Effects of Whole-Body Cryotherapy on Oxidative and Inflammatory Responses in Exercising Populations : A Systematic Review and Meta-analysis
The purpose of this study was to systematically review and quantitatively evaluate the effects of whole-body cryotherapy (WBC) on post-exercise inflammatory and oxidative-stress biomarkers in exercising adults. PubMed, Web of Science Core Collection, Embase, Cochrane Library, and Scopus were searched from database inception to June 2024. Studies were selected according to predefined PICOS criteria and included healthy exercising adults who received one or more WBC exposures and reported inflammatory or oxidative-stress outcomes. Four studies involving 96 unique participants were included in the final analysis. Randomized parallel-group and crossover studies were assessed using the Cochrane Risk of Bias 2 (RoB 2) tool. Continuous outcomes were expressed as mean differences (MDs) with 95% confidence intervals (CIs), and random-effects models were applied because clinical and methodological heterogeneity was expected. Three studies contributed IL-6 data, and the pooled effect showed no statistically significant reduction following WBC (MD=-0.71 pg/mL, 95% CI -1.49 to 0.06, P=.071, I²=77.5%). However, sensitivity analysis excluding the study requiring conversion of median and interquartile-range data produced a significant estimate (MD=-1.03 pg/mL, 95% CI -1.99 to -0.07), indicating limited robustness of the overall IL-6 finding. IL-1β concentrations were significantly lower following WBC (MD=-1.76 pg/mL, 95% CI -3.00 to -0.51, P=.006), although substantial heterogeneity was observed (I²=98.1%). CRP/hsCRP concentrations were also significantly lower following WBC (MD=-0.62 mg/L, 95% CI -0.96 to -0.29, P<.001, I²=0%). In contrast, available evidence was insufficient to reliably pool total leukocyte count, neutrophil count, superoxide dismutase (SOD), or glutathione peroxidase (GPx). These findings indicate that the effects of WBC on inflammatory responses in exercising adults are not consistent across biomarkers or intervention protocols. Although lower IL-1β and CRP/hsCRP concentrations were observed under some conditions, IL-6 did not show a consistent reduction, and interpretation is limited by the small number of studies and substantial heterogeneity. Therefore, WBC should be regarded as a context-dependent adjunct recovery strategy rather than a uniformly anti-inflammatory intervention. Future studies should use larger randomized controlled designs with standardized WBC protocols and biomarker sampling time points.

PubMed, Web of Science Core Collection, Embase, Cochrane Library CRP/hsCRP 또한 WBC군에서 유의하게 낮게 나타났다(MD=-0.62 mg/L, 95% CI -0.96~-0.29, P<.001, I²=0%).
