수술실 CCTV 설치 및 운영에 대한 고찰 KCI 등재

최근 수술 관련 의료사고가 증가하고, 일부 의료사고가 보건범죄와 연루되었다는 사실이 언론에 보도되었다. 환자단체는 수술실 내 CCTV 설치 및 운영 의무화를 촉구하였고, 이에 대한 이해관계인들의 논의가 활발히 이루어지고 있어 관련 법령에 대한 검토가 필요한 상황이다. 본 연구에서는 수술실 CCTV에 대한 특성을 파악하고, 수술실 CCTV 설치 및 운영에 관련 법령에 대해 비판적으로 검토하고자 한다. 현재 의료기관 내에서 CCTV는 시설물 관리용 및 환자안전관리용을 주목적으로 사용되고, 수술실의 경우 의료기관이 선택적으로 CCTV를 설치 및 운영하고 있다. 헌법은 모든 개인의 사생활 및 통신의 비밀과 자유를 침해받지 않을 권리를 보장하고 있으나, 이는 공공복리를 위해 법률로써 제한할 수 있다고 규정하고 있다. 그러나, 수술실 CCTV 설치 및 운영과 관련하여 법률이 현재 존재하지 않기 때문에 이는 법률의 흠결에 해당할 수 있다. 현 법체계상으로는 수술실 CCTV 설치가 의무화될 경우 정보주체인 의료진의 권리에도 불구하고 보건의료서비스 공급자의 특성상 개인정보자기결정권을 침해받을 가능성이 크다. 또한, 영상정보처리기기운영자의 CCTV 조작 시 열람과 업무 중 알게 되는 비밀의 누설에 관한 제한규정이 미흡한 상태여서 영상정보의 안전성이 위협받을 수 있다. 나아가, 수술실 CCTV의 경우 영상정보 보관기간, 보관장소 등이 명확히 규정되지 않는다면 환자안전과 의료사고 예방이라는 본래 목적에 부합되지 못할 가능성이 크다. 수술실 CCTV 설치 및 운영에 대해서 현재 활발하게 논의가 이루어지고 있고, 관련 의안이 몇 차례 발의되고 있어 이에 대한 법적 검토의 필요성이 증가하고 있지만 이에 관한 선행연구가 거의 없다는 측면에서 본 연구의 의의가 있다. 향후 수술실 CCTV 설치 및 운영 관련 법령 제․개정 시 본 연구 결과를 활용할 수 있을 것으로 기대한다.

Recently, medical accidents related to surgical procedures have increased. In addition, the media reported that some of these accidents were involved in health crimes. Patient-advocate groups have called for mandatory establishment and management of CCTV in operating rooms. There is a lot of discussion among the interested parties, so it is necessary to review the relevant laws and regulations. The purpose of this study is to identify the characteristics of CCTV in operating rooms and to review legislations related to establishment and management of the CCTV in operating rooms. Medical institutions use CCTV for management of facilities and patient safety and install it in operating rooms optionally. The Constitution guarantees the privacy and the privacy of correspondence of every citizen, but it can be limited by the law for public welfare. Currently, however, there is no existing law about establishment and management of the CCTV in operating rooms and it can be defect of legal system. Under the current legislations, it is likely that the Self-determination can be violated due to the characteristic of healthcare provider when CCTV is mandatorily installed in operating room. In addition, the regulations on access and leakage of confidential information known by operator are insufficient. So that, the safety of the visual data might be threatened. Furthermore, unless the period and the place of storage of the visual data are clearly defined, it is highly unlikely to meet the original purpose of patient safety and prevention of medical accidents. This study is meaningful as there is few previous study on this topic although the need for legal review about this is growing and several bills are being proposed. It is expected that the results of this study can be utilized as basic data for enactment or amendment of the laws and regulations about establishment and management of CCTV in operating rooms.

