This study aims to broaden the understanding of healthcare utilization among immigrants by examining the unique pattern of Traditional Korean Medicine (TKM) use among permanently returned Sakhalin Koreans, particularly their self-imposed limitation on the number of acupuncture treatments. To this end, the study first reviews previous research on the use of TKM by immigrants. It then provides an overview of the history of migration and return of Sakhalin Koreans and analyzes their use of TKM after resettlement in South Korea. The study interprets and explains the self-limiting pattern of acupuncture use among permanently returned Sakhalin Koreans through the lens of how Traditional Chinese Medicine (TCM) was received and integrated into Soviet and Russian medical systems. Based on a combination of literature review, anthropological fieldwork—including interviews and participant observation at the Sakhalin Centers and the Sakhalin Senior Centers—and formal interviews with TKM doctors, this study finds that the distinctive pattern of TKM use among permanently returned Sakhalin Koreans stems from their adherence to biomedicalized acupuncture guidelines shaped by the Soviet and Russian reception of TCM. The findings of this study suggest that immigrants can interpret and utilize the healthcare systems of their destination countries through the lens of their own cultural and historical contexts.

이 연구는 영주귀국사할린한인들이 스스로 침 치료 횟수를 제한하는 독특한 한 의학 이용 양상을 검토함으로써 이주민들의 수민국 의료이용에 대한 이해의 폭을 넓히는 것을 목적으로 한다. 이를 위해 먼저 이 연구는 이주민들의 한의학 이용에 대한 선행연구를 개관한다. 이어 영주귀국사할린한인들의 이주와 역이주의 역사 를 개관하고 이들의 귀환 후 한의학 이용 양상을 살펴본다. 그리고 영주귀국사할 린한인들의 한의학 이용 양상, 특히 침 치료 횟수를 스스로 제한하는 양상을 소련 과 러시아에서 중의학이 수용되고 통합되는 과정을 통해 해석하고 설명한다. 문헌 조사와 함께 사할린 센터와 사할린 경로당에서의 면담과 참여관찰을 포함한 인류 학적 현지조사, 한의사들과의 공식적인 면담 등을 통해 이 연구는 영주귀국사할린 한인들의 독특한 한의학 이용 양상이 소련과 러시아에서 생의학화된 중의학의 침 처치 지침을 그대로 따른 결과라는 것을 발견하였다. 이 연구의 결과는 이주민들 이 자신들의 문화적 맥락에서 이주국의 의료를 해석하고 이용할 수 있다는 것을 시사한다.
