The purpose of this study is to critically analyze the current status and issues of China's medical insurance system and explore ways to improve the system. To this end, the study adopted the four dimensions of studies of product—allocation, provisions, delivery system, and finance—from Gilbert and Terrell’s social welfare policy analysis framework. Specifically, first, the dimension of allocation was analyzed by considering the scope of the main application objects and the appropriateness of the selection methodology. Second, the dimension of provisions was investigated with a focus on the types of benefits, the appropriateness of those benefits, and their function in income redistribution. Third, the dimension of the delivery system was examined with a focus on the organizational structure of medical insurance management and operation, as well as the methods of service provision. Lastly, in the financial dimension, the method of financing was analyzed. As a result of the study, the limitations of the scope of the application objects, optional entry system, and absence of a dependent qualification system cause the problem of not signing up for the insurances. In terms of provisions, several issues were identified, including a regressive benefit structure, reduced access to healthcare due to restrictions based on the household registration system, and significant disparities in healthcare services. In terms of the delivery system, inefficiencies resulting from the separation of medical insurance administrative bodies were identified. From a financial perspective, issues such as disparities in premium contributions between urban and rural residents, as well as insufficient coverage due to high copayment, were also analyzed. To address these issues, the study proposes the following measures for improvement. First, in terms of application objects, the study proposes expanding the eligible population, shifting from a optional entry system to a mandatory enrollment system, and introducing a dependent eligibility system. Second, in relation to provisions, the reform agenda includes abolishing individual accounts and restrictions of household registration, as well as determining the benefits level under the authority of the central government. Third, with regard to the delivery system, the study recommends integrating the medical insurance management and operation system. Finally, in relation to financing, the study suggests transitioning from a flat-rate scheme to an earnings-related system, reduction of copayment, and financial support from the central government as key measures for improving the system.

본 연구의 목적은 중국 의료보험제도의 현황과 문제점을 비판적으로 분석하여 제도의 개선방안을 모색하는 데 있다. 이를 위해서 길버트와 테렐(Gilbert ＆ Terrell)의 사회 복지정책 분석 유형 중에서 산출분석의 4가지 차원인 대상, 급여, 전달체계, 재정을 분석틀로 활용하였다. 즉, 적용대상, 급여, 전달체계, 재원을 분석틀의 구성요소로 활용 하여 2가지 유형의 기본의료보험을 분석하였다. 구체적으로 첫째, 적용대상은 주요 적 용대상자가 누구이며, 대상자 선정방법은 적절한지, 배제된 대상자는 없는 지 등을 고 려하여 분석하였다. 둘째, 급여는 급여의 형태는 어떻게 이루어져 있으며, 그 급여가 가입자의 생활을 어느 정도 보장해 주고 있는지, 그리고 소득재분배 기능을 하고 있는 지에 초점을 두고 분석하였다. 셋째, 전달체계는 급여가 어떤 방식으로 어떤 조직에 의해서 서비스가 제공되고 있는가를 분석하였다. 마지막으로 재원은 의료보험제도의 재원을 어떤 방식으로 조달하고 있는지를 분석하였다. 연구의 분석결과, 적용대상의 측면에서 적용대상 범위의 제한성, 임의가입 방식과 피부양자 자격제도의 부재로 인한 미가입 문제 등이 발견되었다. 급여에 있어서는 역진적인 급여 구조, 호적 제한으로 인한 의료접근성 저하, 의료격차의 문제가 있었다. 전달체계에서는 의료보험 집행기관 의 분립으로 인한 비효율성 무제가 있었고, 재정차원에서는 도시주민과 농촌주민의 보험료 부담의 격차문제, 높은 본인부담금으로 인한 보장성의 미흡 문제 등이 분석되 었다. 이러한 문제점을 해결하기 위해 다음과 같은 개선방안을 제언하였다. 첫째, 적용 대상과 관련하여 적용대상 범위의 확대, 임의가입에서 강제가입으로의 전환, 피부양자 자격제도의 도입 등을 제언하였다. 둘째, 급여와 관련하여 개인계정과 호적제한의 폐 지, 중앙정부차원에서의 급여수준 결정 등을 제언하였다. 셋째, 전달체계와 관련해서는 의료보험 관리운영체계의 일원화 등을 제언하였다. 마지막으로 재정과 관련해서는 정 액보험료 부과방식에서 소득비례방식으로의 전환, 본인부담금 경감, 중앙정부의 재정 지원 등을 제도의 개선방안으로 제언하였다.
