Background: This study aimed to provide foundational data for the instructional design of an effective and customized smoking cessation program based on the ADDIE model and the Transtheoretical Model (TTM). The program development was informed by a comprehensive literature review and an analysis of the characteristics and needs of the target population. Methods: In September 2024, 40 adult smokers were recruited through an online community and classified into the precontemplation, contemplation, and preparation stages based on the stages of behavior change. A literature review was conducted on smoking cessation programs published between 2014 and 2023. Data were collected on participants’ general characteristics, smoking-related factors, oral health symptoms, smoking-related knowledge, and TTM components, including processes of change, decisional balance, and self-efficacy. The data were analyzed using descriptive statistics, one-way ANOVA, and Fisher’s exact test with SPSS 25.0. Results: Participants reported using both combustible cigarettes and e-cigarettes. Knowledge levels regarding e-cigarette safety, nicotine dependence, and the relationship between smoking and stress were generally low. Among the processes of change, dramatic relief, self-reevaluation, and stimulus control scored low in the precontemplation and contemplation stages, whereas social liberation and helping relationships scored relatively high. Self-efficacy was consistent across behavior change stages and was identified as a mediating factor for smoking cessation success. Conclusion: This study analyzed the characteristics and needs of adult smokers according to their stages of behavior change and provided foundational data for the development of a smoking cessation program based on the ADDIE model and the TTM. These findings offer valuable insights for the systematic and structured design of customized instructional programs.

목적: 본 연구는 문헌 조사와 대상자의 특성 및 요구도 분석을 바탕으로, ADDIE 모형과 범이론적 모형을 적용한 효과적인 맞춤형 금연 프로그램 개발을 위한 교수설계의 기초자료를 제공하고자 하였다. 방법: 2024년 9월, 온라인 커뮤니티를 통해 성인 흡연자 40명을 모집하였으며, 이들을 행동변화단계에 따라 계 획전단계, 계획단계, 준비단계로 분류하였다. 2014년부터 2023년까지 발표된 금연 프로그램 관련 문헌을 검토하 였고, 대상자의 일반적 특성, 흡연 관련 특성, 구강 내 증상, 흡연지식, 그리고 TTM의 구성 요소인 변화과정, 의 사결정균형, 자기효능감을 파악하였다. 수집된 자료는 SPSS 25.0 프로그램을 이용하여 기술통계, 일원분산분석, 피셔의 정확 검정을 통해 분석하였다. 결과: 대상자들은 일반담배와 전자담배를 사용하고 있었으며, 전자담배의 안전성, 니코틴 의존성, 흡연과 스트레 스 간의 관련성에 대한 흡연지식 수준은 전반적으로 낮게 나타났다. 변화과정 중 극적 해소, 자기재평가, 자극통 제는 계획전단계와 계획단계에서 낮은 점수를 보였으며, 사회적 해방과 조력관계는 비교적 높은 점수를 나타냈 다. 자기효능감은 행동변화단계 간 큰 차이를 보이지 않았으나, 금연 성공을 예측하는 중재변수로 확인되었다. 결론: 본 연구는 행동변화단계에 따른 성인 흡연자의 특성과 요구를 분석하고, ADDIE 모형과 범이론적 모형을 적용한 금연 프로그램 개발을 위한 기초자료를 제시하였다. 이러한 결과는 교수설계의 체계적이고 구조적인 맞 춤형 프로그램 개발에 유용한 근거자료로 활용될 수 있을 것이다.
