On ocean-going vessels, the Person in Charge of Medical Care Onboard (PCMCO) is responsible for managing crew health, providing medical care for onboard patients, and maintaining sanitary conditions. This qualification can be obtained either by passing the medical administrator exam or by holding a relevant medical or hygiene-related license. I conducted this study to evaluate the validity of obtaining the PCMCO qualification through relevant licenses and propose improvements. To analyze this, I compared PCMCO exam subjects, relevant licensing exams, and university curricula. The analysis showed that medical doctors and emergency medical technicians covered all PCMCO subjects, whereas dentists, Korean medicine doctors, pharmacists, and hygienists did not. Nurses included all PCMCO subjects in both their qualification exams and university curricula. Internationally, the UK recognizes medical doctors, nurses, and emergency medical technicians for PCMCO qualifications, while the US includes medical doctors, nurses, Marine Physician Assistants, and Hospital Corpsmen. Based on these findings, I propose revising the Seafarers’ Work Processing Instruction to recognize PCMCO qualifications for medical doctors, nurses, and emergency medical technicians. This revision aims to establish a structured qualification system and improve medical response capabilities on ships.

원양을 항해하는 선박에는 선원들의 건강관리 및 선내 환자에 대한 의료 관리, 선내 환경위생 유지 등을 위해 선내 의료관리자를 두고 있으며, 해당 자격을 취득하는 방법은 의료관리자 자격시험을 통한 방법과 의료 위생 관련 면허 혹은 자격증을 보유한 경우 취득할 수 있다. 본 연구는 선박 내 의료관리자 자격증 취득 방법 중 관련 면허 및 자격증을 통한 자격취득의 타당성을 검토하고 개선점을 제안하고자 수행되었다. 이를 위해 의료관리자 자격시험의 과 목(의료관계법령, 기초응급처치학, 기초간호학, 공중보건학)과 관련 면허(의사, 치과의사, 한의사, 약사, 위생사, 응급구 조사, 간호사)의 시험 과목 및 대학교 교육과정을 비교·분석하였다. 또한, 영국과 미국의 의료관리자 인정 면허 기준 을 조사하여 국제적인 기준과의 차이를 검토하였다. 분석 결과, 의사와 응급구조사, 간호사의 경우 해당 시험 과목이 모두 포함된 반면, 치과의사, 한의사, 약사, 위생사의 경우 일부 과목이 부족한 것으로 나타났다. 해외 사례를 살펴본 결과, 영국과 미국에서는 공통적으로 의사와 간호사 면허를 의료관리자 인정 면허로 규정하고 있음을 확인할 수 있었 다. 이러한 분석을 바탕으로, 현행 선원업무처치지침을 개정하여 의료관리자 인정 면허를 의사, 간호사, 응급구조사로 조정할 것을 제안하였다. 이를 통해 선박 내 의료관리자 자격 인정 기준을 보다 체계적으로 정립하고, 실질적인 의료 대응 능력을 강화하는 데 기여할 수 있을 것으로 기대된다.
