IT 기술의 발달과 평균 수명의 연장 등의 이유로 모바일 헬스케어 시장이 주목받고 있다. 대학병원 등 의료기관들도 모바일 기기를 이용한 환자 교육 및 설명서비스를 통해 편의성과 치료효율성이라는 목적을 동시에 달성하기 위해 노력하고 있다. 본 연구는 삼성서울병원이 출시한 모바일 기반의 환자 교육 서비스인 ‘스마트설명서비스’ 앱 구축 및 사용 사례를 비즈니스 모델 분석 방법로인 린 캔버스를 적용하여 분석하였다. 아울러 환자 및 의료진과의 인터뷰를 통해 모바일 환자 교육 서비스의 필요성과 효과성에 대한 의견을 수집하였다. 본 연구의 결과는 모바일 기반의 환자 교육 서비스를 준비 중인 의료기관과 관련 업계에 유의미한 시사점을 제공할 것으로 기대한다.

Researchers and industry managers highlight the emergence of mobile healthcare market given the rapid development of IT and the expansion of average life expectancy. Medical institutions, such as university hospitals, are aiming to achieve convenience and effectiveness of treatment by providing patient education through a mobile device. This study examines a case related to the development and usage of a mobile application based on Lean Canvas, namely, the Smart Explanation Service released by Samsung Medical Center. This application comprehensively analyzes the usage status of mobile application-based patient education and its application in clinical settings. This study also examines the level of satisfaction of patients and medical workers toward this application. The result of this study is expected to provide meaningful implication to medical institutions and related industries and enable them to create and offer prepare mobile healthcare services.

Purpose: The purpose of this study was to develop and evaluate the quality (understandability and actionability) of health education materials for Korean-Chinese (KC) female migrant workers, using Patient Education Materials Assessment Tool for Printable Materials (PEMAT-P). Methods: Educational needs assessment was conducted with 3 focus groups with 20 KC women and a focus group with 4 community stakeholders. The quality of the educational materials was evaluated by 3 experts and a community stakeholder, followed by a survey with 15 KC women using 17 items for understandability and 7 items for actionability by means of a Korean version PEMAT-P. Results: The health educational calendar consists of 12 subjects out of 9 topics related to healthy lifestyles for preventing cardiovascular diseases. The overall mean understandability score was 98.8% and the overall mean actionability was 100%. Conclusion: Involvement of KC women and community stakeholders in the development of educational materials was found to be an effective strategy for increasing understandability and actionability of educational materials for KC female migrant workers. This study also demonstrates the PEMAT-P is a useful evaluation tool, emphasizing the actionability of educational materials.

Effect of Patient Education on Reducing Medication in Spinal Cord Injury Patients With Neuropathic Pain

Objective To determine whether providing education about the disease pathophysiology and drug mechanisms and side effects, would be effective for reducing the use of pain medication while appropriately managing neurogenic pain in spinal cord injury (SCI) patients.Methods In this prospective study, 109 patients with an SCI and neuropathic pain, participated in an educational pain management program. This comprehensive program was specifically created, for patients with an SCI and neuropathic pain. It consisted of 6 sessions, including educational training, over a 6-week period.Results Of 109 patients, 79 (72.5%) initially took more than two types of pain medication, and this decreased to 36 (33.0%) after the educational pain management program was completed. The mean pain scale score and the number of pain medications decreased, compared to the baseline values. Compared to the non-response group, the response group had a shorter duration of pain onset (p=0.004), and a higher initial number of different medications (p<0.001) and certain types of medications.Conclusion This study results imply that an educational pain management program, can be a valuable complement to the treatment of spinal cord injured patients with neuropathic pain. Early intervention is important, to prevent patients from developing chronic SCI-related pain.