본 연구는 수술실 간호사를 대상으로 하여 환자안전관리활동의 영향 요인을 규명하고 수술실 간호사의 환자안전관리 활동 증진을 위한 프로그램 개발하기 위한 기초자료 작성을 목적으로 시도되었다. 본 연구의 자료수집은 2022년 9월 26일부 터 10월 8일까지 G광역시와 J도에 근무하는 수술실 간호사 140명을 대상으로 수집하였고, 수집된 자료는 SPSS/WIN 22.0 program을 이용하여, 기술통계, Independent t-test, one-way ANOVA, Pearson‘s correlation, multiple regression 분석을 실시하였다. 본 연구결과 조직몰입, 팀워크, 간호전문직관 및 환자안전관리활동은 각각 서로 유의한 양적 상관관계가 있는 것으로 파악되었다. 수술실 간호사의 환자안전관리활동에 영향을 미치는 요인으로는 간호전문직관(β=.43, p<.001), 팀 워크(β=.31, p<.001), 인증평가 경험(β=.16, p=.008) 순으로 나타났으며, 총 설명력은 46.9%였다. 따라서 수술실 간호사의 간호전문직관, 팀워크, 인증평가에 대한 경험을 향상시켜 환자안전관리활동을 증진시킬 수 있는 방안이 필요하다.

This study aimed to create basic data to identify factors influencing patient safety activities for operating room nurses and to develop a program to promote patient safety activities of operating room nurses. The data collection of this study was collected from September 26 to October 8, 2022, from 140 operating room nurses working in G Metropolitan City and J Province. Data were analyzed using SPSS/WIN 22.0, employing descriptive statistics, independent t-tests, one-way ANOVA, Pearson correlation, and multiple regression. As a result of this study, it was found that organizational commitment, teamwork, nursing professionalism, and patient safety activities had a significant positive correlation with each other. Factors influencing the patient safety activities of operating room nurses were nursing professionalism(β=.43, p<.001), teamwork(β=.31, p<.001), and certification evaluation experience(β=.16, p=.008), in which the total explanatory power was 46.9%. There is a need for a plan to improve the experience of nursing professionalism, teamwork, and certification evaluation of operating room nurses to enhance patient safety activities.

The Impact of Operating Room Nurses' Attitudes toward Wearing Personal Protective Equipment, Physical Discomfort, and Obsession with COVID-19 on Perceived Psychosocial Stress during the COVID-19 Pandemic: A Cross-sectional Study

Purpose: This study describes the attitudes toward Personal Protective Equipment (PPE), physical discomfort with wearing PPE, obsession with Coronavirus Disease 2019 (COVID-19), and perceived psychosocial stress among operating room nurses during the COVID-19 pandemic and identifies factors affecting perceived psychosocial stress. Methods: This study used a descriptive, cross-sectional design to evaluate physical discomfort experienced when wearing PPE, attitudes toward PPE, and obsession with COVID-19. We collected the data between December 2020 and January 2021. The participants were 127 nurses who worked in operating rooms at three hospitals in South Korea. Perceived psychosocial stress was assessed using the Psychosocial Well-being Index Short Form. Data were analyzed using descriptive statistics, the independent t-test, one-way analysis of variance, Pearson correlation coefficients, and multiple linear regression. Results: Having a bachelor's degree (β=.28, p=.031), attitude toward PPE (β=-.22, p=.011), experiencing shortness of breath while wearing PPE (β=.26, p=.008), and obsession with COVID-19 (β=.23, p=.006) were identified as factors affecting the psychosocial stress of nurses who wore PPE and participated in surgery. Conclusion: Nurses who had a negative attitude toward PPE, experienced shortness of breath while wearing it, and had a high level of obsession with COVID-19 had higher psychosocial stress. To reduce the psychosocial stress of nurses who wear PPE and participate in surgery, we suggest research on education that can reduce repetitive thinking about infectious diseases, such as obsession with COVID-19. Additionally, support is needed to improve positive attitudes toward PPE and alleviate physical discomfort.

