A Study on Development of Secondary Traumatic Stress Prevention Program KCI 등재

본 연구의 목적은 클라이언트의 트라우마에 지속적으로 노출되는 전문가의 이차외상스트레스를 예 방하고 레질리언스를 강화하기 위해 조직적 차원에서 활용될 수 있는 이차외상스트레스 예방 프로 그램을 개발하고자 하는 것이다. 이를 위하여 개입조사모델이 활용되었다. 아동보호전문기관 상담 원들을 대상으로 욕구조사와 포커스 집단면접을 실시하여 상담원들이 경험하는 이차외상스트레스 의 수준, 이차외상스트레스가 업무와 개인적 삶에 미치는 영향, 기관에서 트라우마 관련 교육의 현 황, 이차외상스트레스 예방 프로그램을 실시한다면 필요한 내용과 운영방법 등에 대해 확인하였다. 이러한 과정을 거쳐 개발된 프로그램을 아동보호전문기관 상담원 7명을 대상으로 초기 현장실험을 실시한 결과, 개발된 프로그램이 상담원들의 이차외상스트레스와 우울 수준을 감소시키고, 스트레 스 대처방식의 사용 수준을 향상시켰음을 확인할 수 있었다. 이러한 결과를 바탕으로 장차 트라우마 에 노출된 클라이언트를 주된 대상으로 실천하는 전문가들을 위한 조직적 차원의 예방 프로그램을 최종 개발하였다.

The purpose of this study is to develop a program that can be adopted at an organizational level, preventing secondary traumatic stress and strengthening resilience of practitioners who are continuously exposed to client’s trauma. During the process of the program development, the intervention research model was employed. Both need survey and focus group interview were performed to counselors in child protection agencies, which confirmed the level of secondary traumatic stress experienced by counselors, impact on their work and personal life, and the status of the trauma-related training programs offered at an organizational level. The required contents and operational methods of the programs were also identified. A pilot study performed on 7 counselors found that the program reduced the level of secondary traumatic stress and depression, and improved the stress-coping style of the counselors. Based on the results, a preventative program was finally developed at an organizational level for practitioners working with trauma-exposed clients.

본 연구는 성매매 방지법을 집행함에 있어서 주된 방해요인 중의 하나로 제기되고 있는 성매매 여성에 대한 부정적인 인식을 개선하기 위하여, 향후 사회를 이끌어나갈 대학생들의 성매매 여성에 대한 인식을 조사하고 이러한 인식에 영향을 주는 요인을 분석하였다. 선행연구를 토대로 나이, 성별, 학년, 전공, 종교, 종교적 신념, 가정형편, 공감능력, 성 평등의식, 음란물 시청 횟수, 성매매 경험 여부, 성매매·성폭력·인권교육 여부 등이 영향요인으로 조사되었다. 502명이 설문조사에 참여하였으며, 다중회귀분석결과 성별, 사회복지 및 신학전공, 가정형편, 공감능력, 성평등 의식이 성매매 여성에 대한 태도에 유의미한 영향을 주었으며 성평등 의식이 가장 강력한 영향을 주는 변인이었다. 즉, 남학생, 사회복지 및 신학 전공자, 가정의 경제적 지위가 낮은 응답자, 공감능력과 성평등 의식이 높은 응답자가 성매매 여성에 대해 긍정적/지지적인 태도를 보여주었다. 분석결과를 토대로 대학생들의 성매매 여성에 대한 왜곡된 인식을 개선하기 위해서 공감능력과 성평등 의식을 함양할 수 있는 교양과목의 개발과 포괄적인 인성교육의 실시, 효과적인 성매매·성폭력 예방교육의 개발 등이 정책제언으로 제시되었다.