Changes in Health-Related Quality of Life by Patient Education and Rehabilitation Based on a Behavior Change Program in Knee Osteoarthritis

Objective: The purpose of this study was to examine how rehabilitation and patient education for knee osteoarthritis improves health-related quality of life (HRQOL) and to identify factors influencing HRQOL.Methods: Between May 2020 and March 2022, 30 patients with osteoarthritis of the knee were treated conservatively and rehabilitated with a patient education program. The patient education program was based on the health belief model by Sedlak et al., and patient education using pamphlets was provided during the rehabilitation intervention. The survey items were patient basic information, instrumental activities of daily living (ADL) (FAI), fear of falling (FES), degree of depression (GDS), HRQOL (SF-8), knee function assessment (JOA score), and X-ray classification (K-L classification), and the survey method was a self-administered questionnaire at the start of rehabilitation, 1 month after the intervention, and at the end of the rehabilitation intervention. We examined factors affecting the physical component summary (PCS) and mental component summary (MCS) of HRQOL scores.Results: JOA score, FES, FAI, GDS, and SF-8 improved significantly (p<0.01). MCS was also negatively correlated with FES and age (r=-0.486, -0.368). Sex was extracted as a factor for PCS as a factor affecting HRQOL (p<0.01). MCS was extracted with FES as a factor (p=0.046).Conclusion: A rehabilitation intervention incorporating patient education in osteoarthritis of the knee showed improvement in HRQOL and may be useful for improving depression, fear of falling, and instrumental ADL.

Therapeutic patient education and exercise therapy in patients with cervicogenic dizziness: a prospective case series clinical study KCI 등재

The purpose of this study was to evaluate the effectiveness of a treat-ment for patients with cervicogenic dizziness that consisted of thera-peutic education and exercises. The Dizziness Handicap Inventory and Neck Disability Index were used. Secondary outcomes included range of motion, postural control, and psychological variables. Seven patients (two males and five females) aged 38.43±14.10 with cervicogenic dizzi-ness were included. All the participants received eight treatment ses-sions. The treatment was performed twice a week during a four weeks period. Outcome measures included a questionnaire (demographic data, body chart, and questions about pain) and self-reported disability, pain, and psychological variables. Subjects were examined for cervical range of motion and postural control. All of these variables were as-sessed pre- and postintervention. Participants received eight sessions of therapeutic education patient and therapeutic exercise. The majority of participants showed an improvement in catastrophism (mean change, 11.57±7.13; 95% confidence interval [CI], 4.96–18.17; d=1.60), neck disability (mean change, 5.14±2.27.28; 95% CI, 3.04–7.24; d=1.32), and dizziness disability (mean change, 9.71±6.96; 95% CI, 3.26–16.15; d=1.01). Patients also showed improved range of motion in the right and left side. Therapeutic patient education in combination with thera-peutic exercise was an effective treatment. Future research should in-vestigate the efficacy of therapeutic patient education and exercise with larger sample sizes of patients with cervicogenic dizziness.

통풍은 관절염뿐만 아니라 심혈관질환 등을 유발하는 만성염증질환이다. 하지만 대부분의 환자들이 단순히 관절질 환으로 인식하고 있어 치료에 장애로 작용한다. 이 연구를 통해 간호사 주도의 통풍환자 대상 교육이 환자의 통풍에 대한 인식 개선 및 질병 경과에 미치는 영향을 확인하고자 하였다. 교육전문 간호사가 상급종합병원 류마티스내과 외래에 내원 한 환자를 대상으로 통풍에 대한 일대일 교육을 시행하였고 환자의 만족도 및 복약순응도, 혈중 요산치의 변화 정도를 측정 하였다. 교육을 받은 환자군의 진료만족도는 대조군에 비해 유의하게 높았으며 두 번째와 세 번째 외래 방문 시에 측정한 복약순응도와 혈중 요산치는 대조군에 비해 교육군에서 유의하게 개선이 되었다. 결론적으로 간호사 주도 환자교육은 진료 만족도 개선 및 질환 경과의 호전에도 도움이 되는 것으로 확인되었다.

