This study is on drug use, addiction, and dealing experience in eight North Korean defectors. Researchers analyzed participants’ reasons for drug use and dealing, contexts, and results. Data were structured by an in-depth one on one interview. Researchers approached in the multiple units of analysis multi-case design proposed by Yin(2018). 96 summarized concepts and 20 subjects were derived from data and rearranged to the analysis unit of causes and context results. The results are as follows. The causes included a shelter of psychological comfort and a rise in pleasure desire. The contexts were classified into a political and social context, a medical and environmental context, and an individual and family context. First, a political and social context included the collapse of ethical foundation in society, a deadly threat of life, a state where moral authority is lost, means to a rise in status, mammonism, proletarian’s social property, corruption chain, and the lack of social alert. Second, a medical and environmental context included alternative medicines, popularization of drug, punitiveness policy without treatment, and high barrier to health care. Third, an individual and family context included generation transfer of drug use, sales taking self-consumption, and internalized desperation. The results were being driven to the edge of a precipice and a collapse of life foundation. It is necessary to accurately perceive therapeutic approach and medication and provide scientific knowledge rather than punish for North Korean defectors who got addicted to drug in North Korea with remarkably low medical system. Researchers discussed based on study results and made practical suggestions to help North Korean defectors’ recovery of drug addiction.

본 연구는 북한 이탈주민 중 남성 8명의 마약 사용과 중독, 밀매 경험에 관한 사례연구이 다. 연구자들은 연구참여자의 마약 사용과 밀매의 원인 그리고 맥락과 결과를 분석하고자 자 료는 1대1 심층 면담으로 구성하였다. 연구자들은 Yin(2018)이 제시한 다중사례 다중 분석 단위로 접근하였다. 연구자들은 자료에서 96개 개념 요약과 20개의 주제를 구성하여, 이를 원인과 맥락 결과라는 분석 단위로 재배열하였다. 연구 결과는 다음과 같다. 원인은 심리적 위안의 도피처, 쾌락 욕구의 상승이라는 주제가 나타났다. 맥락은 정치·사회적 맥락, 의 료·환경적 맥락, 개인·가족적 맥락으로 구분하였다. 첫째, 정치·사회적 맥락으로는 사회 적 윤리 토대의 붕괴, 극한의 생존 위협, 도덕적 권위가 실추된 국가, 신분 상승의 수단, 황금 만능주의 풍조, 무산자의 사회적 자산, 부패의 고리, 사회적 경각심의 부재로 나타났다. 둘 째, 의료·환경적 맥락으로 대체의약품, 마약의 대중화, 치료 없는 엄벌주의 정책, 높은 의료 서비스 장벽으로 나타났다. 셋째, 개인·가족적 맥락은 마약 사용의 세대 이전, 자기 소비를 취한 판매, 절망의 내면화로 나타났다. 결과는 벼랑 끝으로 내몰림, 삶의 기반 붕괴로 나타났 다. 의료시스템의 접근성이 현저하게 저하된 북한에서 마약에 중독된 북한 이탈주민들을 처 벌보다는 치료적 접근과 약물에 대한 정확한 인식과 과학적인 지식의 전달이 필요하다. 연구 자들은 연구 결과에 기반하여 논의하였으며, 또한 북한 이탈주민들의 마약 중독에서의 회복 을 도울 수 있는 실천적 제언하였다.