Influence of Person-Centered Perioperative Nursing and Patient Safety Competency on Patient Safety Management Activities among Operating Room Nurses: A Descriptive Survey Study

Purpose: This descriptive survey study aimed to identify the influences of person-centered perioperative nursing and patient safety competency on patient safety management activities among operating room nurses. Methods: Data were collected from June 14 to July 14, 2021, covering 158 operating room nurses working at six general hospitals. Data were analyzed using hierarchical multiple regression to identify the influencing factors of patient safety management activities. Results: The mean scores of person-centered perioperative nursing, patient safety competency, and patient safety management activities were 3.86, 3.88, and 4.55, respectively. These scores increased with increased age and clinical experience. Person-centered perioperative nursing (β=.22, p=.014) and patient safety competency (β=.22, p=.014) influenced patient safety management activities. Conclusion: Therefore, to improve the patient safety management activities of operating room nurses, an efficient nursing work environment must be created along with systematic and continuous education and programs to enhance person-centered perioperative nursing and patient safety competency.

A Study on the Effects of Ankle Pump Exercise in Reducing Lower Limbs Edema and Pain of Operating Room Nurses

Purpose: The purpose of this study was to identify the effects of Ankle Pump Exercise on lower limb edema and pain of operating room nurses. Methods: This research was arranged on the basis of the nonequivalent control group pre-post test design. The participants were 41 nurses working in the operating room in ‘S’ hospital in Seoul and ‘H’ hospital in Guri City, Korea. Data were collected from August 20 to September 30, 2012. Edema of the lower extremities was measured in terms of the girths of ankles and calves. Pain of the lower extremities was measured in terms of VAS (Visual Analogue Scale) for pain. The data were analyzed by frequency, percentage mean, and standard deviation, using the chi2(p) and t-test, repeated measures ANOVA, and a paired t-test. The SPSS Win 20.0 program was used for analysis. Results: Hypothesis 1 ? “There are significant differences in lower extremity edema between the experimental group and the control group.” - was partially supported. Hypothesis 2 ? “There are significant differences in lower extremity pain between the experimental group and the control group.” - was supported. Conclusion: The results of this study demonstrate positive practical effects of the Ankle Pump Exercise in reducing the circumference of both ankles, reducing the circumferential edema of both calves, and alleviating the pain of nurses in the operating room. Based on these findings, application of the Ankle Pump Exercise can be considered as an effective method to prevent nurses’ health problems in the lower extremities that can be induced by their work during surgical operations.

Radiation Exposure Monitoring of Operating Room and Endoscopy Nurses at a Singapore Public Hospital KCI 등재 SCOPUS

Background: Healthcare workers’ radiation safety during fluoroscopy-guided procedures remains a critical occupational health concern, particularly for nursing staff with intermittent exposure. With evolving healthcare practices, increasing procedural volumes, and technological advancements in fluoroscopic equipment, there is a need to evaluate whether continuous personal radiation dose monitoring is necessary for occasionally exposed nursing staff in operating rooms and endoscopy suites. Materials and Methods: A prospective observational study was conducted at a Singapore public hospital from July to December 2024. Eighty nurses from operating theatre and endoscopy departments were equipped with a personal dosimeter to record their dose exposure. A secure digital platform with QR code integration enabled immediate exposure logging via participants’ mobile phones, capturing 1,512 exposure data from various procedures. Radiation doses were measured bimonthly by Singapore’s National Environment Agency. Results and Discussion: Analysis revealed significant variations in the median of total exposure time across specialties, with participants specializing in vascular having the highest median exposure (4.4 hours), followed by endoscopy (2.8 hours), urology (0.3 hours), and orthopedics (0.2 hours). Despite these variations, total effective doses remained consistently low across participants from all specialties below the study period threshold of 1 mSv. The absence of a strong correlation between exposure time and effective dose suggests the effectiveness of current radiation protection measures. Cost-benefit analysis indicated potential savings of approximately $60,000 annually through reduced subscription fees, in addition to administrative man-hour savings, which would have been required for dosimeter management and dose reporting for a cohort of 315 staff. Conclusion: This study demonstrates that occasional participation in fluoroscopy-guided procedures results in minimal radiation exposure for nursing staff, supporting the discontinuation of routine radiation monitoring while maintaining periodic reviews. The findings demonstrate that current radiation protection protocols effectively maintain exposure levels well within safety limits.