The purpose of current study is to investigate university students’ attitudes toward prostitutes and factors affecting their attitudes. Based on previous research, age, grade, major, gender, religion, religious faith, watching pornography, prostitution experience, empathy level, anti-prostitution/anti-rape /human right education and gender equality level were constructed as independent variables. Using self-administered survey method, 502 students participated in survey. Research results indicated that average of respondents’ attitudes toward prostitutes 4.19 was higher than median value(3). Results of multiple regression revealed that a male student, a social welfare and theology major student, a respondent with lower social-economic status, a respondent with higher level of empathy, and gender equality is more likely to demonstrate more supportive and positive attitude toward prostitute. Based on research results, developing programs aimed at enhancing empathy and gender equality, and implementing character education and prostitution/sexual violence prevention program were provided as policy implications.

Evaluation of a child abuse prevention program for unmarried mothers in South Korea: a single-case experimental design

Purpose: This study aimed to implement a child abuse prevention program and evaluateits effectiveness based on the Nursing Model of Resilience and Coping Skills TrainingModel for unmarried mothers during pregnancy and puerperium. Methods: This studyhad a prospective single-case, AB design with four repeated self-questionnaire measuresand three observational measures. Seven unmarried mothers were provided with 10 sessionschild abuse prevention program through individual visits from 32 to 34 weeks ofpregnancy to six weeks after childbirth. The questionnaire was composed related to resilience,maternal stress, maternal attitude, parent-child interaction, child abuse potential. The observation was measured by video recording (total 16 times) the interaction of parent-childduring feeding and analyzing it by three experts. Data were analyzed by Wilcoxonsigned-rank test and Friedman’s test. Results: Maternal attitude and parent-childinteraction were statistically significantly improved after intervention compared to beforeintervention. However, maternal stress decreased after intervention compared tobefore intervention, but it was not statistically significant. Additionally, resilience andchild abuse potential were not statistically significant. This program is partially effectivein preventing child abuse by promoting parenting attitudes and parent-child interactions. Conclusion: This study focused on individual resilience and applied systematic interventionas coping skills training to prevent child abuse. This study is meaningful inthat interventions were conducted through individual visits to unmarried mothers athigh risk of child abuse, and the program was applied, including pregnancy and postpartumperiods, to prevent child abuse early.

Effectiveness of a combined obesity prevention program in South Korea for children from low-income families that included primary caregiver participation

Purpose: Obesity among children from low-income families is becoming a social problem. The aim of this study was to evaluate the effectiveness of an obesity prevention program that included physical activity, nutrition education, behavioral modification, and primary caregiver participation components among children from low-income families. Methods: The study analyzed a nonequivalent control group using a pretest-posttest design. A total of 77 children were recruited from six community childcare centers using purposive sampling. For the intervention group (n=40), the pretest was administered before the combined intervention program involving the participants' primary caregivers was conducted for 8 weeks. The posttest was conducted immediately after the program and again four weeks after the program. Results: Flexibility (F=4.64, p=.020), muscular endurance (F=11.22, p<.001), nutritional knowledge scores (F=4.79, p=.010), body image satisfaction scores (F=4.74, p=.012), and self-esteem scores (F=3.81, p=.029) showed significant differences and interactions between group and time for the intervention and control groups. Conclusion: Strategies to actively engage the primary caregivers of low-income families in children's obesity programs are needed. Obesity prevention programs for children based on the program in this study should be routinely developed, and continuing attention should be given to children from low-income families.

The Effect of a Brief HIV Prevention Program on Risk Reduction Behaviors Among Thai Men Diagnosed With Sexually Transmitted Infections

Purpose: Men diagnosed with sexually transmitted infections (STIs) are at greater risk for Human Immunodeficiency Virus (HIV) infection and STIs reinfection. This study aimed to test the effectiveness of a brief human immunodeficiency virus (B-HIV) prevention program on HIV and sexually transmitted infection (STI) knowledge, perceived benefits of condom use, risk reduction self-efficacy, risk reduction behaviors, and reinfection rate among Thai men with STIs. Methods: A quasi-experimental design was conducted. Participants were selected from men with STI symptoms. They were randomly assigned to a B-HIV prevention program or usual care, 100 each. The program consisted of 3 modules. Key messages for HIV prevention were sent weekly through Line. Outcomes were HIV and STI knowledge, perceived benefits of condom use, risk reduction self-efficacy, risk reduction behaviors (condom use, the number of sexual partners, and condomless sex), and STI reinfection rate. Independent t-test and binary logistic regression were performed. Results: The B-HIV prevention program significantly increased HIV and STI knowledge and resulted in perception of greater benefits from condoms and greater risk reduction self-efficacy. Program participants used condoms more frequently with many types of partners, especially with casual partners and sex workers. The intervention group practiced condomless sex less frequently than the control group. The program did not improve participants’ condom use with lovers/steady partners and did not decrease the number of sexual partners and STI reinfection rate at 3-month follow-up. Conclusions: A B-HIV prevention program could reduce the risk of HIV infection among male clients with current STIs by enhancing their condom use with casual partners and sex workers. Strategies to improve condom use with lovers/steady partners among this high-risk population is needed.

