The purpose of this study was to determine the effect of two-hand backhand shots on wrist extensor and flexor muscle activation. Fourteen collegiate tennis athletes were participated in this study. The subjects does not have experiences of surgery on lateral epicondyle of elbow and musculoskeletal for at least last 6 months. The mean and standard deviation of data was calculated by SPSS/PC 18.0 and One-way ANOVA (post-hoc: LSD) was used to verify differences among groups. Significant level was p<.05. The results of this study were as follows; 1. There was significant difference on maximal value of muscle activation of extensor carpi ulnaris (D>S>V) and on maximal value of muscle activation of flexor carpi radialis(D>S>V) in male athletes by types of two-hand backhand shot(p<.05). 2. There was significant difference on maximal and average value of muscle activation of flexor carpi radialis(D>V>S) in female athletes by types of two-hand backhand shot(p<.05). In conclusion, this study suggests that beginers should learn a drive after aquiring the other skills to prevent their injuries since the level of muscle activation is higher than the others.

Objectives This study aimed to compare the effects of different external load conditions—bodyweight, weighted vest, and aqua vest—on lower-limb muscle activation and center of pressure (COP) sway during a landing and direction-change task. Methods Thirty healthy male university students (20–29 years) performed single-leg landings from a 30-cm box followed by a 90° direction change. All participants completed three load conditions: bodyweight, 8-kg weighted vest, and 8-kg aqua vest. sEMG was recorded from four lower-limb muscles and normalized to %MVIC, while COP total distance and mean velocity were collected using a force plate. A repeated-measures ANOVA with Bonferroni corrections was used to compare conditions. Results During the landing phase, the aqua vest condition elicited significantly higher rectus femoris and biceps femoris activation than the bodyweight and weighted vest conditions (p<.05). During stabilization, the aqua vest also showed the highest activation in the rectus femoris, semitendinosus, and biceps femoris (p<.05). COP total distance and mean velocity were significantly greater under the aqua vest in both phases, indicating increased postural sway and neuromuscular demand. Conclusions The aqua vest generated greater multidirectional perturbations than the weighted vest, leading to higher neuromuscular activation and COP displacement during landing and direction change. Inertial water load appears to provide a challenging dynamic stimulus and may be useful for improving neuromuscular control and dynamic stability in ACL-relevant movements.

The purpose of this study is to clarify the effects of whole-body vibration and to provide scientific basis for effective exercise by analyzing the differences in lower extremity muscle activation according to squat position during whole body vibration. In this study, muscle activation during whole-body vibration was measured in 10 healthy males in their 20s with no orthopedic conditions during standing posture, deep squat, half squat, and heel raise squat. Muscle activation was compared by measuring the root mean square (RMS) with electromyography electrodes attached to lateral gastrocnemius, tibialis anterior, vastus lateralis, and biceps femoris. The vibration intensity was applied in combinations of the volumes of 50 and 80 and the frequencies of 10, 25, and 40 Hz in each posture. The results showed that there were differences in muscle activations of external gastrocnemius, tibialis anterior, and vastus lateralis among the position(p<.05). The heal raise squat was found to have a high muscle activation effect.

PURPOSE : The purpose of this study was to compare effects of isometric hip adduction using tools on muscle activity rates of VMO and VL while performing squat. METHOD : Seventeen healthy male subjects, between twenty and fourty years old, performed traditional squat exercise and squat with isometric hip adduction using ball, Theraband, Pilates circle on stable and unstable base of support(BOS) for 3 sets of 5 seconds. They had a rest for 30 seconds between each set and rest for 30 minuts between each tool. The muscle activities of VMO and VL while performing the exercise were measured by electromyography(EMG) and muscle activity rates of VMO and VL was calculated. The obtained data were analyzed using SPSS 23.0 version. RESULT : Squat with isometric hip adduction using ball produced significantly high muscle activity rate of VMO(p<0.001). Muscle activity rate of VMO during performing squat with isometric hip adduction using tools and traditional squat was significantly greater on stable BOS than on unstable BOS. CONCLUSION : The exercise on unstable surface is more effective than on unstable surface. The ball squat exercise is the most effective than another 3 exercise group(TS, TS, PS). The best method for the selective strengthening of VMO muscle is ball squat exercise on unstable surface.