목적: 본 연구의 목적은 수술실 간호사를 대상으로 수술실 간호과오에 미치는 영향요인을 파악하기 위함이다. 방법: 2021년 수술실에서 1년 이상 근무하고, 현재 수술간호 업무를 수행하는 125명을 대상으 로 데이터를 수집했다. 수집된 자료는 SPSS WIN 27.0 program을 사용하여 분석하였다. 결과: 수술실 간호과오에 영향을 미치는 요인은 직위(=-0.18, p=.036), 수술실 경력(=-0.31, p=.006), 업무수행능력( =-0.24, p=.020), 피로(=0.28, p<.001)였으며, 직위가 일반 간호사일수록, 수술실 경력, 업무수행능력이 낮아질수록 모두 수술실 간호과오에 유의한 음의 영향을 미치는 것으로 나타났다. 또한, 피로가 높아질수 록 수술실 간호과오에 유의한 양의 영향을 미치는 것으로 나타났다. 이들 요인들의 수술실 간호과오에 대한 설명력은 48.5%였다. 결론: 이상에서 수술실 경력, 피로, 업무수행능력, 직위가 수술실 간호사의 간호과오 영향요인으로 확인되었다. 수술실 간호사의 간호과오를 낮추기 위해 수술실 경력을 늘리고 피로도를 낮추며 업무수행능력을 높일 수 있는 중재가 필요함을 확인하였다.

Purpose: This study was to identify the influencing factors of Nursing Malpractice among Operating Room nurses. Methods: Data were collected from 125 nurses who had worked in the operating room for more than one year in 2021 and were currently practicing surgical nursing. The collected data were analyzed using SPSS WIN 27.0 program. Results: The factors affecting were position (β=-0.18, p=.036), OR experience (β=-0.31, p=.006), work performance (β=-0.24, p=.020), and fatigue (β=0.28, p<.001), and the more general nurses, OR experience, and lower work performance all had a significant negative effect on OR Nursing Malpractice In addition, higher fatigue had a significant positive effect on OR Nursing Malpractice. The explanatory power of these factors for OR Nursing Malpractice was 48.5%. Conclusion: experience in the OR, fatigue, job performance, and position were identified as factors affecting Nursing Malpractice in OR nurses. Interventions to increase OR experience, decrease fatigue, and increase job performance are needed to reduce Nursing Malpractice among OR nurses.

Emergency surgery in COVID-19 confirmed patients in hospitals without a negative-pressure operating room

Background: Confirmed coronavirus disease 2019 (COVID-19) patients sometimes require emergency surgery, but protecting healthcare workers and other patients from infection is a high priority. This report describes two cases where surgeries were performed in a hospital without a negative-pressure operating room. Case: Two patients diagnosed with COVID-19 required emergency surgery in a negative-pressure isolation room. Two operating rooms were designated an operating room and an anteroom, and the central air conditioning system was turned off. The disinfection exhaust fan of the operating room was used to create a negative-pressure environment (–1.0 Pa). Conclusions: Performing surgery on COVID-19 patients remains challenging in hospitals not specialized for infection. It is necessary to share experiences between hospitals, and changing the pressure inside the operating room from positive to negative pressure with a simple maneuver is a good option for protection.