Objectives: This study was performed to analyze the effect of nurse-led education on results for patients with gout. Methods: The face-to-face education was conducted by a specialist nurse. Patient satisfaction and drug adherence, serum uric acid was assessed. Results: Patient satisfaction was significantly higher in the nurse-led education group than in the usual care group. The serum uric acid level on the second visit was lower after patient education . In addition, the mean drug adherence in the nurse-led group was significantly higher than in the usual care group on the second and third visits. Conclusion: The nurse-led education about gout improves patient satisfaction, drug adherence, and serum uric acid level in patients with gout.

Motor control exercise and patient education program for low resource rural community dwelling adults with chronic low back pain: a pilot randomized clinical trial SCOPUS KCI 등재

This pilot randomized clinical trial assessed the feasibility of implement-ing motor control exercise (MCE) and patient education (PE) program for the management of chronic low back pain (CLBP) in a low resource rural Nigerian community. Thirty patients with CLBP were recruited and randomly assigned to MCE, PE, or MCE plus PE groups. The MCE pro-gram was provided twice a week while the PE program was provided once a week all for 6 weeks. Feasibility was assessed through recruit-ment rate, treatment compliance, retention/dropout rate, report of ad-verse events, perceived helpfulness, overall satisfaction, and clinical outcome of pain (numeric pain rating scale) and functional disability (Oswestry Disability Index). Many patients were willing to participate in the study and the recruitment rate was 77%. Treatment compliance in all the three groups were >65% for supervised treatment sessions and <50% for prescribed home program. Retention rate was high and greater overall satisfaction with the interventions was reported. Com-pared with the baseline, all the three groups improved significantly in pain and disability (P<0.05) after 6 weeks. Pairwise comparison re-vealed that the MCE plus PE group was superior to the PE group for pain and to the MCE for disability (P<0.05), with large effect size. It was concluded that the designed interventions are promising and conduct-ing a full-scale randomized clinical trial in the future is feasible to con-firm the effectiveness of the interventions for the management CLBP in rural Nigeria. 환자의 입원전후, 입원생활, 수술전후, 퇴원후 정보교육 및 식사 영양지도 등 의료 기관에서의 적재 적시적 환자교육 정보의 부재, 지연과 오류는 임상적 예후는 물론 의료의 질 저하와 의료분쟁 등 심각한 문제를 야기 시키고 있다. 따라서 최근 의료선진국을 중심으로 의료의 질 관리, 상호소통 및 의료분쟁의 화두와 함께 맟춤형 환자교육 체계의 중요성이 크게 부각되고 있다. 의료진, 원무행정 및 임상 실험실들을 중심으로 환자교육이 네 트워크기반으로 활발하게 이뤄질 때 의료 질이 향상되고, 궁극적으로 병원의 경영이익창출에도 도움이 되는 것 으로 S병원 사례에서 유의미하게 나타났다. 이러한 동기에서 본 논문에서는 CPES 개념의 연구를 바탕으로 웹 기반 맞춤형 환자교육 시스템을 제안하고 S 병원을 대상으로 설계와 구현을 진행하였다. 더불어 S병원에서의 실증적 계량분석을 통해 맟춤형 환자 중심 교육 시스템의 도입 효율성과 효과성을 분석하였다.

Recent healthcare studies evaluate the effective impacts of a patient educational information intervention in hospitals on adherence to communicating knowledge, life quality, and therapeutic response in patients treated with highly active therapy in medical organizations. Therefore, the aim of this study is to design and implement the effective and systematic frameworks by providing clinical patient-oriented education system(CPES), which enhances health promotion and medical cures. CPES initiatives accelerate positive financial returns with network-driven educational system. To validate the CPES, the author analyzes the efficiency and effectiveness using statistical survey works. With this outcome, it is compared with previous system. One of the findings is that the system can improve compliance with treatment right dietetics and give patients self-manage their conditions in S hospital. Patients' acknowledge of their condition and its treatment process can be improved through the educational systems.

