통합보육 어린이집에서 보이는 장애영유아의 문제행동에 관한 연구

This study aimed to identify problem behaviors of disabled infants in integrative daycare center. Data of 83 infants with disability were collected from 55 childminders, and then were analyzed to determine problem behaviors. In the results, the problem behaviors of infants with disability appeared to be categorized into three types: (1) excessive behaviors - the infants is usually very fidgety and shows immoderate activities, (2) behavioral shrinking - the infants is shy and activities are shrank, and (3) bulling - the infants harass peers. These findings suggest that several problem behaviors have been identified from infants with disability, and it should be considered as important aspects to improve the effectiveness of their care in ordinary life. This study provides valuable information for further researches to develop advanced strategies to ameliorate the quality of care-giving service.

고빈도 반복적 경두개자기자극이 뇌졸중 환자의 우울증과 운동기능에 미치는 영향

This study aimed to identify the effect of a high frequency repetitive transcranial magnetic stimulation (rTMS) on the depression and motor function in patients with stroke. Thirty subjects with post-stroke hemiparesis volunteered to participate in this study. All subjects underwent the high frequency rTMS for averaging 20 minutes daily for 4 weeks (total of 20 sessions). Assessment tools were made with Beck depression index (BDI) and Fugl-Meyer assessment (FMA) before and after the 4-week training period. BDI and FMA showed significant improvement after high frequency rTMS (p<0.05). This study demonstrates that a high frequency rTMS may be favorable for enhancing depression and motor function of patients with stroke. This study provides valuable information for future studies in this field. Further studies with wider range of subjects and longer experiment span are required to strengthen the results of this study.

젊은 여성에서 신발 뒤굽 높이가 신체 동요 시 다리 근육의 근 활성도와 개시시간에 미치는 영향

This study aimed to identify the influence of high-heeled shoes on activation patterns (muscle activity and onset time) of leg muscles during body perturbation in healthy young females. Thirty-one healthy females in their 20s were recruited for this study. Subjects underwent two experimental conditions: (1) barefoot, and (2) wearing high-heeled shoes with a 7-cm heel height. And then forward and backward body perturbation impacts were randomly applied in each condition while subjects stand on perturbation board. Electromyographic measurements were performed on gastrocnemius (GCM), hamstring (HAM), and erector spinae (ES) for the forward perturbation, and tibialis anterior (TA), quadriceps (QAUAD), and rectus abdominis (RA) for the backward perturbation. During the forward perturbation, there were found for significant differences in the onset time among GCM, HAM, and ES for both conditions (p<0.05), suggesting differences between GCM and HAM and between GCM and ES for the barefoot condition, and between GCM and HAM for the high-heeled shoes condition. Muscle activities showed significant differences in both conditions (p<0.05), indicating differences between GCM and HAM and between GCM and ES for both conditions. Furthermore, during the backward perturbation, the onset time of muscle activity was significantly different among TA, QUAD, and RA in the barefoot condition (p<0.05), suggesting a difference between TA and QUAD. Muscle activities appeared to be significantly different between the muscles for both conditions (p<0.05), indicating differences between TA and QUAD and between QUAD and TA for the both condition.These findings indicate that wearing high-heeled shoes leads to changes for recruitment pattern of leg muscles, indicating alterations of ankle strategy to maintain balance during body perturbation.