Objective: To assess the correlation between surface electromyography (SEMG) and conventional EMG in patients with facial nerve palsy. Additionally, compare the discomfort and the time required by the patients in two methods. Method: 36 patients with facial palsy were given nerve conduction studies (NCS) via conventional EMG. Then, the peak root mean square (RMS) values were obtained from the SEMG. We also recorded visual analogue scale (VAS), House- Brackmann scale, and the time required for the examination. Results: Pearson’s correlation coefficient between the amplitude loss ratio of the RMS values obtained by SEMG compared to the unaffected side (RSEMG) and the amplitude loss ratio of CMAP amplitudes compared to the unaffected side (RCMAP) was 0.567 at the frontalis, 0.456 at the orbicularis oculi, 0.393 at the nasalis, and 0.437 at the orbicularis oris. An increase in RSEMG is positively correlated with an increase in RCMAP. The mean VAS score with conventional EMG was 3.55 ± 1.42, whereas that experienced when using SEMG was 0.11 ± 0.52 and the mean time required for conventional EMG was 610 ± 103.84 seconds, while that required for SEMG was 420 ± 86.32 seconds. Conclusion: This study demonstrated a significant positive correlation between facial muscle activities as measured by SEMG and conventional EMG in patients with facial nerve palsy. SEMG has the benefits of being more comfortable and faster when diagnosing facial palsy.

Objective: To assess whether surface electromyography (SEMG) can provide additional information to conventional nerve conduction studies (NCS) in patients with mild Carpal tunnel syndrome (CTS). Method: Patients with CTS were classified into mild and moderate-to-severe groups based on conventional NCS. Healthy subjects served as the control group, and NCS were performed on them. The root mean square (RMS), median power frequency (MDF), mean power frequency (MNF), and neuromuscular efficiency (NME) values were obtained using SEMG. We observed significant changes in the SEMG signals in CTS patients compared to healthy subjects. We also examined the correlation between SEMG signal changes and conventional NCS according to the severity of CTS in the patients. Results: The moderate-to-severe group showed significantly lower RMS values than the control group, but there was no difference between the other groups. RMS values were associated with mid-palm-to-wrist sensory nerve conduction velocity in the mild group. However, there was a significant correlation between RMS and all NCS parameters in the moderate-to-severe group. Simple linear regression showed that the finger-to-wrist sensory nerve conduction velocity among the NCS parameters affecting RMS had the strongest effect. Conclusion: SEMG does not supplement conventional NCS in patients with mild CTS. SEMG can support conventional NCS to some extent for moderate-to-severe CTS patients.

Objective To identify the center of extensor indicis (EI) muscle through cadaver dissection and compare the accuracy of different techniques for needle electromyography (EMG) electrode insertion. Methods Eighteen upper limbs of 10 adult cadavers were dissected. The center of trigonal EI muscle was defined as the point where the three medians of the triangle intersect. Three different needle electrode insertion techniques were introduced: M1, 2.5 cm above the lower border of ulnar styloid process (USP), lateral aspect of the ulna; M2, 2 finger breadths (FB) proximal to USP, lateral aspect of the ulna; and M3, distal fourth of the forearm, lateral aspect of the ulna. The distance from USP to the center (X) parallel to the line between radial head to USP, and from medial border of ulna to the center (Y) were measured. The distances between 3 different points (M1?M3) and the center were measured (marked as D1, D2, and D3, respectively). Results The median value of X was 48.3 mm and that of Y was 7.2 mm. The median values of D1, D2 and D3 were 23.3 mm, 13.3 mm and 9.0 mm, respectively. Conclusion The center of EI muscle is located approximately 4.8 cm proximal to USP level and 7.2 mm lateral to the medial border of the ulna. Among the three methods, the technique placing the needle electrode at distal fourth of the forearm and lateral to the radial side of the ulna bone (M3) is the most accurate and closest to the center of the EI muscle.