최근 국가 경제 및 의학 발전으로 국민의 의료생활이 크게 변화하였다. 의료산업은 다양한 검사와 치료법으로 국민 개인의 삶과 질을 향상시켰다. 그로인해, 수명연장 및 고령화 사회로 되어졌다. MRI 보급 증가는 검사 건수가 증가하였 다. 그리고 MRI 검사가 진행되는 동안 환자의 움직임으로 인해 영상의 진단적 가치는 낮아지기 때문에 환자의 움직임을 최소화해야 한다. 검사를 위해 환자에게 충분한 설명이 되었을 때 환자의 검사 이해 및 만족도가 높을 것이다. 디지털 리플릿 교육 대상자는 실험군 50명, 대조군 50명으로 총 100명을 대상으로 하였다. 본 연구 결과 MRI 경험은 유의한 차이가 없었으며, MRI 지식은 대조군 2.62±0.90보다 디지털 리플릿 교육 후의 실험군 3.10±0.76으로 높은 것을 확인할 수 있었다. MRI 이해는 대조군 2.30±0.95보다 디지털 리플릿 교육 후의 실험군 4.44±0.61로 높았으며, 만족도 조사도 대조군 2.36±0.78보다 4.70±0.54로 높게 나타났다. 디지털 리플리 교육 후 환자들의 MRI 이해, 지식 및 만족도가 향상됨을 확인하였으며, 차후 연령대별 이해도에 맞는 디지털 리플릿 교육의 적용이 필요하다.

Recently, people's medical lives have changed significantly due to the development of the national economy and medicine. The medical industry has improved the lives and quality of people's lives through various tests and treatments. As a result, life expectancy has been extended and an aging society has emerged. The increase in MRI availability has led to an increase in the number of tests. Additionally, the patient's movement must be minimized during the MRI examination because the patient's movement reduces the diagnostic value of the image. When sufficient explanation is provided to the patient for the test, the patient's understanding and satisfaction with the test will be high. The subjects of digital leaflet training were 50 in the experimental group and 50 in the control group, for a total of 100 people. As a result of this study, there was no significant difference in MRI experience, and MRI knowledge was confirmed to be higher at 3.10±0.76 in the experimental group after digital leaflet training than in the control group at 2.62±0.90. MRI understanding was higher in the experimental group after digital leaflet education at 4.44±0.61 than in the control group at 2.30±0.95, and the satisfaction survey was also higher at 4.70±0.54 than in the control group at 2.36±0.78. It was confirmed that patients' MRI understanding, knowledge, and satisfaction improved after digital leaflet education, and it is necessary to apply digital leaflet education appropriate for understanding by age group in the future.

This study aims to identify the effect of a virtual reality (VR)-based isolation patient infection control education program on nursing students' infection control knowledge, awareness, and performance confidence. This research design was a quasi-experimental pre-test and post-test research design. The participants were 55 nursing students (experimental group 28, control group 27). Data were analyzed using descriptive analysis, the independent t-tests, and paired t-tests with SPSS version 27.0. The VR content included hand hygiene, donning and doffing personal protective equipment (PPE), post-exposure prophylaxis of sharps injury, and disposal of medical waste. The experimental group showed significantly improved in infection control knowledge (t=3.117, p=.003) and performance confidence (t=3.059, p=.004) than the control group,. However, after applying the education program, awareness of the control group showed a more significant change. This VR-based isolation patient infection control education program for nursing students can help increase competency in infection control practice.

