Health Inequality and Democratic Representation : The Relationship between Local Health Vulnerability and Electoral Participation in South Korea
This study examines health inequality as a question of political participation and democratic representation rather than merely as an issue of health care or social welfare. Health constitutes a basic condition for citizens to exercise political rights, and local medical, welfare, housing, and safety conditions form part of the social context related to electoral participation. Using data from 229 cities, counties, and districts in South Korea, this study analyzes the relationship between local health vulnerability and voter turnout. The main analysis focuses on the 22nd National Assembly election held in 2024, while the 21st presidential election held in 2025 is used as a supplementary analysis. Local health vulnerability is measured as a composite local vulnerability index that combines medical accessibility, welfare demand, housing vulnerability, social welfare infrastructure, and health and safety risks. The findings show that districts with higher levels of local health vulnerability tend to exhibit relatively lower levels of overall voter turnout. The analysis of early voting indicates that the use of institutional arrangements designed to increase voting convenience may vary depending on local conditions. Age-specific turnout analyses also show that the negative association between local health vulnerability and turnout is not confined to a single age group but is observed across multiple age groups. However, because this study relies on aggregate data at the city, county, and district level, the findings do not directly test the causal effect of individual health status on individual voting behavior. Rather, they provide exploratory evidence that health-related local vulnerability, combining medical, welfare, housing, and safety conditions, may be associated with regional disparities in electoral participation. These findings suggest that health inequality should be discussed not only as a matter of health and welfare policy but also as an issue related to the social conditions of political participation and democratic representation.

본 연구는 건강불평등을 보건·복지정책의 문제에 한정하지 않고, 정치참여와 민주적 대표성의 문제로 확장하여 검토한다. 건강은 시민이 정치적 권리를 행사하는 데 필요한 기초적 조건이며, 지역의 의료·복지·주거·안전 환경은 시민의 투표참여와 관련된 사회적 기반을 구성한다. 본 연구는 전국 229개 시군구 자료를 활용하여 지역 건강취약성과 투표참여 사이의 관계를 분석하였다. 주 분석대상은 2024년 제22대 국회의원선거이며, 2025년 제21대 대통령선거는 보조분석으로 활용하였다. 본 연구에서 지역 건강취약성은 의료 접근성, 복지 수요, 주거 취약성, 사회복지 인프라, 건강·안전 위험을 종합한 복합적 지역 취약성 지표로 측정하였다. 분석 결과, 지역 건강취약성이 높은 시군구에서는 전반적 투표참여 수준이 상대적으로 낮게 나타나는 경향이 확인되었다. 사전투표율 분석에서는 투표 편의성을 높이는 제도적 장치의 활용 역시 지역 조건에 따라 다르게 나타날 수 있음을 보여주었다. 연령대별 분석에서는 지역 건강취약성과 투표참여의 부정적 관계가 특정 연령층에만 국한되지 않고 여러 연령대에서 관찰되었다. 다만 본 연구는 시군구 단위 집계자료에 기반한 분석이므로, 개인의 건강상태가 개인의 투표 여부에 미치는 인과효과를 직접 검증하는 것은 아니다. 이러한 결과는 건강·복지·주거·안전 조건이 결합된 지역 취약성이 정치참여의지역 격차와 연결될 수 있음을 보여주는 탐색적 증거이며, 건강불평등이 민주적 대표성의 문제와 함께 논의될 필요가 있음을 시사한다.
