This study conducted a scoping review to examine the effects of virtual reality (VR)-based music interventions on anxiety disorders, with the dual purpose of mapping existing research trends and identifying directions for future inquiry. Following a scoping review protocol, international studies published between 2015 and 2025 were systematically searched in the PubMed and Scopus databases using the keywords virtual, music, anxiety, therapy, and rehabilitation. The search initially yielded 407 records, of which 14 studies met the predefined eligibility criteria and were included in the final analysis. The review synthesized findings across several domains: the general characteristics of interventions, the types and modalities of musical activities, the VR devices and systems employed, the measurement tools utilized, and the reported effects and underlying mechanisms of VR and music. With respect to participant characteristics, studies involved diverse age groups, though the largest proportion comprised university students in their twenties and patients in medical contexts. Regarding musical activity, passive music listening was the most prevalent modality. In terms of technology, standalone head-mounted displays (HMDs) emerged as the most commonly used VR hardware, and Unity was the predominant software platform, reflecting a steadily increasing adoption trend. In terms of therapeutic outcomes, VR-based music interventions demonstrated positive effects on the emotional functioning of individuals with anxiety disorders. Music contributed to anxiety reduction through attentional distraction and physiological entrainment, thereby alleviating anxiety-related physiological responses. Moreover, the immersive nature of VR, combined with customizable features, was shown to further enhance the efficacy of interventions by promoting greater emotional relief and psychological comfort.

본 연구는 가상현실(virtual reality, VR) 기반 음악중재가 불안장애에 미치는 효과에 대한 연구를 대상으로 주제 범위 문헌고찰을 실시하여, 현재 연구 동향을 파악하고 향후 연구에 대한 제안을 도출하고자 하였다. 이를 위해 주제범위 문헌고찰 프로토콜에 근거한 방법론을 적용하여 2015년부터 2025년까지 발표된 해외 연구를 대상으 로 PubMed와 Scopus 데이터베이스에서 ‘virtual’, ‘music’, ‘anxiety’, ‘therapy’, ‘rehabilitation’ 등을 핵심 어로 검색하였다. 총 407편의 문헌이 검색되었으며, 이 중 선정 기준에 부합하는 최종 14편을 선정하였다. 분석 내용은 중재의 일반적 특성, 음악의 유형과 활동 방식, 중재에 사용된 VR 기기와 시스템, 각 연구에서 활용된 측정 도구, 그리고 음악과 VR의 효과 및 작용 메커니즘으로 구성되었다. 분석 결과, 일반적 특성 측면에서 참여자 의 연령대는 다양하였으며, 이 중 가장 높은 비율을 차지한 연령층은 20대 대학생으로 나타났다. 음악 활동 유형에서는 음악 감상이 가장 보편적으로 사용되었고, 시스템 측면에서 독립형 헤드마운트 디스플레이(head mounted display)가 Unity 소프트웨어를 기반으로 운영되는 것으로 나타났다. 효과의 측면에서 VR 기반 음악 중재는 불안장애 환자의 정서기능 개선에 긍정적 영향을 미친 것으로 나타났는데, 이러한 효과는 음악이 주의 분산(distraction)과 생리적 동조화 기제를 통하여 불안의 전반적 경험과 불안 관련 생리 반응을 완화시킨 것으로 분석되었다. 또한, VR이 제공하는 몰입감과 사용자 맞춤형 설정은 불안 완화에 기여한 것으로 나타났다.
