하지운동으로 구성된 가정운동프로그램이 시설거주 노인의 무릎근력에 미치는 영향

The purpose of this study was to investigate the effects of apply to home based exercise program that consisted of lower extremity exercise on isokinetic knee strength in elderly at nursing home. Thirty eight subjects were participated in current study and divided two groups that home exercise program group and control group (home exercise group 76.83±4.06 years, control group 78.82±3.00). The subject’s isokinetic knee strength was measured by Biodex isokinetic system 4 (Biodex medical. U.S.A) at pre-intervention, post-intervention. We measured muscle’s peak torque, endurance and ratio(H/Q ratio). Home exercise program group has receive lower extremity exercise program that consisted of 6 exercises for 5 weeks. Control group has no intervention. The home exercise program group has significant change of muscle’s peak torque and endurance after applying intervention. But muscle ratio has no significant change both groups. On basis of these results, we concluded that home based exercise program that consised of lower extremity exercise has improved isokinteic knee strength in elderly at nursing home.

The purpose of this study was to analyze the factors affecting the return to work, classified list of personal damage, and mild personal damage among workers who experienced an industrial accident. We conducted a stepwise regression analysis on the data of 2000 workers who experienced an accident to analyze the 1st Workers’ Compensation Insurance Panel of the Labor Welfare Research Institute. As a result, the financial status of individuals and their households, status of support from the affiliated company, status of working environment with the affiliated company, and status of negative impact on life due to industrial accident were related to the return to work. The variables of health and quality of life were strongly related to the classified list of personal damage. The compensation for industrial accident services was related to the degree of personal damage. Therefore, there are necessary control variables relating to finances, variables relating to working environment to improve the return to work. Additionally, it is important to improve the variables of health and quality of life to alleviate the classified list of personal damage. On the other hand, it is important to provide early rehabilitation services to reduce mild personal damage.

The purpose of this study was to accurately translate the Korean version of the American Physical Therapy Association's Professionalism in Physical Therapy: Core Values (PPTCV) self-assessment to assess the levels of professionalism among Korean registered physical therapists in an industrial accident hospital. The 68-item PPTCV self-assessment was translated, and the content review was performed by a translation panel. After content review, a bilingual physical therapy professor translated the document back into English. A semantic equivalence test was conducted with 5 American physical therapist professors. A survey study was conducted with a sample of 96 registered physical therapists in Korea to test the reliability of the translated PPTCV. The Cronbach's alpha test for the Korean version of the PPTCV was .882. The translated version could be used to assess levels of professionalism for other health professionals as well as physical therapists in an industrial accident hospital.

Purpose: The purpose of this study was to investigate the effect of muscle activity and balance when the women’s university students trained through bio-feedback. Methods: Fourteen healthy 20~30-year-old women participated in the study. Subjects were divided into two groups. The control group (n =7) did not undergo exercise and/or stimulation. The bio-feedback training group (n =7) underwent stimulation while looking at the screen using the bio-feedback balance system (3 times/day for 4 weeks). All tests were completed before and after the experiment. Muscle activity was measured using surface electromyography (Bagnoli EMG system, Delsys Inc. USA). Balance ability was measured by the GOOD BALANCE system (Metitur Ltd, Finland). Results: There were significant improvements in the muscle activity and balance ability tests for those in the bio-feedback training group (p <.05), while the control group showed no significant changes (p >.05). Conclusion: The above results revealed that bio-feedback training can improve the muscle activity and balance ability of functional ankle instability in women’s university students.