Background/Objectives: This study aims to apply a structured nursing process education program and test the effectiveness of nursing college students in order to effectively conduct case studies in their first clinical practice. Methods/Statistical analysis: This study is a methodological study to develop a structured nursing process education program suitable for the competency of first time nursing clinical practice college students for the first time through literature review and a group of experts. Findings: The results of case studies of clinical practice students by applying the structured nursing process education program are as follows. In the first clinical practice, anxiety about the patient case study nursing process was reduced. It strengthened the learning ability of clinical practice students. The application of the structured nursing process education program increased satisfaction with teaching methods. Through structured nursing process education, quantitative studies are conducted to determine whether the academic competency of clinical practice students has increased. Improvements/Applications: It is suggested that the structured nursing process program be modified to be used in various nursing area.

암환자교육이 암환자의 심리적 디스트레스와 자가간호지식 및 자가간호행위에 미치는 효과: 메타분석

Purpose: The purpose of this study was to evaluate the effectiveness of patient education interventions on distress, self-care knowledge and self-care behavior in cancer patients. Methods: A total of 1,102 studies were retrieved from 6 electronic databases in Korea. From these studies, 18 studies met the inclusion criteria with a total of 850 participants. Two authors independently assessed the methodological quality by Cochrane’s Risk of Bias and Methodological Items for Non Randomized Studies. The data were analyzed by the RevMan 5.1 program of Cochrane library. Results: Overall effect size of education interventions on anxiety was -2.12 (95% CI: -3.90, -0.34) (p<.001). The effects on self-care knowledge and self care behavior were -1.08 (95% CI:-1.73, -0.43) (p=.001), and -1.41 (95% CI: -2.13, -0.68) (p<.001), respectively. Publication bias was detected as evaluated by funnel plot, but the fail-safe number was moderate. Conclusion: This study suggests that patient education interventions can relieve anxiety and self-care. Further randomized controlled trials studies are needed to evaluate the effects of patient education intervention on depression.

다발골수종 환자의 안전한 투약을 위한 약사의 환자교육 프로세스 개선방안

Background : As medication errors can cause serious harm to patients, preventing and avoiding them is important. Recently, a series of medication errors associated with taking dexamethasone at the wrong frequency occurred in patients with multiple myeloma in St. Mary’s Hospital. To prevent medication errors, the pharmacy improved the overall process including medication preparation, labeling, patient education, and monitoring. This study aimed to evaluate the effectiveness of these changes and the need for additional action. Methods : The improvements included changing the educational materials, including labeling, and increasing the number of medication counseling sessions. Medication counseling was conducted three times. The first was before starting the medication, the second was 2~3 days after starting medication, and the third was at the end of the first cycle of treatment. Assessment of medication awareness (usage, drug information, 3 points each) and compliance (based on Morisky’s scale, 5 points) was done at each counseling session. Results : The mean age of the study population was 60.8 years. The mean score of medication usage awareness at the 1st, 2nd, and 3rd sessions was significantly increased (2.13, 2.97, 2.97, respectively, p<0.001). The mean score of medication information awareness at the 2nd and 3rd sessions was 1.72 and 1.75, respectively (p=0.872). The medication compliance at the 2nd counseling was higher in the group with medication usage awareness scores of 3 (n=49) than in the group with scores of 2 (n=2; p<0.001). Conclusion : Medication counseling could enhance the awareness of medication usage and the compliance. However, there was no significant difference in the awareness of medication information after repeated education. These results suggest that additional improvement in packaging and clear labeling could be more helpful than enhancing drug information education.

