 치료 효과성의 차이를 검증한 결과, ADHD 저위험군이 고위험군에 비해 BA 치료전후 ADHD와 우울 증상의 감소 폭이 상대적으로 큰 것으로 나타났다. 이러한 결과는 행동활성화가 ADHD 증상과 우울을 보이는 대학(원)생의 우울 개선에 활용될 가능성을 보여준다. 또한 행동활성화의 구체적인 행동 실행과 활동 계획, 강화 경험의 축적이 ADHD 증상 및 목표중심 자기조절과 관련된 어려움의 개선에 기여할 가능성도 더불어 시사한다. 본 연구는 ADHD 증상을 보이는 대학(원)생을 대상으로 행동활성화의 적용 가능성을 탐색한 예비연구라는 점에서 의의가 있으며, 이러한 연구 결과를 바탕으로 임상적 함의와 향후 ADHD 증상을 보이는 성인을 대상으로 한 행동활성화 개입 연구의 방향을 논의하였다.
