The purpose of this study was to assess the feasibility of task-oriented arm training for chronic hemiparetic stroke patients. The experimental design in this study was the pre-test and post-test with control group for 4-week intervention. Thirty patients with chronic hemiparetic stroke were recruited from 2 rehabilitation units. The subjects were divided randomly into experimental and control groups. The experimental group conducted task-oriented approach, involving 3 subparts of upper extremity activities, and the control group involved in the general upper extremity exercises. Functional movements of the upper extremities were assessed using clinical measures, including the Fugl-Meyer Assessment-Upper Extremity Section, Box and Block Test, and Action Research Arm Test. The score of Fugl-Meyer Assessment showed greater increases in the experimental group than in the control group after training. The improvement in Box and Block Test between pre-test and post-test measurements was significantly greater after task-oriented arm training compared to general upper extremity exercises. Action Research Arm Test scores also improved after task-oriented arm training compared to exercises in the control group. The task-oriented arm training improves the gross and fine motor activities and encouraging the use of the paretic arm through activity dependent intervention expedites the recovery of functional activities in the upper extremities for chronic hemiparetic stroke.

Purpose: This study examined the effects of task-oriented training combined with deep neck and abdominal muscle activation on the gross motor function and balance in children with cerebral palsy (CP). Methods: Twelve children with CP (Gross Motor Function Classification System levels I-IV; aged 6-18 years) were assigned randomly to either an experimental group (n=6) or a control group (n=6). The experimental group received 30 minutes of neurodevelopmental treatment (NDT), followed by 10 minutes of deep neck and abdominal muscle activation using a pressure biofeedback unit and 10 minutes of task-oriented training incorporating muscle activation. The control group received 30 minutes of NDT and 20 minutes of task-oriented training without deep muscle activation. Both groups trained for 50 minutes, twice per week, for six weeks. The gross motor function was assessed using the Gross Motor Function Measurement (GMFM), and static and dynamic balance were evaluated using the left-right weight-bearing ratio in the seated Limit of Stability (LOS) and the seated LOS Surface Area. Results: Both groups showed significant improvements in the GMFM total scores and in the walking, running, and jumping domain (p<0.05). The experimental group showed significantly greater improvements in the GMFM total scores and in the standing domain compared to the control group (p<0.05). In balance assessments, both groups showed improvements in the static and dynamic trunk balance (p<0.05), but the experimental group showed significantly greater improvements in the total and posterior movement areas in the dynamic balance (p<0.05). Conclusion: Task-oriented training that incorporates deep neck and abdominal muscle activation in NDT-based interventions effectively enhances the gross motor function and trunk balance in children with CP.

Objective: This study was to investigate the effects of Task-oriented training for Gross Motor Function Measure (GMFM), gait and balance function in cerebral palsy. Design: Randomized controlled trials. Methods: Twenty four subjects were recruited by means of a convenience sampling from Kangseo-Gu G rehabilitation center. Subjects were 24 inpatients and were randomly divided into a task-oriented training group and a conventional group. Twelve patients were experimental group who executed the task-oriented training (5 times/wk) for 4 weeks. The task-oriented program mainly focused on the capabilities of independent walking, with the angle of inclination set at 0 degrees and walking at a self-selected comfortable speed. In addition, balance training included the one-legged standing with weight-shifting and task-oriented training. Twelve patients were control group who executed only general conventional therapy (5 times/wk) for 4 weeks. All subjects were evaluated about the motor function, gait and balance function. Subjects have conducted the measured variables, GMFM, GAITRite, PDM Multifunction Force Measuring Plate after treatment. Results: There was statistically significant increase of Gross Motor Function Measure scores of the experimental group and control group after 4 weeks (p<.05). There was statistically significant increase of gait and balance function of the experimental group after 4 weeks of task-oriented training (p<.05). The experimental group showed a significantly improvement in GMFM, gait, and balance compared to the control group (p<0.05). Conclusions: This study proved that task-oriented training after stroke can improve Gross Motor Function Measure, gait and balance. Thus this study can suggest that task-oriented training for gross motor function, gait and balance be effective on the cerebral palsy.

