닫힌-사슬 운동이 뇌졸중 환자의 체중지지율과 균형능력에 미치는 영향

Purpose:The purpose of this study is to analyze the effect of closed-chain exercise on weight supporting rate change within hemiplegic side and static · dynamic balance ability in stroke patients. Methods:The subjects of the study were 13 hemiplegic patients who carried out closed- kinematic chain exercise program over 6 weeks. The exercises of the program are stand to sit with stall bar, stair-up & down by a hemiplegic leg and bridging exercise crossing the non-hemiplegic leg onto the hemiplegic leg. Each exercise was carried out over 3 sets of 10reps. Results:The results of this study were summarized as follows: 1. After intervention, there was a statistically significant change in the weight supporting rate within hemiplegic and non-hemiplegic side(P<0.05). 2. After intervention, there was a statistically significant change in the static balance (FICSIT-4) ability(P<0.05). 3. After intervention, there was a statistically significant change in the dynamic balance (FSST, TUG, FRT) ability(P<0.05). Conclusion:The results of the study suggests that closed-chain exercise program in stroke patients improves their weight supporting rate and enhance the static · dynamic balance.
보강적 피드백의 형태가 편마비 성인의 운동학습에 미치는 영향

The purpose of this study was to investigate whether it makes difference to use of the Augmented Feedback Type(Verbal Feedback, VTR Feedback and Verbal & VTR Feedback) to hemiplegic adults on learning of motor skill. For the purpose 15 hemiplegic adults who are received rehabilitation program at H hospital in Hong-Sung, Choong-Nam Province. Subjects were classified into three groups by random assignment; the Verbal Feedback group, the VTR Feedback group and Verbal & VTR Feedback group. Each groups received 5 subjects from hemiplegic adults. Subjects were tested by Timed Up and Go test for 9 weeks. And to find out the improvement measured by Pre- Test, Acquisition Test and Retention Test. To find out the improvement of each group's measures took average and standard deviation. To probate the significance of difference between the improvement conducted the one-way ANOVA and to probate the significance of difference of Acquisition Test and Retention test conducted paired t-test. The results of this study were as follows; First, All of Augmented Feedback Types had a positive effect on hemiplegic adults to learning of motor skill. Second, The Verbal Feedback group and the VTR Feedback group had no significantly difference at Acquisition Test, But They had the most improvement at Retention Test. Third, In hemiplegic adults, the Verbal & VTR Feedback group had the highest Retention Effect.

Purpose:The purpose of this study was to assess the effects of the lower extremity muscle activity on bridging exercise according to the knee joint angle. Methods:Twenty-five healthy adults volunteered to participate in this study. Subjects were required to complete following four bridging exercises; knee joint flexion 120°, 90°, 60°, 45°. Surface electromyography from selected lower extremity muscles was normalized to maximum voluntary isometric contraction. Muscle activity was measured by QEMG-4 system (LXM 3204, Laxtha Korea). A repeated measures of one-way ANOVA was used to determine the influence of bridging exercise on muscle activity for each muscle and descriptive statistics was used to determine muscle ratio. Results:The biceps femoris of all bridging exercises showed significantly(p<.05). The vastus medialis and lateralis of all bridging exercises showed significant excepted 120°(p<.05). The rectus femoris of all bridging exercises showed no significant. Median of vastus medialis/rectus femoris ratio of 120° was 2.03, 90° was 2.16, 60° was 2.67, 45° was 4.10. Median of vastus lateralis/rectus femoris ratio of 120° was 1.70, 90° was 1.70, 60° was 2.08, 45° was 2.58. Median of vastus medialis/vastus lateralis ratio of 120° was 1.26, 90° was 1.50, 60° was 1.52, 45° was 1.47. Conclusion:Angular motion decreasing with knee joint flexion made increase biceps femoris and vastus medialis activation. This result will be use knee joint stabilizing exercises during bridging or unstable surface training and biceps femoris strength training.

Purpose:Sit-to-stand (STS) is one of the important activities of daily living (ADL) and each of its parameters is used frequently. This study aimed to examine the influence of different seat height on performance time and floor reaction force strength during an STS movement. Methods:Fifteen young-adult male subjects participated in this study. The subjects were divided into three groups based on lower leg length. Subjects performed an STS movement twice from chairs at height adjusted by the lower leg length of each subject. To examine the influence of the chair seat height, ground reaction forces during a STS performed with 3 chair heights adjusted to each subjects lower leg length were compared. Results:Vertical ground reaction force and time during an STS movement were measured to evaluate. Parameters regarding ground reaction force were selected for analyses. Significant differences were found in ground reaction force at G1 was greater than G2 and G3. Conclusion:The STS movement achievement strategy differed since chair seat height changes relatively by the difference in lower leg length. When conducting the ability to achieve STS movement rating test, chair seat height considering each subject’s lower leg length may be needed.