The purpose of this study was to investigate the new concept of aerobic exercises using an anti-gravity treadmill for rehabilitation after posterior lumbar internal fixation in elderly individuals. We investigated the case of a 77-year-old man who underwent an L2-4 posterior lumbar internal fixation. We assessed his isokinetic knee strength and low back pain status using the Oswestry disability index before and after the anti-gravity treadmill exercise program. The isokinetic knee strength progressively improved over time and recovered to its preoperative status. The Oswestry disability index also progressively improved. Aerobic exercises using an anti-gravity treadmill positively affect patients’ knee strength and ability to perform activities of daily living associated with back pain. Clinically, the new concept of aerobic exercises using an anti-gravity treadmill may be applied to patients after back surgeries for recovery of knee strength and ability to perform activities of daily living. However, more clinical studies are needed to support its application in patients after back surgeries.

Purpose: This study was to identify the change of the movement strategy according to restricted ankle motion during sit to stand movement by analyzing the biomechanics of the lower extremity. Methods: A total of eighteen subjects voluntarily participated in the experiment. We classified 18 subjects into 3 groups of 6 persons (NFR: non flexion restriction, DFR: dorsi flexion restriction, PFR: plantar flexion restriction) and measured the biomechanical factors of the lower limb during sit to stand movement. The ground reaction force was measured using a force plate, and the other factors were measured using an infrared camera. The non-parametric test was used to analyze the changes in the biomechanical factors of the lower limbs according to restricted ankle motion. Results: Biomechanical factors were statistically significant difference in ankle-restricted groups. In the DFR group, the maximum joint force, the displacement of the center of gravity and the maximum ground reaction force showed statistically significant differences, however the maximum flexion angle was not. In the PFR group, the maximum joint force, the displacement of the center of gravity showed statistically significant differences, however the maximum ground reaction force and maximum flexion angle was not. Conclusion: These results show that restriction of ankle dorsi flexion use the strategy using the force of the unaffected lower limb, and restriction of ankle plantar flexion use the strategy positioning the center of gravity within the base of support.

This study aimed to confirm the effects of an application of new conceptual high frequency treatment on blood pressure, the muscle tone of erector spine and calf muscles, and the quality of life in elderly individuals. The experiment included 20 elderly patients with another diagnosis and no surgical experience. All subjects underwent high frequency treatment for 20 minutes or 30 minutes. First, after the intervention, blood pressure was measured using the Oscillometric tonometer. Second, muscle tone of both the erector spine and calf muscles were measured using Myotonepro. Quality of life (SF-36) was measured using a questionnaire. As a result, blood pressure significantly changed after the intervention. However, muscle tone did not change. Quality of life significantly changed regarding physical function, role limitation-physical, role limitation-emotional, vitality, and bodily pain. Thus, we conclude that high frequency treatment can help to improve the health condition of the elderly population.

Purpose: The purpose of this study is to find the effects of Yoga exercise program on trunk stability and balance ability of normal adults. Methods: Organize 20s 20 participants into two groups, the experimental group had yoga exercise program, the contrast group had walking exercise program. For analysis, One leg standing test was used to measure balance ability and EMG in plank position and isometric trunk extension position was used to measure muscular activity of the rectus abdominis, external oblique and erector spinae. Result: Experimental balance ability and muscle activities were than the walking exercise program group considerably higher to yoga exercise program group. Conclusion: Results illustrate that yoga exercise program could be used as effective exercises for trunk stability and balance ability.

The aim of the study was to systematically review the effect of aquatic exercise on physical function among athletes. Aquatic exercise can be important role for physical function of human body. But, each evidence of aquatic exercise has difficult for understanding overall effects. Therefore, it is necessary to systematically review the evidence of aquatic exercise for better application in sports filed. The Pubmed, Scopus and Google scholar were systematically searched for relevant studies published between January 2006 and May 2018. The following search terms were used: “aqua”, “aquatic”, “athlete”, “water”, “exercise”, “sports”, “underwater” and “underwater exercise” Ten of the 945 retrieved articles met the 10 inclusion criteria. And, used for analysis effect of aquatic exercise on physical function. The results of this study were include analysis factor as follows: speed (7 papers), strength (3), heart rate, agility, flexibility (2), balance, performance, endurance, power, pain, anthropometric and physiological (1). There were improvement in aquatic exercise in all factors, but especially in heart rate, agility, endurance, and power. However, it showed lower effect than land-based exercise in the balance part. In this study, it is necessary to consider the clinical application for athletes through the effectiveness of each factor. However, there was a deviation in the number of articles on each factor, therefore it is need to various study to aquatic exercise for sports athlete.

