Background: As dental implant therapy has become more widespread, the prevention and management of peri-implantitis and peri-implant mucositis have gained increasing clinical importance. Long-term maintenance, early detection, and risk-factor control are essential to improving treatment outcomes, and dental hygienists play a central role in these processes. However, limited research has systematically analyzed the specific tasks performed by dental hygienists in peri-implant disease management. This study reviewed intervention strategies used for peri-implantitis and derived the scope of dental hygienists’ roles based on current evidence. Methods: A systematic review was conducted following PRISMA guidelines, targeting studies published between 2015 and 2024. Literature searches were performed in KISS, RISS, NDSL, and PubMed. Clinical studies reporting non-surgical or surgical management of peri-implantitis or peri-implant mucositis were included. Thirty-seven studies met the eligibility criteria. Data regarding study design, intervention type, clinical outcomes, and dental hygienist involvement or potential tasks were extracted and analyzed. Results: Non-surgical interventions were most frequently reported (22 studies), including mechanical debridement, air-polishing, antiseptic irrigation, local antibiotics, probiotics, and laser-assisted therapy. Surgical approaches—such as flap surgery, guided bone regeneration, and laser-assisted regenerative procedures—were also documented. Across studies, the dental hygienist’s roles were categorized as follows: prevention and risk-factor counseling, non-surgical mechanical debridement, support for chemical and antimicrobial adjunctive therapies, assistance in pre- and postoperative surgical care, measurement of clinical parameters and disease monitoring, maintenance and recall program management, and patient education and data management. Conclusion: Peri-implant disease management requires a multifaceted and long-term approach, and dental hygienists contribute significantly across prevention, treatment, and maintenance phases. By systematically identifying their evidence-based roles, this study provides foundational insights for developing clinical guidelines and improving educational and policy frameworks for dental hygiene practice.

연구배경: 임플란트 보급의 증가와 함께 임플란트 주위염 및 점막염의 예방·관리에 대한 중요성은 더욱 강조되고 있다. 특히 조기 발견, 위험요인 조절, 장기적 유지관리 등은 임플란트의 성공과 예후를 결정하는 핵심 요소로, 이 러한 과정에서 치과위생사의 전문적 역할이 점차 확대되고 있다. 그러나 임플란트 주위염 관리 과정에서 치과위생 사가 실제로 수행하거나 수행 가능한 업무 범위를 체계적으로 정리한 연구는 매우 제한적이다. 본 연구는 임플란 트 주위 질환 관리에 적용된 다양한 중재 기법을 고찰하고, 그 과정에서 도출될 수 있는 치과위생사의 역할을 근거 기반으로 제시하고자 하였다. 연구방법: PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) 지침에 따라 2015–2024년 발 표된 국내·외 문헌을 대상으로 체계적 문헌고찰을 수행하였다. KISS, RISS, NDSL, PubMed에서 검색을 시행하 였으며, 임플란트 주위염 또는 주위 점막염의 비외과적·외과적 처치를 보고한 임상 연구를 포함 기준으로 설정하 였다. 최종적으로 37편의 문헌이 선정되었으며, 연구 설계, 중재 유형, 임상 결과, 치과위생사 업무 관련 요소 등을 추출·분석하였다. 연구결과: 분석 결과, 비외과적 처치를 다룬 연구가 가장 많았으며 기계적 세정, 공기 연마, 항균제·프로바이 오틱스·레이저 보조요법 등 다양한 중재가 보고되었다. 외과적 처치 연구에서는 flap surgery, GBR (guided bone regeneration), 레이저 병행술식 등이 포함되었다. 문헌에서 도출된 치과위생사의 역할은 예방 및 위험요인 관리, 비 외과적 기계적 세정, 보조요법 지원, 수술 전·후 관리, 임상지표 측정 및 모니터링, 유지관리·리콜 운영, 환자 교 육 및 자료 관리 등으로 범주화되었다. 결론: 본 연구는 임플란트 주위 질환 관리가 다면적 접근을 필요로 하며, 치과위생사는 예방·치료·유지 전 과정 에서 핵심적 기여를 할 수 있음을 확인하였다. 이는 향후 임상실무 가이드라인 개발과 치위생 교육·정책 개선을 위한 기초자료로 활용될 수 있다.