Effect of Fall Prevention Exercise Program on Fall Efficacy, Depression, and Quality of Life of Elderly in the Community

Objective: This study aimed to investigate the effects of fall-prevention exercise programs on fall efficacy, depression and health-related quality of life in elderly. Design: a randomized controlled trial Methods: A total of 57 participants over 65-years-old have been allocated to control and experimental groups. The experimental group received 8 weeks of fall-prevention exercises that included strengthening of lower extremities, balance and gait training. They received the exercises twice a week for 50 minutes as a group.Both groups were assessed using Modified Falls Efficacy Scale (MFES), Geriatric Depression Scale (GDS), and 36-Item Short-Form Health Survey (SF-36) prior and post to the intervention. The control group also received the same exercise program after the post evaluation. Results: The experimental group showed overall improvement in MFES, GDS, and physical components of SF-36 (p<0.05). The MFES was significantly increased in the experimental group after the intervention (p<0.05). The GDS was significantly decreased in the experimental group after the intervention (p<0.05). The SF-36, only physical Function, Role limitations due to physical health, general health, and energy and fatigue categories were improved in the experimental group after the intervention (p<0.05). Conclusions: The results showed 8 weeks of fall-prevention exercise programs can increase fall efficacy and physical related quality of life while reducing depression of elderly over 65.

Development of a Violence Prevention Educational Program for Elementary School Children Using Empathy (VPEP-E)

Purpose: This study describes the development of a violence prevention educational program for elementary school children using empathy (VPEP-E) that teachers can use during class. Methods: Hoffman's theory of empathy and Seels and Richey's (1994) ADDIE model were applied to develop this program. Results: The developed program consisted of eight sessions: "Orientation/ definition of violence and empathy", "Types and boundaries of violence", "Look into my feelings", "Say it with a facial expression", "Preventing non-empathic violence due to social prejudice", "Preventing physical violence", "Verbal and online violence prevention: empathic conversation", and "I can do well: review of the whole curriculum". The program was evaluated by 15 elementary school teachers, who considered it to be easily accessible to elementary school students. The final VPEP-E, which will be provided in eight times for 40 minutes each for fifth-grade students, will provide a basis for preventing violence by fostering empathy. Conclusion: We expect the developed educational program to be effective in preventing violence among elementary school students. However, further research involving children from various age groups is needed.

Effects of Sexual Abuse Prevention Education Program on Sexual Knowledge and Attitude among Elementary School Students

Purpose: The purpose of this study was to investigate the effects of sexual abuse prevention education program linked to elementary physical and psychological development on sexual knowledge and attitude. Methods: The participants were the elementary school students of fifth and sixth grades in S city (experimental, comparison, and control group=96, 96, and 74, respectively). The experimental group received sexual abuse prevention education linked physical and psychological development, 6 sessions (3 sessions are physical and psychological development educations and 3 other sessions are sexual abuse prevention educations). The comparison group received sexual abuse prevention educations, 3 sessions (the same curriculum of the experimental group). The control group didn't receive any sexual education. The data were analyzed by x2 test and t-test, and ANOVA using the SPSS program. Results: The experimental group showed significantly better sexual knowledge and attitude than the comparison and control group. Conclusion: Sexual abuse prevention education program linked physical and psychological development is required for elementary school students, to improve the sexual knowledge and attitude.