Background The Pilates method has recently been recommended as the best way to improve spinal stability by strengthening the trunk and gluteus maximus (GM) muscles. Purpose In this study, we compared trunk and GM muscle activities and pelvic rotation angles during four traditional Pilates exercises. Study design Cross-sectional study Methods Twenty healthy volunteers were recruited for this study. During the four exercises, the activity of the trunk and GM muscles were measured via electromyography and pelvic rotation was measured using a smart phone-based measurement tool. Results The activities of the transverse abdominis/internal oblique and external oblique muscles were highest during the leg pull front (LPF) compared to the other exercises. GM activity was significantly higher during the LPF compared to the shoulder bridge and swimming (SW), but not during the one leg kick (OLK). Multifidus (MF) activity was significantly higher during SW than during the other exercises. There were no significant differences in MF activity between the SW and OLK exercises. Pelvic rotation angle was significantly reduced during the OLK compared to the LPF (p=0.004). Conclusions We recommend the LPF exercise if the goal is increased trunk and GM muscle activity and the SW exercise if you target for increased MF activity. The OLK exercise is useful for activating the trunk and GM muscles while reducing uncontrolled pelvic movements.

Objective To find the optimal needle insertion site for needle electromyography of the pronator teres (PT) muscle among commonly used sites. Methods Fifty forearms of 25 healthy subjects were evaluated. Four expected needle insertion points were designated as follows. Point 0 was positioned at the midpoint between the medial epicondyle and medial border of biceps tendon in the elbow crease. Points 1, 2, and 3 were located 2 cm, 3.5 cm and 5 cm distal to point 0, respectively. We assumed that the thickness of PT and the distances between a vertical line from each point to the medial margin of the PT were significant parameters for finding the optimal site. Thus, we measured these parameters through ultrasonographic examination. Results In men, the PT was thickest at point 2, and in women, at point 1. The distance between the expected needle insertion line and medial margin of PT was longest at point 1 in both men and women, and was statistically significant compared to points 2 and 3. Both men and women had neurovascular bundles located lateral to the expected needle insertion line. Conclusion The most appropriate and safe needle electromyographic insertional site for the PT is 2?3.5 cm distal to the mid-point between the biceps tendon and medial epicondyle in the elbow crease and the needle should be inserted upward and medial.

Being located in the hypogastric area, the ilioinguinal nerve, together with iliohypogastric nerve, can be damaged during lower abdominal surgeries. Conventionally, the diagnosis of ilioinguinal neuropathy relies on clinical assessments, and standardized diagnostic methods have not been established as of yet. We hereby report the case of young man who presented ilioinguinal neuralgia with symptoms of burning pain in the right groin and scrotum shortly after receiving inguinal herniorrhaphy. To raise the diagnostic certainty, we used a real-time ultrasonography (US) to guide a monopolar electromyography needle to the ilioinguinal nerve, and then performed a motor conduction study. A subsequent US-guided ilioinguinal nerve block resulted in complete resolution of the patient’s neuralgic symptoms.

A 22-year-old woman visited our clinic with a history of radiofrequency volumetric reduction for bilateral masseter muscles at a local medical clinic. Six days after the radiofrequency procedure, she noticed a facial asymmetry during smiling. Physical examination revealed immobility of the mouth drawing upward and laterally on the left. Routine nerve conduction studies and needle electromyography (EMG) in facial muscles did not suggest electrodiagnostic abnormalities. We assumed that the cause of facial asymmetry could be due to an injury of zygomaticus muscles, however, since defining the muscles through surface anatomy was difficult and it was not possible to identify the muscles with conventional electromyographic methods. Sono-guided needle EMG for zygomaticus muscle revealed spontaneous activities at rest and small amplitude motor unit potentials with reduced recruitment patterns on volition. Sono-guided needle EMG may be an optimal approach in focal facial nerve branch injury for the specific localization of the injury lesion.

