This study examined the possibility of addiction treatment through the recognition of the domestic and international status of digital therapeutics developing into next-generation new drugs and the regulatory environment. To define software as a digital therapeutics, the IMDRF's SaMD analysis summarizes differences from SiMD, risk categories from SaMD, quality control factors from the software aspect and clinical evaluation methods. It also confirmed that digital therapeutics are effective for chronic diseases that require psychological factors and behavioral changes. It is believed to be an effective treatment for addiction such as Internet addiction and gambling addiction, which could replace existing drug treatments. In a proposal to activate digital therapeutics, First, lower entry barriers by securing ease of licensing and clinical trials and providing legal grounds. Second, it is necessary to establish clear definitions and scope of digital treatments, guidelines for determining their validity, and to prepare items and implementation environmental standards for clinical trials. Finally, it is necessary to have an institutional environment that can be applied to RWE, RWD data collection and monitoring criteria and clinical evaluation for the presentation of evidence to ensure the safety and effectiveness of the treatment.

본 연구는 차세대 신약으로 발전하는 디지털 치료제의 국내외 현황과 규제 환경의 인식을 통하여 중독치료의 가능성을 알아보았다. 소프트웨어를 디지털 치료제로 정립하기 위하여 IMDRF의 SaMD 분석을 통하여 기존 SiMD와의 차이점과 SaMD의 위험성 범주 분류와 소프 트웨어 측면의 품질 관리 요소 및 치료제로서의 임상 평가 방법을 정리하였다. 또한 디지털 치료제는 심리적 요인과 행동 변화를 요구하는 만성질환에 효과적임을 확인하였다. 이는 인 터넷 중독이나 도박 중독 등의 중독치료에 효과적인 치료제로 기존 약물 치료 등을 대체할 수 있을 것으로 사료된다. 디지털 치료제 활성화를 위한 제언으로 첫째, 인허가 및 임상시험의 용이성 확보와 법리적 근거 마련을 통하여 진입장벽을 낮추고 둘째, 디지털 치료제에 대한 명 확한 정의와 범위, 유효성 판단을 위한 가이드라인 마련 및 임상시험에 대한 항목 및 시행 환 경 기준 마련이 필요하다. 마지막으로 치료제의 안전성과 효과성 확보를 위한 근거 자료 제시 에 대한 RWE, RWD 방식의 데이터 수집 및 모니터링 기준과 임상 평가에 적용할 수 있는 제도적 환경이 필요하다.