Purpose: This study examined the effects of unpredictable task-oriented training on electroencephalogram (EEG) activity and balance in subacute stroke patients, compared with predictable task-oriented training and conventional physical therapy. Methods: Thirty subacute stroke patients were randomly assigned to an unpredictable task-oriented training group (UG, n=10), a predictable task-oriented training group (PG, n=10), or a control group (CG, n=10). The UG performed five task-oriented exercises with random directional changes and verbal speed cues, whereas the PG performed the same exercises in a consistent and predictable manner. The CG received conventional physical therapy only. All interventions were conducted for 5 weeks, three times per week, for 30 minutes per session. EEG was recorded using SmartingMobi with six channels (Fp1, Fp2, F3, F4, P3, P4) to measure relative alpha and beta waves. Balance was assessed with the BT-4 force platform for sway area and sway length under eyes-open and eyes-closed conditions. Results: The UG showed significantly greater increases in relative beta activity and greater decreases in relative alpha activity across brain regions compared with the PG and CG (p<0.05). The UG also demonstrated superior improvements in static balance, with larger reductions in sway area and sway length in both visual conditions (p<0.05). Conclusion: Unpredictable task-oriented training produced greater enhancements in brain activity and balance than predictable training or conventional therapy. These findings suggest that unpredictability within task-oriented practice may facilitate neuroplasticity and motor learning in subacute stroke rehabilitation.

PURPOSE: The purpose of this study is to examine how task-oriented training focused on lower extremity strengthening can affect mobility function and postural stability. METHODS: The study's subjects included 10 children with cerebral palsy: 7 girls and 3 boys between the ages of 4 and 9 whose Gross Motor Functional Classification System (GMFCS) level was I or II. Their functional mobility was gauged using the Gross Motor Function Measurement (GMFM), and their postural stability was evaluated using a force platform. Participants received task-oriented training focused on lower extremity strengthening for 5 weeks. The study used a paired t-test to investigate the difference in mobility function and postural stability of children with cerebral palsy before and after the lower extremity strengthening exercise. RESULTS: The GMFM dimensions D (standing) (p<.02) and E (walking) (p<.001) improved significantly between the pre-test and post-test. A significant increase in the posturographic center of pressure (CoP) shift and surface area of the CoP were found overall between the pre-test and post-test (p<.001). CONCLUSION: The present study provides evidence that an 8-week task-oriented training focused on strengthening the lower extremities is an effective and feasible strategy for improving the mobility function and postural stability of children with cerebral palsy.

The purpose of this study was to investigate the effects of the task-oriented training according to the application time with the change of motor and cognition function. Focal ischemic brain injury was produced in Sprague-Dawley rats (20 rats, $250{\pm}50$ g) through middle cerebral artery occlusion (MCAo). Before MCAo induction, all rats were trained in treadmill training and Morris water maze training for 1 week. Then they were randomly divided into groups: Group I : MCAo induction ($n_1$=5), Grop II: the application for simple treadmill task training after. MCAo induction ($n_2$=5). Group III: the application for Morris water maze cognitive task training after MCAo induction ($n_3$=5). Group IV: the application for progressive treadmill task training and Morris water maze cognitive task training after MCAo induction ($n_4$=5). Modified limb placing tests (MLPTs) and motor tests (MTs) were performed to test motor function and then Morris water maze acquisition test (MWMAT) and Morris water maze retention test (MWMRT) were performed to test cognitive function. For MTs, there were significant interactions among the groups with the time (p<.001). Group IV showed the steeper increasing pattern than those in other Groups on the 7th and 14th day. For MLPTs, there were significant interactions among the groups with the time (p<.001). The scores in Group III. IV had showed the more decreasing pattern than those in Group I, II since the 7th day and 14th day. For MWMAT, there were significant interactions among the groups with the time (p<.001). Group II found the Quadrant circular platform showed the steeper decreasing pattern than that in Group I on the 9th, 10th, 11th and 12th day. Group III. IV found the quadrant circular platform showed the slower decreasing pattern than that in Group I, II, For MWMRT, there were significant differences among the four groups (p<.001). The time to dwell on quadrant circular platform in Group IV on the 13th day was the longest compared with other groups. These results suggested that the combined task training was very effective to improve the motor and cognition function for the rats affected on their focal ischemic brain injury.

Background: Children with spastic cerebral palsy (CP) often undergo orthopedic surgery for muscle contractures, followed by rehabilitation to restore mobility. Evidence on intensive rehabilitation with task-oriented therapy (TOT) after low-burden orthopedic surgery is limited. Objects: This study examined the effects of a 16-week intensive rehabilitation program incorporating TOT in a child with spastic CP who received hamstring and adductor lengthening. Methods: A single-case experimental design was used. Assessments included range of motion (ROM), the Gross Motor Function Measure (GMFM), and the Childhood Health Assessment Questionnaire (CHAQ). The participant received structured rehabilitation with TOT 5 times per week for 16 weeks. Results: Improvements were observed in hip abduction ROM, popliteal angle, GMFM, and CHAQ. A previously unachievable task, reaching a walker and walking to a sofa, became independently achievable, representing a meaningful gain in functional mobility. Conclusion: Intensive rehabilitation with TOT after low-burden orthopedic surgery may yield clinically meaningful improvements in motor function and daily activity performance, and support greater independence in children with CP.