Objective: To examine back muscle activity and fatigue behavior in female patients with degenerative thoracolumbar kyphosis (DTK) using high-density surface electromyography (HDSEMG) and evaluate the effects of using a soft spinal orthosis. Methods: Seven female participants with DTK (mean age: 73.6 years) were assessed during static standing and a weighted holding task with and without a soft spinal orthosis. HDSEMG signals were obtained from the lumbar erector spinae using a 64-electrode array. Time to fatigue (TTF), spatial displacement, and directional consistency of muscle activation were analyzed using the spatial center of activity (SCoA) and circular variance (CV).Results: Orthosis use significantly reduced the sagittal vertical axis and low back pain. TTF was significantly prolonged during the weighted holding task with the orthosis (p=0.012), indicating delayed fatigue onset. SCoA displacement was significantly reduced in both tasks (p<0.001), whereas CV analysis demonstrated improved directional consistency of muscle activation. Conclusion: HD-SEMG revealed early fatigue onset and unstable muscle activation patterns in patients with DTK, particularly during load-bearing tasks performed without orthotic support. Orthosis used improved endurance and neuromuscular efficiency by reducing spatial and directional variability in muscle recruitment. These findings underscore the utility of HDSEMG for elucidating the neuromuscular pathophysiology of DTK and support the use of spinal orthoses as a conservative treatment approach.

Background Shoulder horizontal abduction exercise is a common exercise for promoting lower trapezius muscle activity. However, studies on the effectiveness of shoulder horizontal abduction exercises performed in various positions are lacking. Purpose This study aimed to verify the effect of shoulder horizontal abduction exercise with thoracic extension performed in the prone and sitting positions on upper and lower trapezius muscle activity. Study design Case series study Methods A total of 12 healthy adults without shoulder pain performed prone shoulder horizontal abduction, prone shoulder horizontal abduction with thoracic extension, and sitting shoulder horizontal abduction with thoracic extension. Upper and lower trapezius muscle activity during each shoulder horizontal abduction exercise was measured using surface electromyography. Results Upper trapezius muscle activity was lowest during sitting shoulder horizontal abduction with thoracic extension compared to prone shoulder horizontal abduction with and without thoracic extension, whereas lower trapezius muscle activity was highest during prone shoulder horizontal abduction with thoracic extension compared to prone shoulder horizontal abduction and sitting shoulder horizontal abduction with thoracic extension. The upper trapezius/lower trapezius muscle activity ratio did not differ between exercises. Conclusions The findings indicate that considering various exercise positions based on the purpose of the shoulder horizontal abduction exercise is necessary.

Objective: Skeletal muscle dysfunction, which is characterized by decreased muscle strength and endurance, contributes to dyspnea during exercise in patients with chronic obstructive pulmonary disease (COPD), regardless of pulmonary function. This study analyzed skeletal muscle function in patients with COPD through surface electromyography (sEMG) evaluations and signal processing and attempted to determine whether sEMG parameters for muscle fatigue reflect the course of the disease. Methods: In 24 patients with COPD, maximal voluntary isometric contraction and ramp contraction were performed during isometric knee extension, and the sEMG activity of the rectus femoris muscle was measured. The patients were divided into three groups according to their modified Medical Research Council (mMRC) grade and Global Initiative for Chronic Obstructive Lung Disease (GOLD) stage. Patients with mMRC grades 0 and 1 were classified into group A, grade 2 into group B, and grades 3 and 4 into group C. Patients with GOLD stage 1 were classified into group I, stage 2 into group II, and stages 3 and 4 into group III. sEMG parameters were compared between groups using one-way analysis of variance. Results: There were significant differences in the minimum median frequency and fatigue index between the mMRC groups. However, the sEMG parameters did not differ significantly among the GOLD groups. Conclusion: Muscle fatigue differs according to the severity of subjective dyspnea in COPD. sEMG evaluations and signal processing can play supplementary roles in evaluating muscle endurance and predicting functional capacity in COPD.