Rituximab 항암화학요법에 대한 약사의 환자 교육 개선 및 적용 효과

Rituximab is a monoclonal antibody approved for the treatment of non-Hodgkin’slymphoma. Infusion-related reactions have been reported in 50% of the patients within 1-2 hoursof starting the infusion. The patient education had been performed after the administration ofrituximab, due to heavy work-load during the morning shift at Seoul St. Mary’s Hospital. Therefore, an improvement in education method for rituximab-containing chemotherapy wasrequired. The purpose of this study was to evaluate the improved education method and theresult of the application of new method. The patients in this study were over 19 years of age, who received the first combination chemotherapy including rituximab, at Seoul St. Mary’s Hospital from May 2011 to January 2012. The patients who received the education previous to improvement were the control group (n=20),and patients who received the improved education were the study group (n=20). The improvededucation was performed on the patients before and during the rituximab administration,respectively. The contents of the education were the same before and after the improvement. In the control group, 13 patients experienced subjective side effects, and 6 of those patientsimmediately reported their conditions (6/13, 46.2%). In the study group, 9 patients out of 11immediately reported their subjective side effects (9/11, 81.8%), and this result was not statisticallysignificant (p=0.270). The points of the recognition for hypersensitivity were shown to be50.0±25.6 and 67.2±22.6 in the control and study groups, respectively, and this was significantlyhigher in the study group (p=0.031). The patient’s understanding, needs, and satisfaction fromthe education were shown to be 68.9±14.0 and 65.0±20.6 (p=0.612), 74.3±14.0 and 75.0±19.4(p=0.904), and 80.6±14.4 and 81.3±13.1 (p=0.943), in the control and study groups, respectively. The new education methods focusing on hypersensitivity to rituximab improved on the recognitionand the management of hypersensitivity reactions.

Through a comprehensive review of the literature, informational preferences and educational readiness of patients and their families, and associated factors have been examined in the contexts of oncology and hematoncolgy care. It appeared that patients and their families preferred receiving information from a variety of learning methods, particularly verbal communication with physicians. This finding gives rise to the need for diverse teaching methods and physicians' participation in patient education. Moreover, considering that when patients and their families got information, their emotions could be negatively as well as positively influenced by the information provided, their emotional states should be carefully and constantly assessed. Furthermore, there remained a number of assumptions regarding influential factors in patients’ and their families’ informational needs and preferences. Accordingly, designing patient education based on those related factors appears to be unwarranted. Rather, it is suggested that healthcare professionals need to individually approach patients and their families. Also, since patients’ and their families’ educational readiness is influenced by their psychological adjustment to illness, ongoing assessment of their psychological adjustment and responses to illness is required to provide patient education consistent with their educational readiness.

Purpose: This study aimed to evaluate the effects of simulation education in patient safety nursing on nursing students’ attitudes toward patient safety, confidence in patient safety, academic self-efficacy, and academic engagement. Methods: The study was conducted from April to May 2022 with 69 nursing students enrolled in a fundamental nursing practice course. Two simulation scenarios were developed to enhance the patient safety competencies. Each scenario was implemented in two sessions, each lasting 90 minutes. Data were collected using self-report questionnaires, which measured attitudes toward patient safety, confidence in patient safety, academic self-efficacy, and academic engagement before and after the simulation education. Data were analyzed by employing descriptive statistics and paired t-tests using SPSS Statistics Version 26.0. Results: After participating in the simulation program, students showed statistically significant improvements in their attitudes toward patient safety (t=9.11, p<.001), confidence in patient safety (t=15.66, p<.001), academic self-efficacy (t=6.03, p<.001), and academic engagement (t=9.28, p<.001). Conclusion: The findings suggest that simulation education in patient safety nursing is an effective nursing education strategy. The study highlights the need to develop and expand diverse simulation-based learning programs in nursing education.

Purpose: Room of Errors is a little-known method in Korea, effective for patient safety education. This study aimed to examine nursing students’ recognition of pre-staged medical errors while working as individuals or in a team. Methods: Thirty-four errors for a pre-op care scenario and thirty errors for a post-op care scenario were pre-set in two simulated patient rooms. Fifty-six nursing students randomly participated as individuals or as a team in one of two “Room of Errors” to find as many errors as possible within a certain time. The evaluation of error detection and debriefing occurred immediately following the simulation. Results: “Wrong patient name on wrist band” (77-100%) and “bedside rails down” (91-100%) were the most frequently identified errors by both individuals and teams. Few students found “injection of a drug to which the patient is allergic” (0-9%) and “administration of a contraindicated drug to the patient” (0-7.7%). The performance of students working in a team was much better than those working alone. Conclusion: This study found that “Room of Errors” provided very experiential and practical learning to nursing students in identifying simulated patient threats. The method is also useful for interprofessional patient safety education to develop teamwork and communication.

