과제 지향적 순환 훈련이 뇌졸중 환자의 보행과 신체활동에 미치는 영향 KCI 등재

The aim of this study was to determine effect of a task-oriented circuit training for functional performance after stroke. Subject was 20 outpatients and randomly divided into two group: 10 subjects were experimental group who executed the task-oriented circuit training (3 times/wk) and general physical-occupational therapy(5 times/wk) for 4 week. 10 subjects were control group who executed only general physical-occupational therapy(5 times/wk) for 4 week. All subject were evaluated about 10m walking test, 6 minutes walking test, physical activity scale for individuals with physical disability(PASIPD). Subject have conducted the measurement variable, 10MW, 6MW, PASIPD ,before and after 4week of exercise, respectively. An independent t-test was used to determine the between the experimental group and the control group after 4 week of exercise. While a paired t-test was used to examine the differences in the experimental group before and after the task-oriented circuit training. Results are as follows : There was statistically significant increase of 10m walking test of experimental group after 4 weeks of task-oriented circuit training(p<.05). There was statistically significant increase of 6 min walking test of experimental group after 4 weeks of task-oriented circuit training(p<.05). There was statistically significant increase of PASIPD of experimental group after 4 weeks of task-oriented circuit training(p<.05). There were statistically difference of 10MW, 6MW, PASIPD between experimental group and control group(p<.05). This study proved that task-oriented circuit training after stroke can improve functional performance. Thus, this study suggest that task-oriented circuit training for functional performance be effective on the stroke rehabilitation.

Monitoring Step Activity During Task-Oriented Circuit Training in High-Functioning Chronic Stroke Survivors: A Proof-of-Concept Feasibility Study

Objective To explore the amount of practice and progression during task-oriented circuit training (TOCT) in chronic stroke survivors; to test the use of pedometers and observation-based measures in detecting step activity; to verify the possible correlation between step activity and locomotor function improvements.Methods Six community-dwelling chronic stroke survivors underwent 10 TOCT sessions (2 hours/each) over 2 weeks in which they were trained both on a treadmill and on six task-oriented workstations (W1?W6). During the sessions, they wore a piezoelectric pedometer and step activities were recorded. Outcome measures were as follows: % of activities during which pedometers worked properly; pedometer-based measures (total step counts, treadmill steps, workstation steps? total and W2,W3,W5,W6); observation-based measures (number of repetitions in task W1 and W4); walking speed changes measured by the 10-m walking test (10MWT) and walking endurance changes (6-minute walking test) after TOCT.Results During TOCT sessions (n=57), activities were recorded through pedometer-based measures in 4 out of the 6 patients. The total amount of step activity was 5,980.05±1,968.39 steps (54.29% in task-oriented workstations, 37.67% on treadmill, and 8.03% during breaks). Exercise progression was highlighted significantly by observational measures (W1, W4). A positive correlation was observed between increased gait speed and observational stair step repetitions progression (W1) (r=0.91, p=0.01) or pedometer-based tandem exercise step progression (W3) (r=0.98, p=0.01).Conclusion TOCT can be considered a high-intensity, progressive intervention to restore locomotor function in chronic stroke survivors. Pedometer-based measures might help in quantifying TOCT’s volume of practice; however, further investigations are required.

Purpose:The purpose of present study was to determine effects of a task-oriented circuit training(TOCT) for lower limb on walking ability after stroke. Methods:Twenty one chronic stroke patients participated. Participants were randomly divided into either TOCT group or control group(11 experimental, 10 control). All of participants were in-patients at local rehabilitation centre and had been receiving a traditional rehabilitation program, five days a week. TOCT group have additionally undergone for four weeks, three days a week, the TOCT program but control group was not received any additional program except the traditional rehabilitation program. The 10 m walking test (10MWT), the 2 min walking test (2MWT), the step test (ST) and the figure-8 walking test (F8WT) to measure a walking ability were carried out twice before and after training. Results:After participation in the program, subjects of TOCT demonstrated a significant improvement in the scores of the 10MWT, 2MWT, the ST, the F8WT. The control group had no change on the any tests. After the training, the results to improve significantly in TOCT group compared to post-test of control group were the time of 10MWT and the time and the step of curved walking of F8WT. Conclusion:The present study suggests that the TOCT program may become a useful strategy for enhancing walking ability in the rehabilitation of stroke patients.

