Normal aging, mild cognitive impairment, and Alzheimer’s disease (AD) fall along a continuous spectrum of cognitive aging. Recently, there has been a strong recommendation to recognize preclinical stages of MCI. Many studies that focused on this “between normal and MCI” status used various terminologies indicating earlier stages of MCI, such as subjective memory impairment (SMI), subjective cognitive impairment (SCI), subjective cognitive decline (SCD), or pre-mild cognitive impairment (pre-MCI). There is no established common concept for preclinical stages of MCI yet. Different measurement and inclusion/exclusion criteria have been applied in many studies. In this paper, research criteria guidelines for those terms denoting preclinical stage of MCI were briefly introduced. It is expected that one of those terms will provide potential usefulness for identifying preclinical MCI and clinical trials recruitment.

정상 노화나 경도인지장애, 치매는 모두 인지적 노화의 한 연속선상에 존재한다. 최근 정상 노화와 경도인지장애 사이에 해당하는 단계인 “전임상 경도인지장애(preclinical MCI; preclinical mild cognitive impairment)”에 대한 연구가 활발히 진행되고 있다. 이 단계를 주관적 기억손상(SMI; subjective memory impairment)이나 주관적 인지 손상(SCI; subjective cognitive impairment), 주관적 인지 저하 (SCD; subjective cognitive decline), 또는 전 경도인지장애(pre-MCI; pre-mild cognitive impairment) 등으 로 표현하기도 한다. 학술적으로는 연구마다 도구나 기준을 약간씩 다르게 적용시키고 있어서 전임 상 경도인지장애에 대한 공통된 개념을 아직 확립시키지 못한 상태이다. 이에 본고에서는 현재 진행 되고 있는 관련 연구들이 제안한 진단 가이드라인들을 비교해보고자 한다. 과거에 정상과 치매사이 를 정의하려던 여러 용어들 중에서 MCI가 가장 성공적인 임상진단체계로 자리잡은 것처럼, 본고에 서 소개된 용어들 중 하나가 성공적으로 자리잡아 전임상기의 MCI를 효과적으로 분류해내고 알츠하 이머병의 연구와 치료에 도움이 될 수 있기를 기대한다.