The purpose of this study is to find the effects of compare the shoulder stability muscle activity during a modified push up plus exercise according to glenohumeral joint rotation in healthy subjects by surface electromyographic (EMG) activities of the upper trapezius (UT), pectoralis major (PM), and serratus anterior (SA). Twenty-seven healthy young subjects (male - 15, Female - 12) voluntarily participated in this study. Subjects performed modified push up plus exercise on glenohumeral joint rotation (external rotation 0°, 45°, 90°, 110°). Modified push up plus exercise is designed to control the glenohumeral joint rotation angle more than standard push up plus exercise. Surface EMG was recorded from the UP, PM, and SA muscles. All measurement and progress were performed equally. Dependent variables were examined by repeated measurement of ANOVA with a statistical significance level of α= .05. In the result of the study, upper trapezius between the glenohumaral joint rotation were significantly change in 45° and 110° (p<.05), 90° and 110° (p<.05). pectoralis major between the glenohumaral joint rotation were significantly change in 45° and 110° (p<,05), 90° and 110° (p<.05). serratus anterior between the glenohumaral joint rotation were significantly change in all degree (p<.05). The upper trapezius showed greater mean muscle activation on 110° of glenohumeral joint rotation (Lower mean muscle activation on 90°). The pectoralis major showed greater mean muscle activation on 110° of glenohumeral joint rotation (lower mean muscle activation on 45°). The serratus anterior showed greater mean muscle activation on 110° of glenohumeral joint rotation (lower mean muscle activation on 0°). These results showed that the modified push up plus exercise is effective to selectively strengthen the shoulder stability muscles.

수정된 구속치료가 뇌성마비 아동의 상지 운동 기능과 보행의 변화에 미치는 효과

This study aimed to identify the effects of modified constraint-induced therapy (mCIMT) on upper limb’s function and walking in cerebral palsy. Study was carried out by using an ABA design with baseline, intervention, and follow-up phases. And a subject was a child with left hemiplegic cerebral palsy. The mCIMT with unaffected side restricted by a plastic splint was performed averaging five 5 hours per day for 5 weeks. Subjects underwent an intensive training to affected upper limb for 2 hours and performed daily routine activities for 3 hours. Outcome measures included the box and block test, coin transfer test, one-leg test, and walking velocity. After the intervention, values were improved in all parameters, and gains were maintained during the follow-up phase. These findings suggest that mCIMT is helpful to enhance the function of affected upper limb, balance, and walking. Further studies will be continued to warrant the findings of present study.

치매 노인의 인지 기능에 대한 운동치료중재의 효과 : 체계적 고찰연구

Numerous studies have demonstrated the use of an exercise therapy intervention as a beneficial option to enhance cognitive function of older people with dementia presenting mild cognitive impairment. However, evidences to support its efficacy are not enough to establish clinical guideline and recommendation for improving cognitive function of older people with dementia. Accordingly, we systematically reviewed previous studies that evaluated the effectiveness of exercise therapy intervention for cognitive improvement in older people with dementia. Our results indicate that physical exercise seems to improve cognitive function, including execution, attention, recall capacity, memory, and communication, in older people with dementia. In previous studies, the duration of the exercise was ranged from 6 weeks to 12 months, and the type of the exercise comprised walking training and strengthening exercise combined with aerobic exercise such as treadmill and cycling. This systematical review provides valuable information for further studies that inform benefits of exercise therapy for enhancing cognitive function of elderly with dementia and develop appropriated protocols and strategies to manage their cognition. In the future, it needs to investigate the effects of exercise therapy protocol of people with dementia in content.

Balance is one of essential components to improve functional level after stroke, which is complicated motor skill depending on multi sensory-motor process and interaction between environmental factor and functional condition. After stroke, neurological impairments lead to the decrease in functional level and produce compensatory strategy in order to ensure an independence in daily activities. Because of such variety in this aspect, precise evaluation should be performed to identify the problems of each patients in motor, sensory, and cognition as well as functional implications, which is helpful to establish individualized program in stroke rehabilitation. Tools to evaluate balance function should be adequately valid, and selected by reflecting personal characteristics of patients with stroke. Therefore, many evaluation tools have been developed to demonstrate balance function. Laboratory tests are difficult to be commonly used in clinical setting, benefits are to offer quantitative data on balance reaction and performance. Also, clinical tests provide valuable data to understand postural impairment and function disability of patients with stroke. Accordingly, this study aimed to describe balance mechanisms and strategies as well as evaluation methods of balance function in stroke. Focus was to demonstrate clinical tests to readily use in clinical setting.