Purpose:The purpose of this article was to review the literature on change of knee position sense after total knee replacement intervention in knee rehabilitation. Methods:This review outlines scientific findings regarding different literature data in knee rehabilitation, and discusses proprioceptive change of knee joints after total knee replacement intervention. Result:Severe pain and diminished joint sensation may precipitate degenerative changes of knees joint, and a strong association between decreased proprioception and function has been identified in elderly patients with knee osteoarthritis. Total knee replacement(TKR) of the knee joint are effective surgical procedures in osteoarthritis patients, resulting in decreased pain, and improved physical function and quality of life in patients. After total knee replacement, joint position sensation is partially recovered, which may improve functional stability of the knee, but aspects of physical functioning are not fully restored to preoperative status. Conclusions:Thus, recovery rate in proprioception after TKR also is considered to be important because it may be a significant risk factor in failure of total knee replacement and knee rehabilitation intervention.

Purpose:The purpose of this study was to investigate muscle activity of affected lower extremity by unaffected lower extremity exercise and tried to examine muscle activity of affected lower extremity of hemiplegic patients were caused by stroke according to wearing single strap hemisling or non-wearing. Methods:We measured muscle activity of affected lower extremity when wearing single strap hemisling to affected lower extremity or non-wearing according to unaffected lower extremity used MP 150 Eletromyogram. Results:1) Muscle activity of gluteus medius muscle was the highest D1 before wearing single strap hemisling. 2) Muscle activity of hamstring muscle was the highest D3 after wearing single strap hemisling. 3) Muscle activity of quadriceps muscle was the highest D5 after wearing single strap hemisling. 4) Muscle activity of tibialis anterior muscle was the highest D1 after wearing single strap hemisling. 5) Muscle activity of soleus muscle was the highest D2 before wearing single strap hemisling. Conclusion:Wearing single strap hemisling of hemiplegic patients had effect on muscle activity of gluteus medius but didn't effect gluteus medius, hamstring, quadriceps, tibialis anterior muscle and soleus muscle.

Purpose:This study was to investigate the application of Rhythmic Initiation(RI) in the respect of information processing in motor learning. Methods:A computer-aided literature search was performed in PubMed and adapted to the other databases and the others were in published books. The following keywords were used: Rhythmic Initiation, attention, memory, motor accuracy, feedback, motor learning, motor control, PNF, cognition. Results:The characterization of RI is rhythmic motion of limb or body through the desired range, starting with passive motion and progressing to active resisted movement. This study suggested that the relationship between of RI and motor learning through the respect of information processing, memory, attention and motor accuracy. Conclusion:Only Rhythmic Initiation, specifically focused on the effects of information processing approach, suggesting that RI can be positively influeced on sensory-perception, attention, memory, motor accuracy. however, it is unclear whether positive effects in the laboratory and field can be generalized to improve. In addition, sustainability of motor learning with RI remains uncertain.

Background:This study was to investigate effect of hot and cold water for the hand dexterity. Methods:A total of 37 subjects having hemiplegia(men=6, women=11) and normal(men=10, women=10) were participated in this study. After both hands was soaked in the small tube for 1 minute, we measured ‘making dots in circles(BOTMP)’, ‘lifting small objects(JHFT)’, ‘disk turning(MMDT)’ test. Results:In the case of normal's dominant hand, there were statistically significant differences in all tests after soaked in the hot water but there were statistically significant differences in ‘disk turning’ test after soaked in the cold water. In the case of hemiplegia's unaffected hand, there were statistically significant differences in ‘disk turning’ test after soaked in the hot water but there were statistically significant differences in ‘making dots in circles’, ‘ disk turning’ test after soaked in the cold water. In the case of hemiplegia's affected hand, there were statistically significant differences in ‘lifting small objects’, ‘disk turning’ test after soaked in the hot water but there were statistically significant differences in ‘making dots in circles’, ‘disk turning’ test after soaked in the cold water. Conclusion:The hot and cold water were affected in the hand dexterity. Especially, in the case of both normal and hemiplegia, there were statistically significant differences in ‘disk turning’ test after soaked in the cold and hot water. Therefore, We were founded that affected in gross movement than fine movement
