In general, it can be considered that there are two aspects in community care, one that values quality of life and the other that aims to reduce the government's social welfare financial burden. This study examines from a historical perspective how the "quality of life" and "cost savings" that have been difficult to achieve simultaneously had been merged in UK community care policies. The periods of the analysis are set as ‘germination period’ of community care, ‘introduction and adjustment period’ of community care through the Seebohm report, ‘legalization period’ through the Griffith report, and ‘system adjustment and specification period’. Through reforms in each period, community care in the UK has become a basic policy that oversees the macro and micro aspects of social services. In particular, it is revealed that the merger of quality of life and cost-efficiency has been promoted in the form of establishing a service integration organization by local governments, establishing an integrated system of case management, and integrating medical and social services. In promoting this integration, decentralization has also been developed to strengthen the roles and responsibilities of local governments. After all, the role of local governments is key to integrating social services. Therefore, it can be seen that the integration policy and decentralization should be carried out simultaneously. The decentralization for community care is presented as a comprehensive transfer of authority to local governments and a transfer to a sub-system of decision-making authority for regional support, and “decentralization to the community” to solve local challenges by residents.

커뮤니티 케어는 일반적으로 삶의 질을 중시하는 측면과 정부의 사회복지 재정 부담을 덜기 위한 측면 의 두 가지 입장을 동시에 지닌다. 본 연구는 영국의 커뮤니티 케어 정책에서 양립하기 어려운 ‘삶의 질’과 ‘비용 절감’을 어떻게 병합하며 추진해 왔는지 역사적 관점에서 고찰한다. 분석의 시기 구분은 커 뮤니티 케어의 ‘발아기’, 시봄보고서를 통해서 커뮤니티 케어의 기반이 마련되는 ‘도입‧조정기’, 그리피스 보고서를 통한 ‘법제화기’, 이후의 ‘제도 조정 및 구체화기’로 설정하였다. 영국의 커뮤니티 케어는 시기 별로 주요한 개혁을 거쳐 사회서비스의 거시․미시를 총괄하는 기본 방침으로 자리잡는다. 그 가운데 지 방자치단체의 서비스 통합기구 창설, 사례관리의 통합 시스템 구축, 의료와 사회서비스의 통합과 같은 형태로 삶의 질과 비용 효율성의 병합이 추진됨이 나타난다. 이러한 통합의 추진에서 지방정부의 역할과 책임을 강화하는 분권화도 함께 전개되어 왔다. 사회서비스의 통합화는 결국 지방자치단체의 역할이 관 건으로, 통합 정책은 지방분권화와 동시적 추진이 주요 과제임을 드러낸다. 커뮤니티 케어에서 지향되는 분권은 지방자치단체로의 포괄적 권한 이양, 지역 지원을 위한 의사결정권한의 하위체계 이양, ‘지역내 분권’을 통한 주민들의 지역과제해결 모색으로 제시된다.
