The objective of this qualitative case study was to explore in-depth counseling experiences of counselors for Advance Directives(AD) on life-sustaining treatment implemented based on the Life-sustaining Medical Act. Results of this study can be used to improve our understanding of the worksite of AD and to propose an institutional development plan. Participants of this study were five counselors who had more than two years of practical experiences at AD registration institutions. Research data were collected through semi-structured questionnaires and in-depth interviews. Three main categories and seven sub-categories were extracted. The first category of ‘Counselors' experience in the counseling process’ showed a change in the counselor's perception of death and started the discussion about death. The second category of ‘The disparity between the system and counseling field’ showed a dilemma about prioritizing counseling goals, a reality that could not reflect client's needs, and gaps in the registration and operating system. The third category of ‘Recommendations for Counseling System’ included the following. Programs for an efficient management system should be established. It is necessary to train competent counselors and to provide them with stable job environment. Finally, based on research results, practical and policy implications were provided for the enhancement of AD plans.

본 연구는 연명의료법에 근거하여 시행되고 있는 사전연명의료의향서 상담자의 상담경험을 심층적으로 탐색 한 질적 사례 연구 이다. 연구 목적은 사전연명의료의향서 실천 현장의 이해를 돕고, 제도적 발전 방안을 살펴 보고자 한다. 연구 참여 대상자는 사전연명의료의향서 등록기관에서 2년 이상 실무 경험이 있는 상담자 5명으 로 구성되었다. 연구 자료 수집은 반구조화 된 질문지와 심충 면담을 통해 수집하였다. 분석 결과 사전연명의료 의향서 상담자의 상담 경험에 주요 영역들은 3개의 상위범주와 7가지 하위 범주로 도출되었다. 첫째, 상담자들 의 상담과정 경험은 상담자의 죽음인식 변화, 죽음 논의의 시작으로 나타났다. 둘째, 제도와 상담현장의 간극은 상담 목표 우선순위에 대한 딜레마, 내담자의 욕구를 반영하지 못한 현실, 등록･운영 체계의 틈새로 이루어졌 다. 셋째, 앞으로 나아가야 할 현장의 상(像)은 전문 상담자 양성과정 및 체계적 운영 시스템을 구축하기, 근로 환경의 안정성을 찾아나가기 등으로 나타났다. 마지막으로 연구결과에 근거하여 사전연명의료의향서의 발전방 안을 위한 실천적･정책적 함의를 제언하였다.
