소뇌 출혈 환자에 대한 ICF Tool을 적용한 중재전략의 증례

Purpose : The purpose of this study was to describe the Intervention strategy applied ICF Tool about patient with cerebellar hemorrhage. Methods : The data was collected by patient with cerebellar hemorrhage. We applied the ICF Tool for patient with cerebellar hemorrhage. Parameters of result were collected for using the Timed up and go test, Berg balance scale and ICF assessment sheet. Results : Significant differences were observed the patient for Timed up and go test, Berg balance scale and ICF assessment sheet. patient with cerebellar hemorrhage improved all test. Conclusion : ICF Tool applied Intervention about patient with cerebellar hemorrhage is very useful and effective. It is effective in clinical practice.

Purpose : Study on the effects of attention(working memory) for normal adults by applying transcranial Direct Current Stimulation(tDCS). Methods : There are two groups of 10 random people aged between 20 to 30, one is an experimental group(stimulation) and the other is a comparison group(simulate stimulation). For the 1st da y, pre-stimulation test is executed, and for the 2nd day to the 4th day, tDCS is applied stimulated both on the right prefrontal lobe and the left occipital lobe through the electrode. The stimulation lasts for 15 minutes with voltage power of 1mA for each day. The post-stimulation test is executed on the day of 5th. In this paper, a FAIR attention test is used for measuring the attention. Results : As the result of the experiment, there are significant gaps both on the value of P, the ability of selective attention, and the value of C, the consistent ability after the stimulation on right prefrontal and left occipital lobe. And there is no significant gap on the value Q, the quality(Control). However, all the P, Q, and C values are increased according to the average value between the pre-stimulation test and the post-stimulation. Conclusion : Finally, ability of selective attention and self-control, and consistent attention is progressed by applying the tDCS to the normal people.
마비측에 적용한 외측 쐐기 깔창이 뇌졸증 환자의 체중부하율과 균형, 보행에 미치는 영향

Purpose : The study was to evaluate the weight distribution, balance and gait function of stroke patients wearing lateral wedged insole to the shoe of the affected side. Methods : 27 patients with stroke (15 men, 12 women) participated in this study. Participants performed weight distribution, dynamic balance and gait ability with or without wedged insole on affected side in a random order. The balancia was used to evaluate the weight distribution. Deviation from the center line was analyzed by Dartfish during sit to stand to evaluate dynamic balance. The functional walk ability evaluated by 10 m walking velocity. Results : The asymmetry index of weight bearing improved significantly with wedged insole of affected side(p<.05). During sit to stand, center of gravity significantly moved from non-affected side to more mid line of body(p<.05). Improvement were shown in walking speed after wearing the wedged insole(p<.05). Conclusion : Wedged insole applied on affected side have a beneficial effect on weight distribution, dynamic balance and walking speed with stroke.

Purpose : The purpose of this study was to measure the center of mass in body with stroke patients using a tri-axial accelerometer during walking. Methods : Twenty-eight patients were recruited and divided into two groups for this study. To measure their walking ability, Timed Up & Go (TUG) test and Fucntioanl Gait Assessment (FGA) were conducted and acceleration at rotation of center of mass (COM) in body were measure for each group. Results : In the comparisons between the two groups, the TUG and FGA were not significant differences and acceleration at rotation of COM was not significant differences also. Conclusion : Our research results suggesting that the accelerometer may be used as a testing tool and ongoing assessment tool for stroke patients during effects of intervention in walking.
PNF를 이용한 과제지향훈련이 만성뇌졸중환자의 상지기능과 일상생활수행능력에 미치는 영향

Purpose : The aim of this study was to compare the effect of task-oriented training using the PNF(proprioceptive neuromuscular facilitation) and task-oriented training with chronic stroke patients. Methods : Sixteen chronic stroke patients participated. Participants were randomly assigned to the control and experimental group( 8 experimental, 8 control). All of participants were in-patients at local hospital and had been received training appropriate to the purpose of each group program. Experimental period was carried out 30 minutes/ day, five days/week, during four weeks. The experimental group trained the task-oriented training using the PNF and control group trained the task-oriented training. Fugl-meyer assessment scale and Modified Barthel Index was measured to compare the upper arm function and activities of daily living. We were measured before and after the training. Results : The results of the study were as follow: Fugl-meyer assessment scale was significantly increased both groups(p<.05), and significant between groups(p<.05). Modified Barthel index was significantly increased both groups(p<.05) and between groups(p<.05). Conclusion : PNF can be effective in improving upper arm function and ability to perform daily life of chronic stroke patients.