Purpose: This study investigated the effects of balance exercise and foot orthotics on craniovertebral angle, foot pressure, and muscle activity in forward posture. Methods: Twenty-seven participants with forward head posture were included in this study and were randomly grouped into the control, exercise, orthotics, and exercise*orthotics groups based on the intervention provided. Craniovertebral angle, foot pressure, and muscle activity(sternocleidomastoid muscle & upper trapezius muscle) were measured using Image J, BioRescue, and electromyography, respectively. Results: A one-way analysis of variance showed that the craniovertebral angle increased in three groups compared with the control group. Left foot pressure was near perfect balance (50%) in the exercise and exercise*orthotics groups compared with the control group. Conclusion: Both orthotics and exercise led to an improvement in the craniovertebral angle. However, exercise more effectively improved static balance. Therefore, a combination of balance exercise and orthotics is expected to be more effective than only orthotics to correct forward head posture.

This study was to understand the effect of complex cognitive reinforcement training program for seniors with dementia at long term care hospitals and nursing facilities on the cognitive function of seniors with dementia. For 8 weeks from Apr. to May, 2017, 30 men and women having no disqualification affecting this study were selected from ○○ hospital and C center located in Busan, and the purpose of this study was understood and the written consent was submitted by all participants. The trainings composed of the cognitive rehabilitation program of G hospital and the cognitive stimulation activity program of C center were provided twice a week in 8 weeks for 30 minutes in each training. MMSE-KC, GDS, and MoCA-K were used to measure the overall cognitive domain. To investigate the effect of trainings on cognitive status of seniors with dementia, an initial assessment prior to the training and interim and final assessment were performed. First, in the comparison of cognitive function by a training period, the cognitive rehabilitation training group showed the bigger improvement in the cognitive assessment score(MMSE-KC), compared to the cognitive stimulation training group. Second, in the comparison of cognitive function by a training period, the cognitive rehabilitation training group showed the bigger difference in the efficacy in the cognitive assessment score (GDS), compared to the cognitive stimulation training group. Third, in the comparison of cognitive function by a training period, the cognitive rehabilitation training group showed the bigger difference in the efficacy in the cognitive assessment score (MoCA-K), compared to the cognitive stimulation training group. Based on the comprehensive result of this study, there was no difference by participants depending on the training methods that are the cognitive rehabilitation training and the cognitive stimulation training, and both groups showed positive effects on their cognitive function after trainings. Therefore, it will be considered more effective in the future to carry out two programs at the same time, though a different program is currently applied in each institution depending on its environment. Accordingly, it is hoped to conduct a future study based on more segmented method.

The purpose of this study was to demonstrat that the change of functional ratios for isokinetic hamstring : quadriceps strength by flexion angle and angular velocity change in hip joint. The subjects for this study were composed of 30 people who had no medical history of the lower extremity such as surgery of knee and ankle. For each hip position (50°, 70°, 90° of flexion), participants used the dominant leg to perform (1)3 maximal concentric knee flexion-extension at 60° per second, (2)3 maximal eccentric knee flexion-extension 60° per second. (3)3 maximal concentric knee flexion-extension at 180° per second and (4)3 maximal eccentric knee flexion-extension 180° per second. The results of this study, the flexion angle of the hip joints affected the ratio of the hamstring and quadriceps, and the ratio increased as the hip flexion angle increased. In addition, the increase in functional H:Q ratio showed a larger ratio, and the eccentric contraction of the hamstring was higher than the concentric contraction. Therefore, when measuring and exercising with regard to the stability of the knee joint and ligament injuries using isokinetic equipment, the bending angle of the hip joint is 90° and the angular velocity is 180°/s.

