Treatment outcomes of geriatric patients would depend on forming as well as maintaining the relationships with therapists, caregivers, and family members. Empathy of the patients have a critical impact on quality of the relationships. Empathy deficits in neurodegenerative dementias, such as frontotemporal dementia, Alzheimer’s disease, and Parkinson’s disease, aggravate the burden of the therapists and caregivers. This brief paper reviewed current findings regarding methods of measuring empathy, pathological and/or aging impact on empathy, and its neural mechanisms. It was especially suggested that informant-rated empathy measurement should be considered rather than self-rated one when evaluating empathy changes in dementia patients. It was also discussed that research on pathological aging of empathy would be helpful in treating and/or coping with dementia patients because most of them were associated with the empathy deficits.

노인환자의 치료 효과는 치료자나 간병인, 가족들과의 관계 형성이나 유지에 달려 있다. 노인환자의 공감능력은 그 관계의 질에 결정적인 영향을 미친다. 전두측두엽 치매나 알츠하이머병, 파킨슨병과 같은 신경퇴행성 치매를 앓는 노인환자들이 보이는 공감 능력의 상실은 치료자나 보호자에게 부담을 가중시킨다. 본고에서는 근래까지 이루어진 공감의 병리적 노화에 관련된 연구결과들을 간략하게 개 관했는데, 공감 능력을 측정⋅평가하는 방법에 따른 연구결과의 차이를 비롯하여 병리적 노화의 신 경 기제 등을 알아보았다. 특히 치매환자들을 대상으로 공감의 노화를 평가하는 연구에서는 타인이 환자의 공감을 평정하는 방법을 이용할 필요가 있음을 제안하였고, 공감 능력의 결함을 초래하는 치 매에 대한 연구가 효과적인 치료 전략에 주는 함의도 논의하였다.
