This study analyzed the structural characteristics of epidural block utilization patterns from 2014 to 2023 using South Korean national healthcare big data, focusing on the constraints of healthcare consumers' decisionmaking and the impact of information asymmetry. The subjective nature of pain creates an inherent information gap between providers and consumers, often leading healthcare consumers to rely heavily on provider recommendations, which facilitates a structural environment for repetitive procedures. Analysis showed that the number of annual users increased by 6.8% over the decade, with the treatment intensity consistently remaining above 2.0 procedures per person. Notably, 96.4% of procedures occurred in outpatient settings, with a high proportion of female consumers (57.1%). These patterns suggest that chronic pain management in Korea may be driven by established outpatient delivery structures and administrative reimbursement criteria rather than the autonomous choices of informed consumers. Such findings highlight the clinical reality where healthcare consumers face difficulties in objectively assessing their conditions and making independent therapeutic decisions. This study emphasizes the urgent need for implementing shared decision-making systems and transitioning toward outcome-based reimbursement models to protect healthcare consumers' rights and ensure rational medical utilization.

본 연구는 2014년부터 2023년까지 10년간의 국가 보건의료 빅데이터를 활용하여 경막외 차단술 이용 양상의 구 조적 특성을 분석하고, 의료 소비자 관점에서 진료 선택권과 정보 비대칭성 문제를 규명하고자 수행되었다. 통증의 주관 적 특성으로 인해 발생하는 의료 공급자와 소비자 간의 정보 비대칭성은 의료 소비자로 하여금 공급자의 권고에 의존하 게 만들며, 이는 반복적인 시술 이용으로 이어지는 구조적 환경을 형성한다. 분석 결과, 지난 10년간 연간 시술 이용자 수는 약 6.8% 증가하였으며, 1인당 평균 시술 횟수는 2.0회 이상의 높은 치료 강도를 꾸준히 유지하였다. 특히 시술의 96.4%가 외래에서 발생하고 여성 소비자(57.1%)의 이용 비중이 높게 나타난 것은, 만성통증 관리가 의료 소비자의 일상 적 선택보다는 고착화된 외래 진료 구조와 행정적 급여 기준에 의해 유도될 가능성을 시사한다. 이러한 결과는 의료 소 비자가 자신의 상태를 객관적으로 파악하고 주체적인 치료 결정을 내리기 어려운 국내 통증 진료 환경의 단면을 보여준 다. 본 연구는 의료 소비자의 알 권리 보장과 합리적 선택을 위해 환자 중심의 공동 의사결정 시스템 도입 및 성과 기반 급여 체계로의 전환이 시급함을 제언한다.