The purpose of this study was to analyze on Reliability, Validity and Effectiveness in published the literature regarding STarT Back Screening Tool for low back pain. We investigated by database of Medline, Embase, CINAHL, PsycINFO, AMED, Scopus, PubMed, and Web of Science. The One selected efficacy study of the STarT Back Screening Tool showed significant improvement in disability. And the 20 selected reliability and validity studies of the STarT Back Screening Tool version showed moderate significance, but some low significance in construct validity.

In the past, researchers evaluated walking speed and stride during a gait ability test. However, recently, there have been developed methods which evaluate exercise performance beyond result-based assessment. In the past, the individual ability was estimated according to walking index, but the trend has been gradually expanding the scope of researching boundary with questions on the process of walking. The purpose of this study is to explain the muscle synergies that occur during walking. The muscle synergy refers to a pattern in which various muscles present in the human body are simultaneously activated to achieve a given task. Currently, muscle synergy research is actively underway in the exercise regulation and the neurodynamic field. Theoretically, the number of cases of performing the same task due to the many joint freedom degrees in the human body varies greatly. From this point of view, it can be concluded that the simultaneous activity pattern of muscles can be varied by repeating the task for everyone. However, when the actual muscle activity pattern is analyzed, there is no significant difference between the individuals. Considering that there are few differences among individuals with very important patterns in the task performance process, a common strategy for achieving goals in the central nervous system may be a necessary pattern for the successful execution of specific behaviors. Through this common pattern, spatiotemporal data for muscle synergy can provide the theoretical base for gait training in rehabilitation.

Effect of Respiratory Muscle Training Posture on Respiratory Muscle Activation in Stroke Patients

Purpose: This study investigated the effects of respiratory muscle training posture on respiratory muscle activation in stroke patients and presented appropriate posture for the respiratory muscle training. Methods: Thirty-six stroke patients were assigned to either a neutral training posture (n = 13), habitual training posture (n = 12), or side bending training posture (n = 11) group. Respiratory muscle training was conducted using an inspiratory muscle trainer. Each group was asked to adopt either a neutral, habitual, or side-bending posture during respiratory muscle training, which was performed for 30 minutes per day, 10 sets, 3 times per week, for a total of 6 weeks. After each session, external intercostal muscles and sternocleidomastoid muscle activity were measured via surface electromyography. Results: The paretic or non-paretic side intercostal muscle differed significantly in the three groups, and the non-paretic side intercostal muscle was significantly decreased in the neutral training group. The non-paretic side sternocleidomastoid muscle differed significantly in the three groups, and the nonparetic side sternocleidomastoid muscle was significantly increased in the habitual training group. Conclusion: A neutral posture, where appropriate spine and trunk alignment is maintained, is a more effective posture for respiratory muscle training for improving respiratory function in stroke patients.

Purpose: The purpose of this study is to compare the change on pressure pain threshold and vertical jump when foam roll exercises and vibration foam roll exercises. Methods: Participants were selected as having pain in at least three areas of the lower limbs and recording at least 5 points on the VAS. A total of thirty participants were divided into two groups equally, randomized into a foam roll exercises(n =15) and a vibration foam roll exercises(n =15). Both groups performed exercises for 45secs/five times and rest time 15secs. Pressure pain threshold, and vertical jump were all measured. Data was analyzed using SPSS 21.0 and an repeated measure ANOVA was used to find statistical differences within and between the two groups. The statistical significance was determined at α = .05. Results: Both foam roll and vibration foam roll had significant changes on pressure pain threshold and vertical jump among to pre and after 24hrs, 48hrs time. However, there was a no significant differences between both groups. Conclusion: Although there was a slight advantage to doing vibrated foam roll exercise than foam roll exercise.