Purpose : The purpose of this study was compared for rounded scapular between strechting on pectoralis minoor and strengthening on scapular stabilizer muscles. Methods : The main laboratory had an experiment of the exercise for rounded shoulder posture with 30 patients on a pectoralis minor stretching and scapula to stabilize the elastic band to divide into two groups in this study which were conducted. Results : Acromial height measured from changes of the rounded shoulder posture changes before and after workouts; stretching before exercise in the exercise group is from 94.19±3.47 to after result of 94.84±3.19, and the after exercise of an elastic band pre-exercise in the exercise group is from 93.60±3.42 to 88.68±3.49, dividing into two groups, which increased significantly. Compared to an acromial height variation results before and after workouts; stretching exercise group made 3.07±1.43, 4.91±2.24 and elastic band exercise group, 4.91±2.24, which shows the difference between the two groups and was not statistically significant. Conclusion : Against a pectoralis minor that cause the findings of this study, RSP proximately shows serratus anterior and the strengthening of trapezius by exercises of the shoulder peaks which decrease in height arbitration scheme of a pectoralis minor stretching effect as shown. Therefore, it is expected RSP patients in clinical intervention methods combined with stretching exercises strengthens muscles against an elastic band exercise effective.

Purpose : This purpose of this study was to investigate the effects of elastic band resistance exercise program in the falls prevention of elderly women. Methods : The subjects of this study, 46 people elderly women who participated in the exercise program of Jeollanamdo Y elderly welfare center were chosen as subjects and use the elastic band resistance exercise program for 12 weeks. Results : There was a significant increase on the physical function and fall efficacy of all the subjects in the experimental group and the control one after the experiment. Furthermore, the experimental group showed more meaningful improvement than the control one. Conclusion : Falls Efficacy of the experimental group before the experiment, after the changes in difference was noted. More results from the 12 weeks of using the elastic band resistance exercise program to increase the efficacy of body functions (power of lower extremity, muscle endurance, flexibility, balance, and agility), and falls efficacy of elderly women is considered as an effective fall prevention exercise program.

Purpose : This study according to the angle at the shoulder joint abduction compare muscle activity by analyzing abduction in normal depending on the angle of the shoulder joint which muscles are activated exactly know what its purpose is. Methods : 15 students with a healthy shoulder abduction angles (45°, 90°) according to the trapezius (upper, midder, lower), infrasupinatus, deltoid, pectoralis major, serratus anterior, latissimus dorsi muscle activity of the were analyzed. How the% MVIC EMG activity of each muscle EMG signals were standardized. Results : The mean age of the study subjects 23.6 years old, and is a key 175.6Cm, weight 70.66Kg respectively. 45°non-load Pectoralis major, load Deltoid, 90°non-load Deltoid, Latissimus dorsi load showed the most activity. Conclusion : The purpose of this study the muscle activity of the muscles in order to mobilize the comparison of the active muscles, but the experimenter with a range of individual differences that every time I was able to find the average. Based on these results will be helpful in future studies.

Purpose : The purpose of this study is to investigate the effects school business hospital-based integrated health education on learning transfer factor and level. Methods : This study conducted a questionnaire survey of 60 students at D college using metastatic diagnostic tool who took the integrated health education curriculum, statistical analysis utilized the SPSS 17.0 for window version. Results : On comparison of the details 5 clauses, 29 questions using LTSI, this study found that the integrated health education based on the school business hospital is effective for learning transfer. Conclusion : What the integrated health education based on clinic practice system at D college to overcome the limitations of health and medical line is effective for learning transfer and it will be useful to cultivate professional.

Purpose : The purpose of this study was to investigate the effects of closed chain breathing exercise on thorax circumference increase and ROM of shoulder rotation for cervical vertebral cord injury patient's. Methods : 12 cervical vertebral cord injury patients volunteer to participate in this study. closed chain breathing exercise group of 4 subjects performed exercise three time a week. The deep breathing exercise group and general exercise group of 4 subjects performed exercise three time a week. We analyaed the descriptive statistics and 3 × 2 repeated measures ANOVA by SPSS 12.0 for window. Results : In comparison of rotation of shoulder ROM between pre and post value, the increase of rotation ROM of shoulder was significant in the closed chain breathing exercise group(p<.05). Conclusion : The closed chain breathing exercise helped to increased rotation ROM of shoulder.

