The purpose of this study was to identify the difficulties of primary caregivers among the experiences of using the long-term care insurance system for the elderly, and to identify the practical and policy implications of social welfare to improve them. This was a qualitative study in which five research participants were interviewed in depth and analyzed through Giorgi's phenomenological research method. As a result of the analysis, 35 semantic units, 15 sub-components, and 5 high-order components were derived from the experiences of the primary caregivers of the elderly, using the long-term care insurance system for the elderly. The top five components are 『Responsibilities and rewards in difficult care』, 『Helpful but insufficient system』, 『Another caring role that needs to be met』, 『Primary caregiver in ambivalent emotions』, and 『Wish for good care』. The results of this study show that it is necessary to further strengthen the care management system to support the integrated perspective of care in long-term care insurance for the elderly, and it is necessary for the elderly, primary caregivers, and facility operators to embrace the best practices of good care is in long-term care facilities. It is necessary to clearly define good care for the elderly. Additionally, it is necessary to check and improve the administrative system of the long-term care insurance system delivery system for the elderly, and it is necessary to prepare a support system for primary caregivers and families caring for the elderly, before and after entering the long-term care insurance system. Also, It is necessary to improve the professionalism of the care coordinator of the National Health Insurance Service and the case manager of the facility, who play roles as care managers.

본 연구는 노인장기요양보험제도 이용 경험 중 주보호자들의 어려움은 무엇이고, 이를 개선하기 위해 사회복 지 실천적, 정책적 함의를 모색하는 것을 목적으로, 제도 진입부터 노인요양시설을 이용까지 직접 경험해 본 주 보호자 5명을 연구 참여자로 심층 인터뷰하여 이를 Giorgi의 현상학적 연구 방법을 통해 분석한 질적 연구이다. 연구 결과 노인장기요양보험제도 이용 노인의 주보호자의 경험에서 35개의 의미단위와 15개의 하위 구성요소, 그리고 5개의 상위 구성요소가 도출되었다. 상위 구성요소 5개는『힘겨운 돌봄 속 책임과 보람』, 『도움이 되 지만 미흡한 제도』, 『감당해야 하는 또 다른 돌봄의 역할』, 『양가적 감정에 놓인 주보호자』, 『좋은 돌봄에 대한 바람』로 나타났다. 본 연구를 통하여, 노인장기요양보험에 있어서 통합적 관점의 돌봄을 지원할 케어매니 지먼트 체계를 더욱 견고히 할 필요가 있으며, 요양시설에서의 좋은 돌봄이 무엇인지를 인식할 수 있도록 좋은 돌봄이 무엇인지 명확히 공표하는 것 필요하다. 또한 노인장기요양보험제도 전달체계 중 행정체계에 대한 점검과 개선이 요구되고, 제도 진입 전, 후 노인을 돌보는 주보호자와 가족을 위한 지원 시스템 마련이 필요하며, 케어 매니저 역할을 수행하는 국민건강보험공단의 케어조정자와 시설의 사례관라자의 전문성 향상이 요구된다.
