In order to evaluate the factors related to therapeutic compliance of hypertensive patients in small and medium scale industry, the questionnaire survey and the blood pressure measurement were performed on 138 hypertensive patients who were reported to have C or D2 result of hypertension at the workers' periodic health examination from March to November 2001. The contents of questionnaire included the informations of factors related to therapeutic compliance of hypertensive patients such as, age, sex, marital status, income, education levels, scale, occupation, work duration, smoking and drinking habits, exercise, family history, stress and A type personality, employer's concern, organizational culture, health status, awareness, knowledge and attitude toward the needs of hypertension treatment, and experience of health education. The results of the study were as follows: 1. The proportion of those who were compliant to the treatment of hypertension was 41.3% of subjects. Among small scale industry less than 50 employees, the rate of therapeutic compliance group was 27.5% and therapeutic noncompliance group was 72.5%. Among medium scale industry more than 50 employees, the rate of therapeutic compliance group was 60.3% and therapeutic noncompliance group was 39.7%. 2. Among therapeutic noncompliance group, 95.1% of patients were aware of the fact that workers have hypertension, and 77.8% of patients got 6-10 marks of hypertension related knowledge. For health education, 27.2% were experienced and 34.6% said no intention to participate. And for hypertension treatment, 9.9% said no need to get the treatment and 44.4% said have no idea whether get treatment or not. 3. The significant factors related to therapeutic compliance of hypertensive patients in small scale industry were work duration, A type personality(anger), health status, attitude toward the needs of hypertension treatment, and experience of health education. 4. The significant factors related to therapeutic compliance of hypertensive patients in medium scale industry were age, occupation, subjective symptom, attitude toward the needs of hypertension treatment, and experience of health education. In consideration of above findings, it was suggested that in order to improve the therapeutic compliance in small and medium scale industry hypertensives, it be necessary to change attitude, perception, knowledge and treatment of hypertension by various methods such as effective health education and individually consulting programs by occupational health professional.

This study analyzed the managerial efficiency of 11 organizations, the branch centers of a occupational health service organization in Korea, using the Data Envelopment Analysis (DEA) method. The DEA is a good method for evaluating health services since it can handle multiple inputs and outputs simultaneously, and also identify the sources and amount of inefficiency. The author approached this study using two efficient models: the monetary value model and the real value model. The DEA method based on the monetary value model included cost factors, while the real value model excluded cost factors. The input variables used were manpower of physicians, medical technicians, nurses, industrial hygienists and administrators; labor, maintenance, and material expenses. The output variables used were the number of medical examinations, workplace evaluations, group health management services and income from each service. The major results were as follows: First, in the monetary value model, 6 out of 11 organizations (54.6%) showed an efficiency score of 1.0, which means that they have been operating in very efficient ways. However, 5 organizations (46.4%) showed themselves to be relatively inefficient. Second, in the real value model, 7 out of 11 organizations (63.4%) showed an efficiency score of 1.0, which means they have been operating efficiently, while 4 organizations (46.4%) showed themselves to be relatively inefficient. Third, the reliability of DEA method were analyzed by comparing the results of the monetary value model and real value model. The results of 8 out of 11 organizations were same in terms of being efficient or not. Thus, the DEA could be a valid application method for occupational health service organizations. Fourth, the organizations that displayed common inefficiency in both the monetary value model and in the real value model 3, 9, and 10, were also considered to be managed inefficiency from expertise opinion. In summary, this study evaluated the efficiency of occupational health service organizations applying the DEA method with different variables, and found that the results of analysis could be valid in terms of both modeling and expert sense. In the future, the DEA method will be used as a useful tool to identify and evaluate the efficiency of occupational health service organizations through more applications and refinements.
중환자실 간호사의 작업자세에 따른 신체부담도에 관한 연구

Objective: The purpose of this study was to determine the physical load by identifying harmfully working postures and to develop recommendations for improving the existing situation with nurses in ICU, thereby to provide the basis for development of work-related musculoskeletal preventive program. Method: Various types of tasks were recorded with a video camera to chart and analyze different postures by OWAS(Ovako Working Posture Analysing System). Collected data showed that poor postures were adopted, not only for lifting or repositoning a patient, but also for other tasks. Data Analysis: The performed activities were then divided into Nursing Intervention Classification. Altogether 128 postures were selected for analysis. Then they were classified into different OAC (OWAS Action Categories). From all the observation, unhealthy postures, for which corrective measures had to be considered immediately (i.e., 75% classified as OACⅡ+III+Ⅳ) were found. Collected data were analyzed in terms of percentage, 2-tail Mann-Whitney U test. Result: Poor postures mainly occur during 'positioning the patient' and 'airway suctioning' in NIC. No difference was found (p=0.060) between the percentage of harmful posture adopted during the patient handling tasks and non-patient handling tasks. Conclusion: This study shows, that in the nursing profession with ICU not only occur during patient handling, but also during other activities. The OWAS method was useful in determining the physical load by locating potential activities due to harmfully working postures, providing a detailed description with analysis, and suggesting successful means to reduce postural load.

Purpose: This study was conducted to provide the fadamental information to prepare better Occupational Health Program for SSI based on the evaluation of the experience of nurse who were working in the Government-funded subsidized occupational health program for SSI. Method: The focus group method was used. Data was collected using focus group interviews and analyzed in the framework of the Grounded theory method as mapped by Strauss and Corbin(1990). The subjects were 14 nurses. Result: We identified 60 concepts, 30sub-categories and 6 categories(Table 1). Categories were Various services, Difficulties in implement Services, Successful strategies, Program Evaluation, Alternative Plans and Adaptation to new field. Conclusion: Nurses evaluated the effect of this program that autonomous health management ability has not changed although the possibility for the small scale industries to start the health management arose by acknowledging the importance of the health management of the workplace. In spite of many difficulties in implement services, nurses provided various health services using successful strategies. They suggest that various programs such as a model set-up suitable for the quality of workplaces and a role model set-up of the occupational health nurse for SSI as well as various efforts for the activated this program were needed. Nurses had a hard time in unfamiliar field when they began this program and overcame this difficulties with various strategies.

The purpose of this study is to analyze papers of smoking cessation program into method and effect 13 papers that purblished from January, 1980 to Oct., 2001 in the country and 30 papers that purblished from Jan., 1980 to Jun., 1999 in other countries were analized. The result of this study can be summarized as follows : 1) The most of subjects were students(46.5%) and residents(30.2%). 2) There were a few of papers based on the Theory(9.3%). 3) Programs included education(58.3%), reward (13.9%) and support(12.5%). 4) There were experimental design(46.5%), non-experimental design(29.5%) and experimental design(25.0%). The most of papers of non-experimental desigs were significant. 5) Behavior(48.7%), knowledge(15.4%) and attitude(12.8%) were evaluated as a effect of program. Knowledge had higher significant rate than other evaluation items. 6) Programs that was from a year to two years had the highest significant rate.

This study aimed to develop hypertension control programs and to analyse their effects in small scale enterprises. The program was self-efficacy promoting program for worker with hypertension. The target population of the programs are 67 workers. The program was conducted for 18 weeks after a pre- intervention survey. Data were analyzed by the SAS program with paired t-test. The results of the study are as follows; First, after the program, scores on self-efficacy were increased significantly. Second, after the program, life-style change was increased significantly. Third, the blood pressure level was diminished in the program. The workers have lost systolic blood pressure by 18mmHg and diastolic blood pressure by 13mmHg. In conclusion, the self-efficacy promoting program is an effective intervention to lower the blood pressure. Thus this program can be recommended as an intervention program of hypertension in small scale enterprises.
