The purpose of this research was to find the influence of marital dissatisfaction on the outcome of
cognitive behavioral therapy (CBT) in patients (men=25, women=27) with panic disorder who participated in CBT. For demographic data, there was no significant differences between groups that scored low on marital dissatisfaction (L-GDS, n=30) and scored high (H-GDS, n=22). H-GDS group got significantly higher level of depression score in pre-treatment symptom severity. So, we explored which features in panic symptom had a interactional effect with marital dissatisfaction when except effect of depression severity difference. And then, it was found that their, all clinical features besides agoraphobic cognition, was a difference in treatment effect according to marital dissatisfaction. Between pre-treatment and post-treatment, both H-GDS and L-GDS group has improved in symptoms and L-GDS group showed lower level of panic
symptom severity than H-GDS at post-treatment. The findings that patients who were also under the severe life stress could gain some benefit from CBT was supported by this result. The results showed that there might be differences in the degree of improving symptoms depending on the level of marital dissatisfaction. Therefore, the level of marital dissatisfaction could be a predictor for the outcome of CBT.

본 연구에서는 인지행동치료에 참가한 공황장애 환자 중 기혼자(남=25명, 여=27명)를 대상으로, 결혼 불만족도에 따른 공황장애 증상 및 인지행동치료 효과의 차이를 알아보고자 하였다. 연구 결과, 결혼 불만족이 높은 집단(n=22)과 낮은 집단(n=30) 간에 인구통계학적 변인에서는 유의미한 차이가 없었다. 치료 전 임상적 특성에서는 결혼 불만족이 높은 집단이 낮은 집단에 비해 유의미하게 높은 우울감을 보였다. 이에, 결혼불만족도에 따라 치료 효과가 달라지는지를 알아보기 위하여, 우울감의 효과를 배제한 후, 각 치료 전후의 임상적 특성과 결혼 불만족도 결과의 상호작용 효과를 보았다. 그 결과, 광장공포증 인지를 제외한 대부분의 임상적 특성들은 결혼 불만족도에 따라 치료 효과에 차이가 있었다. 치료 전과 후의 차이를 비교한 결과, 불만족이 높은 집단과 낮은 집단 모두 치료 후 뚜렷한 증상의 호전이 있었으며, 결혼 불만족도 수준에 따른 치료 효과의 차이를 보다 명확히 하기 위하여, 치료 후 두 집단의 증상 수준을 비교한 결과, 결혼에 대한 불만족도가 낮았던 집단이 그렇지 않은 집단에 비해 유의미하게 낮은 공황장애 증상을 보였다. 본 연구를 통해, 공황 장애에 대한 인지행동치료가, 현재 공황장애 외에 생활사의 뚜렷한 스트레스를 경험하는 집단에서도 일정 수준 이상의 호전을 가져온다는 긍정적인 결과를 다시 한 번 확인할 수 있었다. 또한 결혼불만족 수준에 따라, 인지행동치료를 통한 전반적인 증상의 감소에 차이를 보일 수 있음을 발견하였으며, 결혼 불만족 수준의 치료에 대한 주요 예측 변인으로서의 가능성을 확인할 수 있었다.