Purpose: This study aimed to develop and implement a multi-patient simulation (MPS) program for nursing students with no prior clinical practice experience. It also examined the effects of the program on the students’ communication competence and clinical reasoning ability. Methods: A one-group pretest-posttest design was used. The MPS program, consisting of four patient scenarios was applied to second-year nursing students with no prior clinical practice experience. Communication competence, clinical reasoning ability, and the perceived effectiveness of the multi-patient simulation program were measured using structured tools before and after the program. Results: Communication competence significantly improved after the MPS program, whereas clinical reasoning did not show a statistically significant difference. Perceived effectiveness of the MPS program was generally high, with the debriefing component scoring the highest. Confidence scores were relatively low, suggesting the need for level-appropriate scenario. Conclusion: The MPS program was effectively enhanced communication competence among preclinical nursing students. Although clinical reasoning scores did not improve significantly, the study highlights the importance of introducing realistic simulation experiences early in nursing education. Future research should focus on developing suitable clinical reasoning assessment tools for early year students and conducting randomized controlled trials to validate the effectiveness of customized MPS programs.

수술 전 통증자가조절기 교육이 슬관절치환술 노인의 수술 후 통증에 미치는 효과

본 연구의 목적은 슬관절 치환술을 받는 노인의 수술 후 통증에 대한 수술 전 통증자가조절기 교육의 효과를 조사하기 위함이다. 방법. 본 유사실험연구의 대상자는 슬관절치환술을 받는 65세 이상 노인 환자 50명이다. 실험군에게 제공된 교육내용이 대조군의 대상자에게 확산되지 않도록 하기위해 대에 대조군의 자료수집을 먼저 한 다음 실험군의 자료 수집을 완료하였다. 실험군의 대상자에는 수술을 받기 전에 15분 가량 통증자가조절기와 통증 조절에 대한 교육을 제공하였다. 교육프로그램은 통증자가조절기에 대한 사용 시범도 포함하였다. 결과. 실험군은 대조군에 비해 통증과 통증자가조절기에 대한 지식수준이 더 높았고 통증과 진통제 사용에 대한 태도가 더 긍정적이었다. 두 그룹 사이에 수술 후 진통제 사용량에 있어서 유의한 차이는 없었다. 실험군의 수술 후 8, 24, 36시간에 측정된 통증은 대조군보다 유의하게 낮았다. 실험군은 대조군에 비해 통증자가조절기 사용에 대한 만족도가 더 높았다. 결론. 노인들에 맞춤식 통증자가조절기 교육은 슬관절치환술 노인의 수술후 통증에 대한 효과적인 간호중재가 될 수 있다.

Purpose: The purpose of this study was to investigate the effect of preoperative education about patient controlled analgesia (PCA) on postoperative pain control for elderly after total knee arthroplasty. Methods: The study applied a quasi-experimental design. To prevent communication between experimental group and control groups, data from control group were collected before provision of preoperative education for the experimental group. A total of 50 elderly patients who underwent total knee arthroplasty and older than 65 years old participated in this study. The preoperative education about PCA was provided for the experimental group before surgery. The preoperative education program consisted of fifteen minute education about pain control, and PCA use, as well as demonstration of PCA use. Results: The experimental group had higher knowledge score about pain and PCA use, and more positive attitudes toward pain and use of analgesics after surgery than the control group. There was no significant difference in use of additional analgesics after surgery between the two groups. The experimental group had significantly lower pain score at 8, 24 and 36 hours after surgery than the control group. The experimental group had higher level of satisfaction about PCA use than the control group. Conclusion: The preoperative education about PCA, customized for elderly patients could be an effective nursing intervention for postoperative pain control after total knee arthroplasty.
