This study investigated knowledge, attitudes, and practices (KAP) regarding antibiotic use and misuse-related risk behaviors among 213 seafarers and analyzed the associated factors. The prevalence of antibiotic use during voyages was 50.7%, and misuse-related behaviors were common, including premature discontinuation of medication (64.8%), self-medication (32.4%), reuse of leftover antibiotics (35.2%), and sharing antibiotics (32.4%). The proportion of the high-risk group with P≥3 was 47.9%. Awareness of the seriousness of antibiotic use on board was low (mean score: 2.45), whereas the perceived need for education and institutional reinforcement was high (4.20 and 4.10, respectively), and 74.6% reported having received no prior education at all. In multivariable analyses, knowledge score was identified as a protective factor that reduced the likelihood of belonging to the high-risk group and engaging in self-medication, whereas prior antibiotic use during voyages was a strong predictor of high-risk status, self-medication, and premature discontinuation. We found that misuse-related antibiotic behaviors were highly prevalent among Korean seafarers, and that a higher level of knowledge functioned as a protective factor, whereas prior antibiotic use during voyages was significantly associated with high-risk status, self-medication, and premature discontinuation. Based on these findings, we suggest that standardized education, medication records, decision-making protocols, and telemedical consultation linkage systems that simultaneously target knowledge correction and behavioral implementation are needed to promote the safe use of antibiotics among seafarers.