Purpose: This study was intended to develop and evaluate the cancer prevention education for general population with different educational media such as booklets, cartoons, web frame comic strips, web flash animation and flash animations on personal digital assistants (PDA). Methods: A total of 125 subjects were divided into 5 groups, each group having 25 members and assigned a different educational media. An educational media was assigned to each group to evaluate the effect of education in the first intervention. In the second intervention, 4 other media, excluding the previously used one, were used to educate the subjects and find out the preferences of educational media. Results: Knowledge about cancer increased significantly after cancer prevention education. They preferred animations on the internet to other media, the highest. Conclusion: Educational programs with various media should be developed for subjects to choose based on their preferences.

Effects of a Peer Cervical Cancer Prevention Education Program on Korean Female College Students’ Knowledge, Attitude, Self-efficacy, and Intention

Purpose: The purpose of this study was to evaluate the effectiveness of a peer cervical cancer prevention education program on Korean female college students’ knowledge, attitude, self-efficacy, and intention. Methods: A quasi-experimental pretest-posttest design with a non-equivalent control group was used. The participants were 58 female college students in a metropolitan city in Korea. The sample consisted of an intervention group (n=28) that participated in a peer education program and a control group (n=30). Data were measured using self-administered questionnaires at two time points: prior to the intervention and after the intervention. Results: Compared to the control group, the experimental group reported significantly positive changes for knowledge, attitude, self-efficacy, and intent to practice cervical cancer prevention behaviors. Conclusion: The findings of this study indicated that a peer education program developed for Korean female college students was a useful and effective intervention strategy to promote cervical cancer prevention behaviors in Korean sociocultural contexts.

Theory-based Osteoporosis Prevention Education and Counseling Program for Women: A Randomized Controlled Trial

Purpose: The purpose of this research was to investigate the effect of an osteoporosis prevention program based on the Health Belief Model for women between the ages of 30 years and 45 years at risk of osteoporosis. Methods: This study was conducted with randomized control group pretest, post-test and follow-up trial. Intervention group (n = 37) and control group (n = 36) participated in the research. Data were collected using a sociodemographic data questionnaire, the Osteoporosis Knowledge Test, the Osteoporosis Health Belief Scale, the Osteoporosis Self-efficacy Scale, a monitoring form for estimated dairy calcium intake, and a monitoring form for estimated weekly exercise. Intervention program was composed of a 4-week education program and a 24-week counseling program. Data were collected pretest, post-test 15 days after the end of the education program, follow-up 1 after 3 months, and followup 2 after 6 months. Mann Whitney U test, chi-square test, Friedman test, Bonferroni test, two means test and Wilcoxon signed-rank test were used for statistical analysis. Results: After the education and counseling program, a significant increase was seen in comparison with the control group in the mean scores of the intervention group on the Osteoporosis Knowledge Test and its subscales (p < .001), on the Osteoporosis Health Belief Scale and its subscales (p < .001), on the Osteoporosis Self-efficacy Scale and its subscales (p < .001), and in their daily calcium intake (p < .001) and duration of weekly exercise (p < .001). Conclusion: The results of this study were evidence that showed the effects of Health Belief Model ebased osteoporosis prevention education and counseling program conducted by nurses.

Community-Based Aerobic Exercise Program for Primary Prevention of Cardiovascular Disease in Adults With Visual or Auditory Impairments: A Feasibility Study