수술실 간호사의 수술 중 인수인계 실태

Purpose: Operating room nurses’ hand-off during surgery is inevitable, and it is very important to ensuring continuity of nursing care. This study aimed to identify the status of operating room nurses’ intraoperative hand-off. Methods: This study used a cross-sectional descriptive survey. The data were collected through structured questionnaire from March 12 to April 30, 2018. Participants were 251 operating room nurses from 6 university hospitals. Results: The mean scores of importance and appropriateness of and satisfaction with hand-off were 4.82±0.39, 3.65±0.56, and 3.32±0.50 respectively. During the operation, nurses need to hand-off and assist the surgeon at the same time, which was the important cause of the inappropriate hand-off. The importance of hand-off had a significant positive correlation with appropriateness (r=.17, p=.002) and satisfaction (r=.18, p=.004), and there was significant positive correlation between appropriateness and satisfaction (r=.43, p<.001). In addition, the recipient’s work difficulty had a significant negative correlation with the appropriateness (r=-.14, p=.005) and satisfaction (r=-.35, p<.001). Conclusion: A standardized hand-off method is essential to ensure the continuity of operative nursing and improve the nursing practice. Future research is needed to examine the effectiveness of the intraoperative hand-off protocol.

수술실 간호사의 건강증진행위와 관련요인

Purpose: This study aimed to investigate the level and related factors related to health-promoting lifestyle in operating room nurses in Korea. Method: A cross-sectional descriptive survey design was employed. The data were collected using questionnaire for three weeks in December 2018 from 110 operating room nurses working for more than six months in a general hospital located in Seoul. The Health Promoting Lifestyle Profile-Ⅱ, The Korean Version of the Practice Environment Scale of Nursing Work Index, and Operating nurse’s Job stress Factor Scale were used. Results: Significant relationships were observed between health promoting lifestyle and educational level(p=.025), wearing a lead apron for protection from radiation during the operation(p<.001), work satisfaction(p=.016), and fatigue related to work(p=.006). Also significant correlations were identified between nursing work environment and health promoting lifestyle. However, the health promoting lifestyle was not statistically different based on occupational stress(p=.365). In multiple linear regression analyses, the level of health promoting lifestyle found to be higher in subjects who did not wear a lead apron for protection from radiation(p=.017), and who had more positive perception of the nursing work environment(p=.034). Conclusion: In order to increase health promoting lifestyle of operating nurses, the strategies to improve the nursing work environment are essential.

폭력을 경험한 수술실 간호사의 대처, 스트레스와 이직의도

Purpose: The aim of this study was to identify the correlations between coping, stress, and turnover intention in operating room nurses who experienced violence. Methods: The study was conducted between April and December 2015 on 269 nurses who worked in operating rooms. The tools used in the study included violence measurement tool, Psychosocial Well-being Index (PWI-SF), Coping Strategy Indicator (CSI), and Turnover intention Scale. The data were analyzed using descriptive statistics, t-test, ANOVA and Pearson's Correlation Coefficient through the IBM SPSS Statistics 23 program. Results: Verbal abuse or physical threat appeared frequently as experience of violence in operating room nurses. The operating room nurse experienced high stress (87%) and used a lot of avoidance coping (23.18±5.48). There was a difference in stress (t=-2.28, p=.023) and seeking support (t=3.46, p=.001) depending on the experience of reporting violence. Violence experience was positively correlated with seeking support (r=.20, p<.001), problem solving (r=.27, p<.001), avoidance (r=.28, p<.001), and turnover intention (r=.28, p<.001). Conclusion: Because violence experiences are related to coping and turnover intention, and coping is correlated with stress level, and turnover intention, it is necessary to reduce stress and turnover intention through violence prevention in order to create safe environment and improve quality of nursing.

수술실 간호사의 방사선 방어행위의 영향 요인

Purpose: This study was designed to identify knowledge, attitude, environment, and self-efficacy among perioperativenurses in terms of radiation protection and to describe those factors affecting radiation protection behaviors. Methods: The sample was comprised of 128 perioperative nurses who agreed to participate in this descriptivestudy. Data were collected from a self-administered questionnaire and were analyzed using the t-test, analysisof variance, Mann-whitney U test, Kruskal-Wallis test, Pearson’s correlation, and multiple regression analyses. Results: Radiation protection behaviors were significantly different by participant’s gender, length of career asa perioperative nurse, educational level and prior experience with radiation protection education. Radiation protectionbehaviors were significantly correlated with radiation protection environment and self-efficacy in terms ofradiation protection behaviors. In the multiple linear regression models, radiation protection environment andself-efficacy on radiation protection behaviors were statistically significant predictors of radiation protection behaviors,which accounted for 50.3% of variance in dependent variable. Conclusion: This study concluded that radiationprotective environment in operating room is important to promote radiation protective behaviors and radiation safetymanagement program to enhance self-efficacy is highly recommended.