Purpose: The purpose of this study was to investigate the effects of a task-oriented upper arm exercise performed in a sitting position on either an unstable support surface or a stable support surface for children with cerebral palsy. Methods: We prospectively evaluated 18 children with cerebral palsy. Eight subjects were randomly assigned to each of the stable and unstable support surface groups. We performed the upper arm exercise three times a week for 6 weeks. To confirm the effects of the intervention, the berg balance scale test, modified functional reaching test (MFRT), timed up and go test (TUG), and Jebsen-Taylor hand function test were conducted before and after the study. Results: Significant differences were observed in MFRT and TUG between the experimental and control groups (p<0.05). In the Jebsen-Taylor hand function test, there were significant differences between the groups for the items picking up small objects, stacking checkers, lifting large light objects, and lifting large heavy objects (p<0.05), but not for writing and stimulation of feeding. Significant differences were observed between the groups in items of card turning, lifting large light objects, and lifting heavy objects. Conclusion: The purpose of this study was to evaluate the effectiveness of a task-oriented upper extremity exercise program for dynamic balance and hand function performed in a sitting position with either stable or unstable support by cerebral palsy patients. There were improvements in the two groups, but performing the exercise while sitting on an unstable support surface had a greater effect on dynamic balance and hand function than exercise while sitting on a stable supporting surface. The results of this study can be used to improve the daily lives of cerebral palsy patients.

Objective To explore the amount of practice and progression during task-oriented circuit training (TOCT) in chronic stroke survivors; to test the use of pedometers and observation-based measures in detecting step activity; to verify the possible correlation between step activity and locomotor function improvements. Methods Six community-dwelling chronic stroke survivors underwent 10 TOCT sessions (2 hours/each) over 2 weeks in which they were trained both on a treadmill and on six task-oriented workstations (W1?W6). During the sessions, they wore a piezoelectric pedometer and step activities were recorded. Outcome measures were as follows: % of activities during which pedometers worked properly; pedometer-based measures (total step counts, treadmill steps, workstation steps? total and W2,W3,W5,W6); observation-based measures (number of repetitions in task W1 and W4); walking speed changes measured by the 10-m walking test (10MWT) and walking endurance changes (6-minute walking test) after TOCT.Results During TOCT sessions (n=57), activities were recorded through pedometer-based measures in 4 out of the 6 patients. The total amount of step activity was 5,980.05±1,968.39 steps (54.29% in task-oriented workstations, 37.67% on treadmill, and 8.03% during breaks). Exercise progression was highlighted significantly by observational measures (W1, W4). A positive correlation was observed between increased gait speed and observational stair step repetitions progression (W1) (r=0.91, p=0.01) or pedometer-based tandem exercise step progression (W3) (r=0.98, p=0.01).Conclusion TOCT can be considered a high-intensity, progressive intervention to restore locomotor function in chronic stroke survivors. Pedometer-based measures might help in quantifying TOCT’s volume of practice; however, further investigations are required.

Objective : This study aimed to provide an overview of task-oriented approaches for children with developmental coordination disorder, including its definition, diagnosis, causes, and assessment, through a literature review. Subject : Eighteen international articles published between January 2000 and September 2023 were analyzed to explore the intervention strategies of Neuromotor Task Training (NTT) and Cognitive Orientation to daily Occupational Performance (CO-OP). The objectives of the interventions mentioned in the reviewed articles were categorized according to the criteria of the Occupational Therapy Practice Framework-4 (OTPF-4). The results showed that NTT more frequently set goals in the area of client factors, while CO-OP set goals only in the area of occupation. This difference in goal setting can be explained by the strategy differences between the two interventions. Conclusion : A broader and deeper understanding of strategies for interventions with task-oriented approaches to developmental coordination disorder can increase awareness of the disorder and provide a more solid basis for evidence-based practice.

