This study aims to reconstruct the history of South Korea's medical insurance integration by moving beyond existing institution-centered narratives and focusing on the practices and experiences of regional medical insurance union workers. While previous research has primarily focused on the policy-making processes and institutional achievements of integration, the role of frontline workers, who stabilized the system and provided the momentum for the integration movement, has not been sufficiently illuminated. To fill this gap, this study adopts the oral life history method, conducting an in-depth analysis of the oral testimony of Geum Seong-cheol, who joined the Gwanak-gu Regional Medical Insurance Union in 1989 and served as a frontline practitioner, a regional medical insurance labor union activist, and a full-time activist for the Citizens' Council for Medical Insurance Solidarity (Uiboyeondaehoe). The analysis reveals that regional medical insurance union workers, who were hired in large numbers in the late 1980s, struggled to stabilize the system by mediating conflicts over premium burdens for the poor, collection disputes, and assessment disagreements caused by a lack of income transparency. Simultaneously, they experienced firsthand the contradictions of regional corporatism, such as structural financial imbalances, operational inefficiencies, and the downward equalization of coverage levels. Having joined the workforce concurrently with peers who had directly or indirectly experienced the 1987 Democratization Movement and the Great Workers' Struggle, these workers naturally publicized the importance of medical insurance integration through union formation and strike actions. Through solidarity with the Citizens' Council for Medical Insurance Solidarity launched in 1994, they transformed integration into a core social reform agenda of the Korean Confederation of Trade Unions (KCTU). This evolved into specific activities where KCTU-affiliated unions pressured management during collective bargaining to support medical insurance integration. Ultimately, this solidarity and bottom-up pressure from the labor movement became the core driving force behind the establishment of a single-payer social insurance system, culminating in the launch of the National Health Insurance Service in 2000 and the completion of financial integration in 2003 despite organized opposition from the Ministry of Health and Welfare and workplace-based unions. By empirically demonstrating through oral history that universal health insurance was not merely a top-down design by state bureaucrats but a historical achievement won through the dynamic combination of frontline workers' concrete experiences and labor-civilian solidarity, this study seeks to provide a multi-dimensional re-evaluation of the labor movement's role in the formation of the South Korean welfare state.

본 연구는 한국 의료보험 통합의 역사를 제도사 중심의 기존 서술을 넘어, 지역의 료보험조합 노동자들의 실천과 경험을 통해 재구성하는 것을 목적으로 한다. 기존 연 구는 의료보험 통합의 정책 결정 과정과 제도적 성과에 주로 주목해 왔으나, 제도를 정착시키고 통합 운동의 동력을 형성한 현장 노동자의 역할은 충분히 조명되지 않았 다. 본 연구는 이러한 공백을 메우기 위해 구술생애사 방법을 채택하여, 1989년 관악 구 지역의료보험조합에 입사해 현장 실무자이자 지역의료보험노조 활동가, 의보연대 회의 상근활동가로 활동한 금성철의 구술을 심층 분석하였다. 분석 결과, 1980년대 후반 대거 채용된 지역의료보험조합 노동자들은 빈곤층의 보험료 부담과 징수 갈등, 소득 파악 부재로 인한 산정 분쟁을 조정하며 제도 안착을 위해 분투하는 한편, 조합 주의가 구조적으로 낳는 재정 불균형, 운영 비효율, 보장성 하향평준화의 모순을 현장에서 절감하였다. 1987년 민주화운동과 노동자 대투쟁을 직간접적으로 경험했던 비 슷한 연배의 사람들이 동시에 입사했던 상황이었던 만큼 지역의료보험조합 노동자들 은 자연스럽게 노동조합 결성과 파업투쟁을 통해 의료보험 통합의 중요성을 사회적으 로 알려 나갔고, 1994년 출범한 의보연대회의와의 연대를 통해 민주노총의 핵심 사 회개혁 의제로 만들어냈다. 이는 민주노총 산하 노조들이 단체협상에서 사측에 의료 보험 통합을 찬성하도록 압박하는 구체적 활동으로 전개됐다. 이처럼 노동운동을 중 심으로 한 아래로부터의 연대와 압력은 보건복지부와 직장노조 등의 조직적 반대에도 불구하고 2000년 건강보험공단 출범과 2003년 재정통합 완성으로 이어지는 단일체 계 사회보험 구축의 핵심 동력이 되었다. 본 연구는 전 국민 건강보험이 국가 관료의 하향식 설계가 아니라, 현장 노동자의 구체적 경험과 노동–시민 연대의 역동성이 결합해 쟁취한 역사적 성취였음을 구술사적으로 실증하며, 한국 복지국가 형성 과정에서 노동운동의 역할을 입체적으로 재조명하고자 한다.
