Family Planning through the Voices of Family Planning Workers : Policy Transition and the Persistence of Frontline Work in Seoul Public Health Centers since the Late 1980s
This study examines how South Korea's Family Planning Program persisted and changed from the late 1980s to the mid-1990s, a period when the government's long-standing goal of curbing population growth began to lose its rationale. Drawing on documentary sources and oral history interviews with two family planning workers who served at public health centers in Seoul, the study analyzes how the relationship between national policy and frontline practice shifted over this period. During the 1980s, the Family Planning Program operated through a combination of method-specific contraceptive targets, performance evaluation, client management, contraceptive counseling, and referral for contraceptive procedures. Family planning workers at public health centers connected residents to the program's administrative system by conducting home visits and counseling, issuing coupons, referring clients to designated hospitals, and managing performance records. Their work involved the physical burden of repeated fieldwork, the emotional labor of handling residents' resistance, and delivering contraceptive counseling, and the administrative responsibility of managing clients and performance records. Around the time of the revised population projection of 1988, population growth and fertility rates had reached the levels targeted by the government, and government-supported contraceptive services began to decline in scale. Even so, the existing system of target management and frontline work did not disappear immediately. This study interprets this persistence not as evidence of deliberate institutional preservation, but as a consequence of policy goals and frontline work systems changing at different speeds. From the mid-1990s onward, as health promotion programs expanded, family planning work gradually became peripheral to the core activities of public health centers. The interviewees' recollections are read not as direct reproductions of their perceptions at the time, but as retrospective memories through which they reassess the meaning of their past labor and of family planning policy itself, shaped by South Korea's current context of ultra-low fertility. Through this analysis, this study understands the later phase of the Family Planning Program not simply as a period of decline, but as a transitional period marked by a time lag between policy change and the reorganization of frontline work. It also illuminates, in concrete terms, the fieldwork and administrative tasks carried out by family planning workers in metropolitan public health centers.
