Purpose : The purpose of this study was to investigate the current status of clinical practice and associated factors with satisfaction among students in the department of emergency medical service. Methods : The study subjects were 498 students who finished clinical practice course in 9 universities. General characteristics, current status of clinical practice, frequency of some medical procedure and practice satisfaction were collected using self-administered questionnaire. The association between clinical practice and satisfaction was tested by t-test, ANOVA and multiple regression analysis. Results : Of 498 students, 257 (51.6%) subjects were male and the mean age was 22.1±2.01. The overall satisfaction score of clinical practice was 96.76±18.04. Male students had higher satisfaction scores than that of female. The students who were satisfied with the emergency room practice wanted to work in hospital or fire station and they had a tendency to take paramedic as their lifelong career. The students preferred to practicing in the emergency department that hire the paramedic. Conclusion : Gender, grades, future jobs, occupational views, practical places, agency orientation, departments, and educators were associated with educational satisfaction. The practice program considering the results of this study will help to increase the educational satisfaction in students of emergency medical service.

Purpose : This study was designed to compare the easiness and speed of insertion of three supraglottic airway devices(SADs) in a manikin setting. Methods : Three different SADs - Laryngeal Mask Classic(cLMA), I-gel and Streamlined Liner of the Pharynx Airway(SLIPA) were applied. One hundred and nineteen paramedical students with(group H) or without (group L) previous airway experience were taught brief manikin training about the use of the cLMA, I-gel and SLIPA. The students inserted each device in a randomized order. Time to effective ventilation was recorded in seconds from holding the device to the first chest inflation. Success was determined as adequate chest wall movement. Results : The insertion attempts were lesser in I-gel(1.00±0.00) and SLIPA(1.05±0.27) than cLMA(1.16±0.41, p<.05). The shortest time to insertion was recorded for I-gel(10.5±3.0 sec), followed by the SLIPA(12.9±4.5 sec) and cLMA(19.6±4.1 sec, p<.05). There were no significant differences in the insertion attempts and insertion time of I-gel between group L and group H. But in cLMA, longer insertion time and more insertion attempts were recorded in group L than group H. Conclusion : Both I-gel and SLIPA were superior to cLMA in the easiness and speed of insertion. Even in novice students, I-gel showed an excellent result in a manikin.

Purpose : The purpose of the present study is to analyze the accuracy and fatigue felt by lay persons receiving CPR training when they perform hands only CPR (HOCPR) and traditional CPR (TCPR). The performance of CPR data will provide the criteria of dispatcher guidelines for the general public. Methods : For 2 minutes duration, HOCPR was conducted by 51 subjects and TCPR was conducted by 48 subjects. The accuracy measurement of chest compressions was based on the 2010 AHA guideline; the subjective fatigue level in before and after experiment was measured by a self-administered questionnaire. Results : There were no significant differences between the average depth, chest compression depth and chest compression location in terms of chest compression accuracy. However, there were significant differences between the two experimental groups in the accuracies for average speed and chest compression speed. The subjective fatigue level showed no significant difference. Conclusion : The experimental group performing HOCPR showed more accurate compression speed and lower fatigue level. These results suggested that HOCPR would be more effective in training the lay persons in accordance with the voice-instructed CPR.

Purpose : We try to disclose how much the keywords of the papers from the Korean Journal of Emergency Medical Services with Medical Subject Headings(MeSH) terminologies and to understand the major subjects of the recent emergency medical technology research in Korea from keywords. Methods : We analyzed keywords from 524 articles of the Korean Journal of Emergency Medical Services that were published between 2005 and 2011. We investigated frequently used keywords and what percentages of keywords agree with MeSH terms using the MeSH browser. Results : There were on average 3.2 keywords per article. The most frequent key words were AED, Attitude, Cardiopulmonary Resuscitation, CPR, EMT, EMT students, External Defibrillator, Job satisfaction, Knowledge, 119 EMT in order. The number of terms in precise agreement with MeSH headings was 101(19.3%); 327 terms(62.4%) were not found in the MeSH browser and 96 terms(18.3%) partially matched MeSH terms. Conclusion : Many keywords used in the Korean Journal of Emergency Medical Services did not agree with MeSH terms. We conclude that contribution rules should be using MeSH terms and authors should be educated in the proper use of MeSH terms in their research and subsequent publication.