Needle electromyography (EMG) is valid investigation routinely performed by physiatrists and neurologists, and quite safe when performed by an experienced examiner. Needle EMG is generally well tolerated and rarely thought to be associated with any significant side effects, other than mild pain, so practitioners usually overlook any potential complications. However, needle EMG is an invasive procedure and under certain situations has the potential to be associated with bleeding complications such as hematoma or acute compartment syndrome (ACS). We report a case of ACS after EMG in the left lower extremity that led to a lawsuit. To prevent complications such as hematoma and ACS resulting from needle EMG, we should be familiar with the clinical features of ACS and perform immediate evaluation and treatment to improve patient prognosis.

Objective: This study aimed to determine the proper and safe needle insertion point in the flexor carpi radialis (FCR) muscle under ultrasonography guidance. Methods: We identified the center point (CP) of the FCR as the optimal needle insertion point using ultrasonography. The location of the CP was analyzed using ratios and distances from other landmarks. The vertical distance (CP-VD) was measured by drawing an imaginary vertical line from the CP to the elbow crease. We measured the horizontal distance from the point where the imaginary vertical line from the CP meets the elbow crease to the biceps tendon at the elbow crease level (CP-HD). We presented the ratio of CP-HD to forearm circumference (HD ratio) and the ratio of CP-VD to forearm length (VD ratio) as percentages. Results: The mean values of CP-HD and CP-VD were 2.0 ± 1.0 cm and 8.2 ± 1.1 cm, respectively. The mean HD and VD ratios were 8.4% ± 4.2% and 32.0% ± 3.1%, respectively. Conclusion: When performing electromyography (EMG) of the FCR muscle, it is recommended to perform EMG at the point about 2 cm medial from the biceps tendon at the elbow crease level, to nearly the proximal one-third of forearm length.

sEMG(Surface electromyography) would have a potential to a diagnostic device with reliable, non-invasive, time-saving, inexpensive, and easy to use for swallowing disorder. Therefore, this study would review the sEMG evaluation method for dysphagia. Swallowing protocols were established with the swallowing of 3, 5, 10, 15, and 20 ml of water to measure the dysphagia in various swallowing loads and in four stages. Commonly, masseter, suprahyoid muscles, and infrahyoid muscles were investigated to observe swallowing movements. The primary sEMG outcome parameters were swallowing onset, pretrigger duration, swallowing duration, swallowing amplitude, and detecting abnormal involuntary action potential. Particularity, investigation in swallowing duration of sEMG is a simple and reliable method for screening and initial evaluation of dysphagia with noninvasive and radiation-free.

The oral cavity serves as the first organ involved in food intake and digestion; therefore, oral health is closely linked to systemic health and quality of life. Particularly, in older adults, tooth loss, occlusal imbalance, and weakened masticatory muscles can impair masticatory function, potentially leading to various health issues, including malnutrition, sarcopenia, and cognitive decline. Maintaining masticatory function is thus essential for healthy aging. To date, most previous studies investigating the association between oral health and systemic health have primarily focused on oral diseases, with very few studies examining oral function. Accordingly, this study aimed to investigate the association between functional occlusal stability during mastication and functional mobility, as measured by the Timed Up and Go (TUG) test, in older adults aged 65 years and older. This study included 187 adults aged 65 years or older (30 men and 157 women) who participated in the “Longitudinal Study on Daily Life of Older Adults through Smart Home Implementation” conducted at OO University between March 10 and October 13, 2025. The mean participant age was 74.79 ± 5.89 years. In this study, functional occlusal stability during mastication was assessed using the Global Index of Neuromuscular Balance (GI) measured with Teethan® (Teethan S.p.A., Italy), a system based on surface electromyography (sEMG). GI was measured three times, and the mean value of the three measurements was used for analysis. Functional mobility was evaluated using the TUG test. The test was performed twice, and the mean value was used for analysis. Multiple linear regression analysis was performed to evaluate the strength of the association between GI and TUG time. In the regression model adjusted for gender, age, osteoporosis, and rheumatoid arthritis, the group with high occlusal stability (GI ≥ 83) demonstrated significantly shorter TUG times compared to the control group (β = -0.166, p = 0.047). These findings suggest that oral function may potentially influence not only oral health but also systemic physical function.