족욕요법이 수술실 간호사의 하지 부종, 스트레스 및 피로에 미치는 효과

본 연구는 족욕요법이 수술실 간호사의 하지부종, 스트레스, 피로에 미치는 효과를 규명하기 위함이다. 방법: 본 연구는 무작위배정 대조군 전후 시차설계이다. 자료수집은 2013년 8월에서 10월까지이며 대상자는 울산광역시 대학교병원의 수술실 간호사 50명을 대상자로 하였다. 실험군 25명, 대조군 25명. 실험군은 40℃ 물의 온도로 족욕기를 사용하여 20분 동안 발이 복사뼈 위까지 잠길 정도로 실행하였다. 족욕요법 횟수는 1회/일, 4일/주, 3주간 총 12회 실시하였다. 결과: 족욕요법 실험처치 후 종아리 하지부종, 신체적 스트레스, 피로가 유의한 감소를 보였다. 결론: 본 연구는 족욕요법이 수술실 간호사의 하지부종, 스트레스, 피로를 감소시키는 결과를 보였다. 따라서 족욕요법은 수술실 간호사의 하지부종, 스트레스, 피로를 감소를 시키는 편안하고 경제적인 중재방법이다.

nurses’ lower extremities edema, stress, and fatigue. Methods: This study used a randomized control group non-synchronized design. The data were collected from August to October 2013, and a total of 50 OR nurses in an university hospital in U Metropolitan City participated in the study: 25 nurses for the experimental group and 25 for the control group. The experimental group received foot bath in which feet were soaked in 40C° water and immersed up to ankle line for 20 minutes per a day for 12 times. Results: There were significant decrease in the calf edema, physical stress, and fatigue between pre and post foot bath therapy. Conclusion: The findings of this study show that foot bath had the effect in decreasing lower extremities edema, stress, and fatigue among OR nurses. Foot bath therapy, therefore, is suggested as a comfort and easy-to-use method in clinical setting to reduce OR nurses’ lower extremities edema, stress, and fatigue.

Purpose: The purpose of this study was to develop a performance tool to evaluate operating room nurses. Method: In the first stage, 59 preliminary items were selected after two focus group meetings were carried out using as background a reference review. At this stage 59 items were developed. Next, the selected preliminary items were modified by an expert group of nurses with over 10 years experience, nurses holding doctoral degrees and professors in nursing. In the third and fourth stage, these items were analyzed by the expert group to determine validity and tested by hospital nurses with over 10 years experience to determine reliability Results: For the final tool, the 59 items, 47 items for the performance evaluation dimension and 12 items for the competence evaluation dimension were found to be valid and reliable. Cronbach's α was .9622, a promising alpha value for the total instrument. Conclusion: This tool is an efficient performance evaluation instrument providing meaningful feedback for operating room nurses.

Purpose: The purpose of this study was to assess the frequency and characteristics of occupational blood and body fluid exposure for operating room personnel during surgical procedures and identify risk factors. Methods: A self-report questionnaire was used to survey 519 operating room personnel who participated in 132 surgeries in an acute care hospital in Seoul between April 12 and 20, 2006. Results: The response rate was 82% (530/646) and analyzed total 519 except for improper 11 questionnaires. Exposure rate was 45.5% (60/132) during operations and 20.4% (106/519) for operating room personnel. By job categories, the exposure rate was as follows; anesthesiologists and nurses 31.2% (44/141), operators and assistants 22.1% (32/145), circulating nurses 12.0% (11/92), scrub nurses 11.4% (14/123). The mucous membrane-eye exposure with a high risk of transmitting infection was 6.6% (7/106) and the most frequent route of exposure was blood and body fluid touching unprotected skin. In multivariate analysis, the risk factors of exposure were services, duration of operation, a patient who had a peripheral arterial catheter line, and job category. Conclusion: Educational programs and appropriate use of personal protective equipment are necessary to prevent operating room personnel from being exposed to blood and body fluid.