Objective To investigate the feasibility of a public health center-based aerobic and resistance training program for primary prevention of cardiovascular disease in people with visual, auditory, or physical/brain impairments. Methods The study included 25 adults aged >40 years who lived in Cheorwon-gun in South Korea, had a disability registered for visual, auditory, or physical/brain impairments under the Disability Welfare Act, and had either known cardiovascular disease or two or more risk factors for cardiovascular disease. The program comprised four education sessions and 12 weeks of customized aerobic and strengthening exercises performed twice a week at moderate intensity, with each exercise session lasting for 1 hour. The body mass index (BMI), percent body fat, 6-minute walk distance (6MWD), and 30-second sit-to-stand test results were measured at baseline and on program completion. Results Seventeen subjects (68%) completed the program. There were significant decreases in BMI and percent body fat (both p<0.05), with a significant increase in 30-second sit-to-stand strength (p<0.05) but no changes in the 6MWD. In subjects with visual or auditory impairments, BMI and percent body fat were significantly decreased after the program; however, there was no significant change in the results of the 30-second sit-to-stand strength test or the 6MWD. Conclusion In people with disabilities, a 3-month community-based exercise program can decrease body mass index and percent body fat and increase sit-to-stand strength. The 30-second sit-to-stand test may be a useful measure of the strength and endurance of the lower extremities in people with disabilities.

Effects of Musculoskeletal Disease Prevention and Treatment Customized Exercise Program according to Various Music Tempo on the Pain, Posture, and Function of Auto Parts Manufacturing Workers

Objective: The purpose of this study is to investigate the effect of exercise programs according to various music tempo on pain, posture, and function of automobile parts manufacturing workers. Design: Two-group pretest-posttest design. Methods: This study was an exercise program tailored to the prevention and treatment of musculoskeletal diseases were performed for a total of 4 weeks and 3 times a week for 60 minutes. The exercise program consisted of customized exercises according to the characteristics of the subject's musculoskeletal system. The subject provided music during the exercise program, and the group who applied the fast tempo music listened to the music of 120~140 bpm, and the group who applied the slow tempo music listened to the music of the low tempo of 60~80 bpm. Results: Significant differences were found in pain and physical function before and after the fast tempo music group and the slow tempo music group (p<0.01). However, there was a significant difference in posture characteristics only in the fast tempo music group (p<0.01). In terms of pain, physical function, and posture characteristics, the fast tempo music group showed significant improvement compared to the slow music tempo group (p<0.05). Conclusions: A customized exercise program using fast-tempo music and slow-tempo music to workers in the automobile parts manufacturing industry, significant differences in pain, physical function, and posture characteristics applied with fast-tempo music were found. In the future, applying an exercise program with a fast tempo is expected to help improve musculoskeletal disorders.

Evaluation of a Mobile-based Maternal Feeding Education Program for Overweight Prevention in Infants

Purpose The purpose of this study was to evaluate a mobile-based maternal feeding education program for overweight prevention in infants based on breastfeeding attitude, breastfeeding self-efficacy, breastfeeding duration, recognition of hunger and satiety cues of infants, and knowledge regarding providing solids foods. Methods A nonequivalent control group pretest-posttest design was used for the study. Participants included 15 primiparas in the experimental group and 14 primiparas in the control group in all the follow-up tests. Using self-reported questionnaires in electronic format, data were collected four times (before the intervention, 1 month after childbirth, 3 months after childbirth, and 6 months after childbirth). Using SPSS 24 version, independent t-test and repeated-measures analysis of variance were used to test the effects of the mobile-based maternal feeding education program. Results The experimental group showed significantly more positive breastfeeding attitude (F = 5.28, p = .008), higher breastfeeding self-efficacy (F = 3.50, p = .041), and increased breastfeeding duration (t = ?2.09, p = .046) than the control group. In addition, the experimental group showed significantly improved knowledge regarding providing solid foods to the infants (F = 4.86, p = .009) in comparison with the control group. However, for education on recognizing hunger and satiety cues of infants, the mobile-based maternal feeding education program was not effective (F = 0.23, p = .878). Conclusion According to the results of this study, the mobile-based maternal feeding education program has the potential to contribute to overweight prevention in infants.

