The Neuroscience Mechanisms of Single-Session Counseling : Focusing on Synaptic Plasticity and Emotional Network Reorganization
The purpose of this study is to theoretically infer the neuroscientific mechanisms by which single-session narrative counseling reconfigures clients’ emotional networks and to interpretatively examine the feasibility of this approach by applying the theoretical framework to actual case studies. Contemporary college students are continuously exposed to complex and chronic stressors, such as academic pressure, career uncertainty, interpersonal conflicts, and financial strain, leading them to experience various psychological difficulties, including depression, anxiety, identity confusion, and lethargy. However, given the difficulty of providing long-term, systematic counseling interventions due to a shortage of professional staff and structural limitations at university counseling centers, the question of whether a single counseling session can elicit meaningful emotional changes at the structural and functional levels of the client’s brain has emerged as a critical issue in both counseling practice and academia. Accordingly, this study adopted a theory application case study method based on the interpretive paradigm, analyzing the verbatim transcript of a single-session narrative counseling session with a female college student client. This study does not perform direct brain imaging or measure neurophysiological indicators; therefore, the results represent “theoretical inferences” grounded in existing neuroscience literature rather than empirical evidence. Based on the verbatim transcript collected after the session concluded, the researcher systematically applied emotion regulation theories and therapeutic stage models grounded in neuroscience to interpretatively analyze the therapeutic mechanisms evident in the counseling process. Hanna’s (2021) five-stage model of emotional regulation was applied as the primary analytical framework, with Zimmerman’s (2017) therapeutic principles serving as a supplementary framework. The analysis revealed that single-session narrative counseling facilitates the activation of the ventral vagal social engagement system through autonomic nervous system stabilization, the integration of emotional and somatic sensations and the restoration of emotional regulation based on prefrontal cortex activation, the self-referential narrative reconstruction via the default mode network, and identity transformation mediated by neuroplasticity and long-term potentiation. In particular, the process of externalization using metaphors and the unique exploration of outcomes are inferred to be potential neuroscientific pathways that can alleviate threat-related hyperarousal in the amygdala and stimulate reward circuits, thereby promoting the reconstruction of positive emotions. Furthermore, the counselor’s empathic listening and emotional reflection are interpreted as contributing to the revision of the client’s internal working model and the formation of more stable attachments from the perspective of neurobiological resonance. These results go beyond existing skepticism regarding the effectiveness of single-session counseling to suggest the possibility of theoretical justification, providing theoretical grounds and practical implications that can be effectively applied in Christian counseling settings despite temporal and environmental constraints.

본 연구는 내러티브 접근 단회기 상담이 내담자의 정서 네트워크를 재구성하는 신경과학적 기제를 이론적으로 추론하고, 실제 사례에 이론적 틀을 적용하여 그 가능성을 해석적으로 검토하는 데 목적이 있다. 현대 대학생들은 학업 부담, 진로 불확실성, 대인관계 갈등, 경제적 압박 등 복합적이고 만성적인 스트레스에 지속적으로 노출되어 있으며, 이로 인해 우울, 불안, 자아정체감 혼란, 무기력 등 다양한 심리적 어려움을 경험하고 있다. 그러나 대학 상담센터의 전문인력 부족과 구조적 한계로 인해 장기적이고 체계적인 상담 개입이 어려운 상황에서, 단 한 번의 상담 회기가 내담자의 뇌 구조적·기능적 수준에서 의미 있는 정서적 변화를 이끌어낼 수 있는가라는 질문은 상담 실무와 학계 모두에서 중요한 과제로 제기되고 있다. 이에 본 연구는 해석적 패러다임에 기반한 이론적용 사례연구(theory application case study) 방법을 채택하여, 여대생 내담자의 단회기 내러티브 상담 축어록을 분석 대상으로 삼았다. 본 연구는 직접적인 뇌영상 측정이나 신경생리학적 지표 측정을 수행하지 않으며, 따라서 분석 결과는 신경과학적 사실의 입증이 아니라 기존 신경과학 문헌에 근거한 '이론적 추론'의 수준에 해당함을 밝힌다. 연구자는 상담 종료 이후 수집된 축어록을 바탕으로, 뇌신경과학에 근거한 정서 조율 이론과 치료 단계 모델을 체계적으로 적용하여 상담 과정에서 나타나는 치료적 기제를 해석적으로 분석하였다. Hanna(2021)의 정서 조율 5단계 모델을 일차적 분석틀로, Zimmerman(2017)의 치료 원리를 보조 분석틀로 적용하였다. 분석 결과, 내러티브 단회기 상담은 자율신경계 안정화를 통한 복측미주신경의 사회적 참여 체계 활성화, 전전두피질 활성화에 기반한 정서-신체 감각의 통합과 정서 조절 회복, 디폴트 모드 네트워크를 통한 자기참조적 서사 재구성, 그리고 신경가소성과 장기강화를 매개로 한 정체성 변형으로 이어지는 단계적 정서 조율 과정과 이론적으로 부합하는 변화 양상을 보이는 것으로 해석되었다. 특히 은유를 활용한 외재화와 독특한 결과 탐색 과정은 편도체의 위협 관련 과각성을 완화하고 보상회로를 자극하여 긍정적 정서 재구성을 촉진할 수 있는 가능한 신경과학적 경로로 추론되는 바이다. 또한 상담자의 공감적 경청과 정서적 반영은 신경생물학적 공명의 관점에서 내담자의 내적 작동 모델 수정과 보다 안정적인 애착 형성에 기여할 수 있는 것으로 해석된다. 이러한 결과는 단회기 상담의 효과성에 대한 기존의 회의론을 넘어 이론적 정당화의 가능성을 제시하며, 기독교 상담 현장에서 시간적·환경적 제약 속에서도 효과적으로 적용될 수 있는 이론적 근거와 실천적 시사점을 제공한다.