간호수가 산정을 위한 상대가치를 이용한 수술실 간호행위 간호강도 분석

본 연구의 목적은 수술실 간호행위를 분석하고, 상대가치에 근거하여 각 간호행위의 간호강도를 측정하는 것이다. 방법 : 본 연구의 상대가치 측정 방법은 Hsiao et al에 기초했다. 첫 번째 단계로 간호행위를 확인하였으며, 두 번째 단계로 간호행위에 대한 간호강도를 기술적 노력, 정신적 노력, 스트레스로 측정하였다. 결과 : 99개 간호행위에 대한 상대가치 점수는 1337.78부터 300.00의 범위를 나타냈다. 흉부외과 수술보조행위, 정형외과 수술보조행위, 자격증 취득하기가 가장 간호강도가 높은 것으로 나타났다. 외과적 손씻기, 외과적 가운입기, 외과적 장갑끼기, 수술 모자와 마스크 착용하기는 가장 간호강도가 낮게 나타났다. 결론 : 수술실 간호사의 간호행위는 별도로 보상하지 않으며, 일반적으로 의사의 수가에 포함되어 있다. 앞으로, 간호수가를 추정하여 수술실 전체 운영수식에 대한 기여도를 보여주어야 한다.

Purpose: The purpose of this study was to perform an operating room nursing activities analysis and estimate nursing intensity of each nursing activity based on the Relative Value Scale (RVS). Methods: The methodology for this study of RVS was based on the work of Hsiao et al. The first stage was to identify nursing activities and the second to measure intensity of nursing activities including technical skill, mental effort, and stress. Results: Calculation of the RVS for 99 nursing practices showed a score range from 300.00 to 1337.78. CS operation assistant, OS operation assistant, and obtaining certification had high nursing intensity. Surgical hand washing, putting on surgical gowns, surgical gloves and surgical caps and mask had low nursing intensity. Conclusion: The activities of operating room are not compensated separately but reimbursement is usually included in physician fees. In the future, an estimation of nursing cost should show the nursing contribution rate to total operation revenue.

Purpose: This study aimed to examine the relationships between grit, retention intention, and the work performance of operating room nurses. Methods: Data were collected from 140 operating room nurses at two university hospitals in Daegu, South Korea. The data were analyzed using t-tests, ANOVA, multiple regression, and mediation analyses (SPSS/WIN 29.0). Results: There were significant differences in grit according to the gender (t=-2.09, p=.038), education (F=4.13, p=.018), health status (F=4.17, p=.017), operating room career (F=3.19, p=.044), and job satisfaction (F=4.98, p=.008). The work performance of operating room nurses was the most strongly influenced by grit (t=2.56, p=.011), followed by total clinical career (t=2.31, p=.022), surgical departments involved (t=2.29, p=.024), and job satisfaction (t=2.04, p=.043), collectively explaining 13.7% of the variance (F=5.426, p<.001). Grit exerted the greatest impact on surgical nursing performance (β=.221, p=.011) and had a full mediating effect on the relationship between retention intention and work performance. Conclusion: These findings indicate that grit serves as a key mediating factor between retention intention and work performance, exerting the greatest influence on the work performance of operating room nurses. Improving operating room environments, job satisfaction, and grit through educations, may strengthen retention intention and performance among operating room nurses.

