미만성 거대 B세포 림프종 환자에서 심장재활이 골격근량, 혈압, 심폐기능 및 삶의 질에 미치는 영향
Effects of Cardiac Rehabilitation on Skeletal Muscle Mass, Blood Pressure, Cardiopulmonary Function, and Quality of Life in Patients with Diffuse Large B-Cell Lymphoma
Objectives This study examined the effects of a 12-week cardiac rehabilitation program on skeletal muscle mass, blood pressure, cardiopulmonary function, and quality of life in patients with DLBCL after anthracycline-based chemotherapy. Methods Thirty patients with DLBCL after completing anthracycline-based chemotherapy and considered clinically stable for exercise by their treating physician were randomly assigned to a control group (CG, n = 15) or a cardiac rehabilitation group (CRG, n = 15). The CRG participated in a 12-week hospital-based multidisciplinary cardiac rehabilitation program; the CG received usual care. Data were analyzed using two-way repeated-measures ANOVA and Bonferroni-corrected Welch's t-tests. Results Significant group × time interactions were found for skeletal muscle mass, systolic and diastolic blood pressure, resting heart rate, VO2max, and quality of life. In the CG, quality of life increased and skeletal muscle mass decreased significantly, with no other significant changes. In the CRG, skeletal muscle mass, systolic blood pressure, VO2max, and quality of life increased significantly, while diastolic blood pressure and resting heart rate decreased significantly. Post-intervention, the CRG showed significantly lower diastolic blood pressure and resting heart rate and significantly higher VO2max and quality of life than the CG, with no between-group differences in skeletal muscle mass or systolic blood pressure. Conclusions A hospital-based multidisciplinary cardiac rehabilitation program improved cardiopulmonary function and quality of life in patients with DLBCL. Skeletal muscle mass increased in the CRG without between-group differences. Increased systolic blood pressure warrants cardiovascular safety evaluation.

본 연구는 안트라사이클린 기반 항암화학요법 후 안정기에 있는 미만성 거대 B세포 림프종(diffuse large B-cell lymphoma, DLBCL) 환자를 대상으로 12주간의 병원 기반 심장재활 프로그램이 골격근량, 혈압, 심폐기능 및 삶의 질에 미치는 효과를 평가하였다. 연구 대상자는 A병원에서 DLBCL을 진단받고 항암치료를 완료한 후 담당 전문의가 운동 참여가 가능하다고 판단한 환자 30명으로, 대조군(CG, n=15)과 심장재활군(CRG, n=15)에 무작위 배정하였다. CRG는 운동중재와 영양·심리·수면 및 생활습관 관리가 포함된 12주간의 병원 기반 심장재활 프로그램에 참여하였으며, CG는 표준관리를 유지하였다. 집단×시기 상호작용은 이원 반복측정분산분석으로, 중재 후 군 간 비교는 Welch의 독립표본 t-검정(Bonferroni 보정)으로 분석하였다. 그 결과, 골격근량, 수축기혈압, 이완기혈압, 안정시 심박수, 최대산소섭취량 및 삶의 질에서 모두 집단×시기 상호작용 효과가 유의하였다. CG는 골격근량 감소와 삶의 질 증가만 유의 하였고, CRG는 골격근량·수축기혈압·최대산소섭취량·삶의 질 증가와 이완기혈압·안정시 심박수 감소가 모두 유의하였다. 중재 후 CRG는 CG보다 이완기혈압·안정시 심박수가 낮고 최대산소섭취량·삶의 질이 높았으나, 골격근량과 수축기혈압은 군 간 차이가 없었다. 이상의 결과를 종합하면, 운동중재와 함께 영양·심리·수면 및 생활습관 관리가 포함된 병원 기반 심장재활 프로그램은 DLBCL 환자의 심폐기능 및 삶의 질 개선에 긍정적인 영향을 미칠 수 있음을 시사한다. CRG에서는 골격근량이 유의하게 증가하였으나 군 간 차이는 확인되지 않았다. 다만 수축기혈압의 증가는 심혈관계 안전성 측면에서도 함께 고려되어야 하며, 임상적 의미를 확인하기 위한 추가 연구가 필요하다.
