Severity Patterns and Associated Factors of Patient Safety Incidents in Intensive Care Units: Analysis of National Incident Reports from Korean Hospitals (2020–2024)
중환자실 환자안전사고의 위해 수준 양상 및 관련 요인 분석 : 2020-2024년 국내 병원 환자안전사고 국가보고자료 분석
This study examined factors associated with the severity of intensive care unit (ICU) patient safety incidents in Korean hospitals with ≥500 beds using the national Patient Safety Reporting Database (2020– 2024). ICU incidents with complete key fields were included. Incident severity was classified as near miss, adverse event, or sentinel event. Multivariable regression analyses identified predictors of adverse and sentinel events. In adjusted models, incident severity was significantly associated with temporal factors (season and duty time) and incident type. Compared with winter and night duty, non-winter seasons and daytime/evening duty were associated with lower odds of higher-severity events. Falls and injury/burn-related incidents showed stronger associations with sentinel events, whereas medication-related incidents were associated with lower odds of both adverse and sentinel events. These findings may inform targeted prevention strategies and refinements of ICU patient safety reporting systems in large hospitals.

본 연구는 의료기관평가인증원이 수집한 2020–2024년 환자안전 보고자료를 이용하여, 중환자실 환자안전사고를 분석하였다. 환자안전사고는 근접오류, 위해사건, 중대사건으로 분류하였다. 중환자실 환자안전사고의 중증도 수준과 관련 요인을 확인하기 위해 다변량 회귀분석을 수행하였다. 분석 결과, 사고 중증도는 계절 및 근무시간대 등 시간적 요인과 사고유형과 유의한 관련성을 보였다. 겨울 및 야간 근무에 비해 비동절기와 주간/저녁 근무에서 높은 중증도 사건과의 연관성이 낮게 나타나는 경향이 관찰되었다. 또한 낙상, 손상/화상 등 일부 사고유형은 중대사건과의 연관성이 상대적으로 높았고, 투약 관련 사고유형은 위해사건 및 중대사건과의 연관성이 낮은 양상을 보였다. 본 결과는 대형병원 중환자실에서 사고유형 및 근무시간대 특성을 고려한 표적 예방전략과 보고체계 개선의 근거를 제공한다.