Preliminary Evaluation of Experiential Learning?Based Fall and Medication Error Prevention Education (EFMPE) using a Virtual Reality Room of Errors program: A Randomized Controlled Trial

Purpose: Falls and medication errors are the most common patient safety incidents globally. Kolb’s experiential learning theory supports the application of cognitive learning in clinical settings. This study examined the effectiveness of Experiential Learning?Based Fall and Medication Error Prevention Education (EFMPE), utilizing virtual reality and room of errors. Methods: A randomized controlled trial was conducted with 28 fourth-year nursing students (15 experimental, 13 control). The experimental group participated in EFMPE from February 1 to 6, 2024, comprising six sessions of 2 hours each. The control group received traditional lectures. Safety control confidence and course interest were measured before and immediately after the intervention; safety control confidence was reassessed 6 weeks later. Results: Both groups showed immediate improvement; however, only the experimental group sustained increased safety control confidence after 6 weeks (Wald χ2=13.21, p<.001). Course interest was significantly higher in the experimental group post-intervention (Wald χ2=10.64, p=.001). Conclusion: These preliminary findings suggest that EFMPE potentially supports the prevention of falls and medication errors in clinical practice.

Purpose: This study aimed to explore the effects of incorporating gym ball exercises for fall prevention on lower limb muscle activity and balance in elderly individuals. Methods: Thirty elderly individuals residing in nursing homes were selected as study participants and randomly divided into experimental and control groups. Each group was comprised of 15 individuals. The experimental group performed fall prevention exercises using gym balls, whereas the control group performed only fall prevention exercises. Both groups were conducted for 6 weeks, 3 times a week, with each session lasting 50 minutes. We compared and analyzed the changes in muscle activity and balance within and between the groups. Results: When comparing the changes in lower limb muscle activity between groups before and after intervention, the experimental group showed significantly higher lower limb muscle activity after the intervention compared to the control group, with a statistically significant difference (p<. 001). Similarly, the experimental group showed significantly higher improvements in both static and dynamic balance abilities after the intervention than the control group, with statistically significant differences (p<. 001). Conclusion: The results of this study demonstrated that incorporating Gym ball exercises into fall prevention programs is an effective method for enhancing lower limb muscle activity and balance in elderly individuals, thereby reducing fall risk.

본 연구는 내과계 중환자실에 입원한 기관삽관 환자의 비계획적 발관 예방프로그램을 개발하여 비계획적 발관율을 감소시키고 간호사들이 실무에서 비계획적 발관에 적극적으로 대처할 수 있도록 도움을 주기 위해 수행되었다. 조사기간은 사전기간이 2008년 5월부터 2009년 4월이었고, 프로그램 적용기간은 2009년 5월부터 2010년 4월로 각각 1년씩이었다. 기간 내 조사 대상 총 삽관환자는 사전시기와 중재 시기에 각각 330명, 332명이었으며, 이들 중 비계획적 발관을 경험한 사전 29사례와 적용기간 23사례에 대해 발관경험 환자와 담당간호사의 발관 관련 제 특성을 추가적으로 살펴보았다. 통계분석은 SPSS 18.0을 이용하였다. 비계획적 발관을 경험한 환자와 간호사의 제 특성은 유의한 차이를 보이는 것은 없었으나 두 시기에 신규 간호사의 진입 비율에 유의한 차이가 있었다(p=.039). 기관내관 삽입 1000일당 비계획적 발관율은 신규 간호사 진입이 많았음에도 적용 기간의 평균이 8.8을 보여 사전 관찰시기의 평균 발관율 12.3과 비교하여 유의하게 감소(t=-2.333, p=.040)하였으므로 본 연구의 결과는 중환자실 삽관환자의 비계획적 발관예방 활동을 통해 환자 안전에 기여할 수 있는 프로그램이라 사료된다.

Unplanned extubation prevention program (UEPP) was developed and applied to intubated patients hospitalized in a medical intensive care unit (ICU). The number of unplanned extubation was 29 during the preliminary investigation and 23 cases during the prevention intervention program. The general features of the patients and nurses who experienced UE did not have any significant differences between the two periods, with only the number of new nurses in the ICU being significantly different (p=.039). Although the number of new nursing staff in the ICU was high, the mean rate of unplanned extubation, which is the number of UE days per 1000 intubation days, was 8.8 during the prevention intervention program, which was significantly lower than 12.3, the mean rate of UE during the preliminary investigation (t=-2.333, p=.040). The results provide a prevention program that could contribute to the safety of patients by actively preventing unplanned extubation in intubated patients in ICU.