Objective : The aim of this study is to suggest evidence to evaluate the effectiveness of a task-oriented approach developed for the treatment of children with developmental coordination disorder. Treatment outcomes are presented using the International Classifications of Functioning, Disability and Health (ICF). Methods : Participants had to be children diagnosed with DCD according to DSM-Ⅳ and aged 0 to 12 years old. Their intervention could be neuromotor task training and cognitive orientation to daily occupational performance. The literature search included the following electronic databases: Medline, Pubmed (January 1997 to November 2007). Results : There were 17 (62.96%) papers on the effectiveness of neuromotor task training, and 12 (44.44%) papers on the effectiveness of cognitive orientation to daily occupational performance. The results of the ICF model presented 14 (51.85%) papers on body structure & function and 13 (48.15%) papers on activity and participation. And there were 6 (22.22%) papers on the effectiveness of body structure & function and 11 (40.74%) papers on the effectiveness of activity and participation. Conclusion : This paper conducted a systematic review of the research literature that reports on the effectiveness of a task-oriented approach developed for the treatment of children with developmental coordination disorder. The results of the review presented greater effect on the activity and participation of the ICF model. There was limit of 8 journals used. But this knowledge is utilized in occupational therapy, and therefore presents evidence of practice.
Mind-Wandering in task-oriented group : An application of The Theory of Interpersonal Behavior
한국경영정보학회 한국경영정보학회 정기 학술대회 디지털플랫폼 성공을 위한 경영정보학의 역할 2023.06 p.1599
Mind-wandering consists of the process of thinking task-unrelated thoughts (TUT) during task performance. In a task-oriented group, the subject and the subject matter of team members’ mindwandering episodes are of critical interest to the team's performance. However, many writers suggested that mind-wandering may be more common in some circumstances than others. Understanding the social-psychological factors that precipitate mind-wandering in a technological setting is necessary because mind-wandering influences numerous outcomes connected with taskoriented groups. Therefore, this paper aims to explore whether certain social-psychological factors predict self-reported engagement behavior in a number of TUT in the assessment of attitudes. Specifically, our objectives are to put in place behavioral experience sampling methodologies and probe-caught methodologies for measuring online mind-wandering. We intend to design a set of virtual teams using laboratory experiments followed by survey. Our study aims to add to the body of research on virtual teams and mind-wandering by determining the relation between attitude, social factors, and emotions of team members and self-reported engagement in TUT.

Purpose: This study aimed to evaluate the effect of task-oriented training on trunk stabilization, coordination of the lower extremities, and dynamic balance of patients with stroke. Methods: Ten patients with stroke were randomly divided into two groups of five patients each who met the selection criteria. The training was conducted five times a week for 60 minutes. The experimental group received task-oriented training and general physical therapy, while the control group received only general physical therapy. The Mann-Whitney U test was used to compare differences between the groups, and the Wilcoxon signed-rank test was used to compare differences between pre- and post-intervention within each group. Results: Trunk stabilization was measured using the trunk impairment scale. Only the coordination assessment on the trunk impairment scale was statistically significant after intervention in the experimental group (p = 0.046) and between the experimental and control groups (p = 0.029). The LEMOCT, figure-of-8 walk test (F8WT), timed up-and-go (TUG) test results were not statistically significantly different in the group after intervention. However, the F8WT and TUG test results showed a statistically significant difference between the experimental and control groups (p = 0.029). Conclusion: The task-oriented training showed positive effects on the trunk stabilization, coordination of the lower extremities, and dynamic balance of the patients with stroke.

Purpose: The purpose of this study was to evaluate the effect of task-oriented exercise and abdominal muscle contraction using functional electrical stimulation (FES) on abdominal muscle thickness and balance of stroke patients. Methods: Ten stroke patients who met the selection criteria were assigned randomly into two groups of five. One group received FES therapy before task-oriented training (experimental group), while the other group received a FES placebo before task-oriented training (control group). The Mann-Whitney U test was used to compare the groups, and the Wilcoxon Signed-ranks test was used to compare differences between the groups before and after intervention. The Mann-Whitney U test was used to compare the rate changes of each item before and after intervention, between the two groups. Results: In the rectus abdominalis and external oblique muscle thickness tests that used ultrasound, there was a statistically significant difference in the experimental group (p<.05),but no significant difference in the control group (p>.05). There was also a significant difference between the groups (p<.05). In the Balance test that used the Berg Balance Scale (BBS) and timed up and go test (TUG), there was a statistically significant difference in the experimental and control groups (p<.05), but there was no significant difference between the groups (p>.05). Conclusions: FES therapy before task-oriented training increases the thickness of abdominal muscles and improves balance abilities.