Purpose: 연구의 목적은 수술실 간호사를 대상으로 건강신념모델의 위험지각에 근거한 혈액매개감염 예방행위 관련 요인을 조사하기 위함이다. 위험지각의 하부요인은 지각된 민감성, 지각된 장애성, 지각된 유익성, 지각된 심각성으로 구성되었다. Methods: 자료수집은 2016년 5월 11일부터 6월 30일까지 대전, 서울에 소재한 4개 종합병원 수술실 간호사 121명을 대상으로 하였다. Results: 대상자의 평균 연령은 31.2세, 수술실 경력은 평균 7.9년 이었다. 혈액매개감염 지식은 평균 13.15점이었고 지각된 민감성 3.76점, 지각된 장애성 3.70점, 지각된 유익성 3.95점, 지각된 심각성 4.64점이었다. 혈액매개감염 예방행위에 대한 상관관계 분석 결과, 지각된 유익성과 감염예방행위 간에는 양의 상관관계가 나타났으나(p=.010), 지각된 민감성(p=.009)과 지각된 장애성(p=.012)은 감염예방행위와 음의 상관관계가 나타났다. 위계적 회귀분석을 실시한 결과 연령(β=.18, p=.038), 지각된 유익성(β=.20, p=.030), 지각된 민감성(β=-.25, p=.005), 지각된 장애성(β=-.18, p=.042)이 유의한 영향요인으로 나타났으며, 이 모형의 설명력은 16%이었다(F=4.85, p<.001). Conclusion: 혈액매개감염 예방행위를 높이기 위하여 수술실의 장비나 기구, 환경관리에 대한 구체적이고 적절한 관리 방안에 대한 교육이 필요하며, 병원조직차원에서 근무환경과 관련된 지지가 이루어져야 할 것이다.

Purpose: The purpose of this study was to identify the factors related to blood-borne infection prevention behaviors based on the risk perception of the health belief model among operating roomnurses. Risk perception factors included perceived susceptibility, barriers, benefits, and perceived severity. Methods: Data were collected from 121 operating room nurses working in four different hospitals in Daejeon and Seoul from June 30 to May 11, 2016. Results: The mean age was 31.2 years, and the average years of clinical experience in operating room was 7.9 years. The mean score of knowledge was 13.15. The mean score of perceived susceptibility, barrier, benefit, and perceived severity were 3.76, 3.70, 3.95, and 4.64, respectively. Blood- -borne infection prevention behaviors had positive correlation with perceived benefits (p=.010), but negative correlation with sensitivity (p=.009) and barrier (p=.012). The hierarchical regression model on infection prevention behavior was statistically significant (F=4.85, p<.001). The sixteen percent of variance in behavior was explained by age (β=.18, p=.038), perceived benefit (β=.20, p=.030), perceived susceptibility (β=-.25, p=.005), and perceived barrier (β=-.18, p=.042). Conclusion: In order to increase infection prevention behaviors among operating roomnurses, there is a need for developing specific education programfocusing on appropriate management of equipment, instruments, and environment in operating room. In addition, support from the hospital organization level need to be provided as well.

Purpose: This study examined the effects of ethical sensitivity, organizational silence, and self-efficacy on the intention to report near misses among operating room nurses, based on the theory of planned behavior. Methods: A descriptive cross-sectional survey was conducted to assess the relationships among these variables and identify factors influencing reporting intention. Participants were 179 operating room nurses working in general and university hospitals in Seoul and Gyeonggi Province. Structured questionnaires included the Ethical Sensitivity Scale for Clinical Nurses, Hospital Nurses Silence Behavior Scale, General Self-efficacy Scale, and Intention to Report Near Miss Scale. Data were analyzed using descriptive statistics, t-tests, ANOVA, Pearson’s correlation, and hierarchical multiple regression. Results: Hierarchical regression showed that use of a near-miss reporting system (β=.29, p=.018), organizational silence (β=-.28, p=.025), and ethical sensitivity (β=.25, p=.047) significantly influenced reporting intention, explaining 23.0% of the variance. Conclusion: Enhancing ethical awareness, reducing silence, and improving nurses’ experience with reporting systems are key strategies. These findings may inform interventions and policy development to foster a patient safety culture in operating rooms.