뇌졸중 환자의 보행기능과 삶의 질에 대한 과제지향적 순환식 보행훈련과 트레드밀 보행 훈련의 효과 비교: 무작위 대조군 예비연구

Background: Many studies regarding task-oriented training have recently demonstrated functional improvement in patients with post-stroke hemiparesis. The task-oriented approach is very diverse, and chronic stroke patients must have access to a sustained systematic treatment program to enhance their walking ability. Objectives: This study aimed to compare the effects of the task-oriented circuit training and treadmill training on walking function and quality of life in patients with chronic stroke. Methods: Fourteen patients with chronic stroke volunteered for this study. The subjects were randomly divided into a task-oriented circuit training group and a treadmill training group with 7 patients in each. Each training regimen was performed for 30 min a day and 3 days a week for 4 weeks. Assessment tools included the Timed Up-and-Go Test (TUGT), 10-m Walk Test, 6-min Walk Test (6MWT), and the Stroke Impact Scale (SIS). Results: The change in results of the TUGT, 6MWT, and SIS measured prior to and following the training regimens appeared to be significantly different between the two groups (p<.05). In addition, after the intervention, significant differences were found for all parameters in the task-oriented circuit training group and for the TUGT, 6MWT, and SIS in the treadmill training group (p<.05). Conclusion: The findings suggest that task-related circuit training and treadmill training may be helpful to improve walking function and quality of life of patients with post-stroke hemiparesis. Additionally, a task-related circuit training program may achieve more favorable outcomes than a treadmill program.

본 연구는 순환식 과제 지향적 집단훈련이 만성 뇌졸중 환자의 상지 기능과 삶의 질에 미치는 영향을 알아보고자 한다. 경기도 부천 소재의 H 병원에 입원한 20명의 만성 뇌졸중 환자를 대상으로 실시하였다. 중재 전과 후 상지 기능과 삶의 질을 평가하기 위해 뇌졸중 기능회복 평가, 운동 활동 지표, 뇌졸중 영향 척도, 캐나다 작업 수행 평가, EQ-5D를 사용하였다. 대상자들을 무작위 할당하여 순환식 과제 지향적 집단훈련을 시행한 실험군 10명과 보존적 작업치료를 시행한 대조군 10명으로 할당하였다. 중재 후, 두 그룹 모두 상지 기능의 향상을 보였으며, 중재 전과 후 운동 활동 지표, EQ-5D 평균 변화량 비교에서 통계학적으로 유의한 차이를 보였다(p<.05). 캐나다 작업 수행 평가 결과 다림질하기, 빨래 널기, 수건 접기 활동이 대조군에 비해 실험군이 수행도, 만족도의 향상을 보였다. 순환식 과제 지향적 집단훈련이 만성 뇌졸중 환자의 상지 기능과 삶의 질 향상에 효과가 있었으며, 이러한 연구 결과는 순환식 과제 지향적 집단훈련을 만성 뇌졸중 환자에게 임상적 중재로 활용하는 근거가 될 수 있다.

The purpose of this study was to investigate the effects of task-oriented circuit training(: TOCT) on upper extremity function and quality of life in chronic stroke patients. 20 stroke patients were randomized and divided into 2 groups: a preservation therapy group and TOCT group. The intervention sessions were given five times a week for four weeks. The Stroke Impact Scale(: SIS), EuroQual-5Domains(: EQ-5D), Fugl-Myer Assessment(: FMA), Motor Activity Log(: MAL), Canadian Occupational Performance Measure(: COPM) were used to measure the upper extremity function and the quality of life. In results, Two groups improved in upper extremity function after the intervention(p<.05). The EQ-5D scores of TOCT group were a significantly higher than preservation group(p<.05). The Ironing, Folding towels, Hang out towels on drying rack in COPM scores in both of performance and satisfaction have improved more than preservation group(p<.05). In conclusion, the TOCT has significant helpful effect to chronic stroke patients. These findings can be used to chronic stroke patient as an intervention for upper extremity function and quality of life.