Purpose : Emergency medical services in China are increase in demand by people and under the greater pressure than ever before. So it is, necessary to advance the pre-hospital system in order to promote the development of emergency medical services. Methods : This is based on China-related articles, books, journals, reports, statistical data and other literature. Results : First, pre-hospital emergency medical care with the introduction of specialist training program should be established. Second, to strengthen pre-hospital emergency services and to develop the EMS guidelines. Third, the "120" reporting systems unification and awareness activation. Fourth, the preparation of the EMS facilities equipment system. Fifth, the rapid transport system establishment to the selected medical institutions. Conclusion : It is necessary to strengthen the emergency medical personnel at the scene, rapid transport, rapid patient triage and to improve the survival rate of the patients.

Purpose : The purpose of this study is to get basal user guidelines of safer bag-valve-mask application on patient with normal pulmonary patho-physiologic condition. Methods : This study was accomplished by pre-qualified 25 EMS junior grade students. Participants were instructed randomly compress bag to one-third, half and total and also with differesnt compression speed. Resultant tidal volumes and mean airway pressures obtained in RespiTrainer were analysed in relation to the each compression depth and rate. Results : Demographic difference does not affect tidal volume with any compression depth and rate change. Increasing compression depth is correlated with tidal volume increasement at any compression rate and also with mean airway pressure. If the compression depth is same, compression rate change did not affect significantly the resultant tidal volume or mean airway pressure. Conclusion : Hand size, Experience, BMI dose not affect tidal volume. Compress the 1600 ml bag half to total amount is safe way to offer sufficient tidal volume without risky high airway pressure delivery to patient airway who with normal lung patho-physiologic condition.

Purpose : This research was designed to provide basic data for advanced pre-hospital airway management by comparing the time to ventilation and success rate for tracheal intubation performed with Macintosh laryngoscope and intubating Laryngeal Mask Airway(ILMA) in a manikin. Methods : All participants were novice users among EMT-Paramedic students and were divided into two groups: (1) the group for Macintosh laryngoscope guided tracheal intubation(MLG-TI) (2) the group for ILMA guided tracheal intubation(ILMA-TI). After an introductory lecture and demonstration, each group made an attempt ten tracheal intubation to compare the ventilation time and success rate for tracheal intubation. Results : 1) There was significant difference in the time to ventilation through MLG-TI, the time to first and second ventilation through ILMA-TI of the 10 attempts. 2) The time to first ventilation through ILMA-TI was significantly shorter than that of ventilation through MLG-TI. 3) There was no significant difference between the time to ventilation through MLG-TI and the time to second ventilation through ILMA-TI. 4) The success rates of ILMA-TI were significantly higher than those of MLG-TI. Conclusion : ILMA-TI can be an alternative method for MLG-TI in advanced pre-hospital airway management.

Purpose : This study was designed to identify factors influencing burnout of nurses in emergency medical center. Methods : Data were collected by questionnaires from 178 emergency room nurses randomly selected from 6 general hospitals in two metropolitan cities. Measures were burnout, QEC, depression, decision latitude and job demand. Data were analysed using frequencies, means, standard deviation, t-test or one way ANOVA, Pearson's correlation coefficients and multiple regression with SPSS/win 18.0 version. Results : The mean score for burnout in emergency room nurses was 2.59 out of possible 4. There were significant differences in burnout according to exercise, leisure, position, job demand and depression. Burnout correlated negatively with exercise and leisure, and positively with job demand and depression. Factors influencing burnout in emergency room nurses were identified as exercise(β=-.399, p<.001), leisure(β=-.354, p<.001), job demand(β=.301, p=.006) and depression(β=.189, p=.021). These factors explained 49.8% of burnout reported by emergency room nurses. Conclusion : The result indicate which factors are major factors influencing burnout in emergency room nurses in general hospitals. Therefore, these factors may serve as predictors of burnout in emergency room nurses.
