Objective: This study investigates the participation trends in return-to-work programs among patients with work-related injuries or illnesses, following recent revisions to industrial accident insurance policies. Methods: A retrospective cohort study was conducted using electronic medical records from hospitals under the Korea Workers' Compensation and Welfare Service. A total of 3,852 patients with work-related injuries or illnesses who participated in return-to-work programs between January 2018 and December 2023 were included. Following the implementation of revised industrial accident insurance policies, the participants were categorized into two groups. Changes in the participants' demographics, occupational injuries, return-to-work programs, and job-related characteristics were analyzed. Results: The proportion of individuals aged 60 years and older increased, accompanied by an increase in upper-limb injuries. Furthermore, the incidence of lower back pain, musculoskeletal disorders, and occupational diseases increased. In terms of the return-to-work program characteristics, the average training duration decreased, whereas the proportion of participants with regular employment increased. Conclusion: This study emphasizes the significance of industrial-accident prevention policies and highlights the necessity for institutional improvements and customized return-to-work programs to enhance return-to-work outcomes.

목적 : 본 연구는 산재보험정책 변화에 따른 직업복귀훈련에 참여한 산재환자의 동향을 분석하여 국내 의료기관 기반 직업재활의 기초자료를 제시하고자 한다. 연구방법 : 근로복지공단 소속병원 전자의무기록을 활용한 후향적 코호트 조사 연구로 2018년 1월부터 2023년 12월까지 직업복귀훈련을 실시한 산재환자 3,852명을 대상으로 하였다. 산재보험정책 변화에 따른 동향을 분석하기 위해 2018~2020년 실시한 1,649명과 2021~2023년 실시한 2,203명을 각 A, B 두 그룹으로 나누어 인구통계학적, 산재관련, 직업복귀훈련 및 직업관련 특성의 변화를 분석하였다. 결과 : 연령에서는 60세 이상 비율이 증가하였고 손상 부위에서는 상지 손상 비율이 높아졌다. 손상 형태와 산재 유형에서는 요통 및 근골격계 질병과 업무상 질병 비율이 모두 증가하였다. 직업복귀훈련 특성에서는 훈련 기간이 줄어든 반면 직업관련 특성에서는 상용직 비율이 높아졌다. 결론 : 산재보험정책 변화로 직업복귀훈련 참여자들의 근골격계 업무상 질병은 증가하였고 일용직 및 임시직 비율은 감소하였다. 본 연구를 통해 산재예방정책과 보다 효과적인 직업복귀를 위한 제도개선 및 맞춤형 직업복귀훈련의 중요성을 제시하였다.
