여성 청소년의 체형 과대인식, 정신건강 취약성 및 부적절한 체중조절행동 간의 관련성 : 2025년 청소년건강행태조사 자료를 이용한 분석
Body Weight Overestimation, Mental Health Vulnerability, and Inappropriate Weight-Control Behaviors Among Adolescent Girls : An Analysis of the 2025 Korea Youth Risk Behavior Survey
Objectives: Adolescent girls may be vulnerable to sociocultural pressure regarding body shape. Body weight overestimation may be associated with mental health vulnerability and inappropriate weight-control behaviors. This study examined associations among body weight overestimation, mental health vulnerability, and inappropriate weight-control behaviors among adolescent girls using data from the 2025 Korea Youth Risk Behavior Survey. Methods: This secondary analysis included 26,409 girls from 54,170 participants. Data were analyzed using the complex sampling design. Unweighted frequencies, weighted percentages, Rao-Scott chi-square tests, and complex-sample logistic regression analyses were performed. Results: The prevalence of body weight overestimation, mental health vulnerability, and inappropriate weight-control behaviors was 28.7%, 58.7%, and 14.8%, respectively. Girls with body weight overestimation had higher proportions of mental health vulnerability (60.1% vs. 57.7%) and inappropriate weight-control behaviors (17.2% vs. 13.5%) than those without overestimation. Mental health vulnerability was significantly associated with inappropriate weight-control behaviors (aOR=1.91, 95% CI=1.71-2.13, p<.001). The interaction between body weight overestimation and mental health vulnerability was also significant (aOR=1.24, 95% CI=1.03-1.50, p=.024). Inappropriate weight-control behaviors were most prevalent among girls with both body weight overestimation and mental health vulnerability (22.2%). Conclusions: Inappropriate weight-control behaviors among adolescent girls were more common when body weight overestimation and mental health vulnerability coexisted. School health education should integrate healthy weight-control education, correction of distorted body perception, and early screening for mental health vulnerability.

목적: 여성 청소년은 체형과 외모에 대한 사회문화적 압력을 민감하게 경험할 수 있으며, 실제 체중상태보다 자신의 체형을 더 살찐 편으로 인식하는 체형 과대인식은 정신건강 취약성과 부적절한 체중조절행동으로 이어질 수 있다. 본 연구는 2025년 청소년건강행태조사 자료를 이용하여 여성 청소년의 체형 과대인식, 정신건강 취약성 및 부적절한 체중조절행동의 관련성을 확인하고자 하였다. 방법: 본 연구는 2025년 청소년건강행태조사 원시자료를 활용한 이차자료분석 연구이다. 전체 대상자 54,170명 중 여학생 26,409명을 최종 분석대상자로 선정하였다. 자료분석은 복합표본설계를 반영하여 수행하였으며, 비가중 빈도와 가중 백분율, Rao-Scott 보정 카이제곱검정, 복합표본 로지스틱 회귀분석을 이용하였다. 결과: 대상자의 체형 과대인식은 28.7%, 정신건강 취약성은 58.7%, 부적절한 체중조절행동은 14.8%로 나타났다. 정신건강 취약성은 체형 과대인식 집단에서 60.1%, 비과대인식 집단에서 57.7%로 나타났으며, 부적절한 체중조절행동은 각각 17.2%와 13.5%로 나타났다. 정신건강 취약성과 부적절한 체중조절행동은 모두 체형 과대인식 여부에 따라 유의한 차이를 보였다. 복합표본 로지스틱 회귀분석 결과, 정신건강 취약성은 부적절한 체중조절행동과 유의한 관련성을 보였으며(aOR=1.91, 95% CI=1.71-2.13, p<.001), 체형 과대인식과 정신건강 취약성의 상호작용항도 유의하였다(aOR=1.24, 95% CI=1.03-1.50, p=.024). 체형 과대인식과 정신건강 취약성이 모두 있는 집단의 부적절한 체중조절행동률은 22.2%로 가장 높았다. 결론: 여성 청소년의 부적절한 체중조절행동은 체형 과대인식과 정신건강 취약성이 함께 존재할 때 더욱 높게 나타났다. 따라서 학교보건교육에서는 건강한 체중조절방법 교육과 함께 왜곡된 체형 인식 교정, 정신건강 취약성 조기 선별 및 상담 연계를 포함한 통합적 접근이 필요하다.