Purpose : The purpose of this study was to investigate a plantar pressure distribution and the trajectory of the center of pressure on double limb support and single limb support according to level surface, air cushion, and aero step. Methods : 21 healthy adults participated in this study. The plantar pressure were assessed at three different surface conditions(on the level surface, air cushion, and aero step) on double limb support and single limb support. Testing orders were selected randomly. Results : Plantar pressure distribution show a significant difference contact area 1 and contact area 3 on double limb support and single limb support. The trajectory of the center of pressure show an significant difference anteroposterior(AP), mediolateral(ML), and total displacement on double limb support and single limb support. Conclusion : Through the use of soft surface as air cushion and aero step will be using the ankle strategy. This will be to strengthen the muscles around the ankle. Consequently, should help to improve stability and coordination.
체중지지 트레드밀 후방 보행 훈련이 불완전 척수 손상 환자의 보행과 호흡에 미치는 효과 : 단일사례연구

Purpose : The aim of study was to assess the effect of backward walking combined with body weight supported treadmill training for patients with spinal cord injury. Methods : Forced vital capacity(FVC), forced expiratory volume at one second(FEV1), peak expiratory flow(PEF) and FEV1/FVC ratio(FER) were measured with a spirometer for on subject of T12 spinal cord injury. 10 meter walking test(10MWT), timed up & go test(TUG) and 6-Minute walking test(6MWT) were the measured. Intervention consisted of backward walking combined with body weight supported treadmill training five times a week for 12 weeks. Results : The date of 10MWT, TUG, 6MWT were improved. Furthermore, the date of PEF, FEV1, FVC, FER showed a improvement. Conclusion : Backward walking combined with body weight supported treadmill training may be a better and more effective method for gait performance and respiratory on incomplete spinal cord injury.
발목관절 자세 변화에 대한 무릎관절 굽힘근과 폄근의 활성도

Purpose : The purpose of this study was to examine the activity of the knee flexor and extensor with ankle plantar flexion and dorsiflexion. Methods : A total of 18 subjects(Male 6 , female 12) performed 4 lower extremity patterns of PNF and the activities of the vastus medialis oblique, rectus femoris, vastus lateralis, biceps femoris, semitendinosus ipsilateral sides were measured using electromyography. Results : During 4 lower extremity patterns of PNF, knee flexor and extensor muscle activity were significantly difference. Knee extensors were shown to be higher knee extension and ankle dorsiflexion combined pattern. Knee flexors were shown to be higher knee flexion and ankle plantarflexion combined pattern. Conclusion : We suggest that it is efficient to strengthening of knee extensors with ankle dorsiflexion and to strengthening of knee flexors with ankle plantar flexion. Also, for the functioning as toe standing, we have to choice appropriate movement pattern

Purpose : The neurologically impaired patients have sensory, motor, cognition, perception problems which cause reduction of body schema, balance control and postural control provoke disfunction. The purpose of this case report was to evaluate effects on gait&balance upon improvement of postural control for left hemiplegia. Methods : This study has performed single subject design from March to April 2013 for 6 weeks. The subject of this study was a 67years old female patient with left hemiplegia. Timed Up and Go (TUG) test and Functional Reach (FR) test were used as evaluation tool. The subject was treated 5 times a week for 30 minutes each. Results : The result is that the walking velocity was decreased, the ability of gait was improved than before the training. The length of FR was increased from 8.33cm to 22.67cm. Conclusion : According to the results, the treatment improves subject's ability of walking in this study.

Purpose : The purpose of this study of normal elderly and stroke patients by comparing the dominant side and recessive side was to evaluate the differents between each Methods : Thirty-two elderly participated in this study. They were classified two groups: stroke patients group and the normal elderly group. Outcome measures were: general characteristics and BMD. General characteristics included age, height, weight and body mass index. BMD was represented in the osteoporosis index, T-score and Z-score. The measurements of bone mineral density were evaluated on the calcaneus region of the dominant and recessive side, using OsteoPro. The data analyzed using SPSS 17.0 software and the Paired-sample T-test and the Independent-sample T-test Results : OI, T-score, Z-score showed no significant differences between dominant side and recessive side in normal elderly group. But stroke patients group showed OI, T-score, Z-score significant differences between paretic side and nonparetic side. Change score subtracted recessive side from dominant side was significant differences between stroke patients group and normal elderly group. Conclusion : There is positive relationship between physical activity and BMD in stroke patients. Therefore, improved physical activity can have a beneficial effect by reducing osteoporosis in stroke patients, considering a positive relationship between physical activity and BMD.
