Objective: The purpose of this study is to analyze the cognitive rehabilitation approach applied to occupational stroke patients as practiced by domestic and overseas occupational therapists, and to propose the effectiveness and practicality of clinical intervention. Methods: We searched for studies from 2008 to 2017 through Pubmed, CINAHL (EBSCOhost), PSYCINFO, KISS, and RISS using the Preferred Reporting Items for Systematic Review and Meta-Analysis guidelines (PRISMA guidelines). The key search terms were "Stroke AND Occupational Therapy AND Cognitive Rehabilitation." The inclusion and exclusion criteria were applied to a total of 17 studies. Results: The results were analyzed through a qualitative analysis of the research, a quantitative meta-analysis, categorization of the cognitive rehabilitation approach, FAME classification for the domestic clinical utility, and a post-intervention measurement tool. The orthodontic approach, adaptive approach, and dynamic interaction approach showed moderate effects, and the functional approach showed a small effect. Conclusion: This study was designed to suggest an approach that can be applied immediately in Korea by examining the cognitive rehabilitation approach applied to stroke patients as practiced by domestic and overseas occupational therapists. In the future, many approaches should be supplemented, and various approaches should be applied to clinical practice in Korea.

목적 : 본 연구의 목적은 국내외 작업치료사에 의해 실시되고 있는 뇌졸중 환자에게 적용된 인지재활 접근 법을 분석하여 효과와 임상 중재의 실용성을 제시하고자 한다. 연구방법 : Preferred Reporting Items for Systematic review and Meta-Analysis guideline(PRISMA guideline)을 이용하여 Pubmed, CINAHL(EBSCOhost), PSYCINFO, KISS, RISS를 통하여 2008년부 터 2017년까지의 논문을 검색하였다. 주요 검색용어는 “Stroke AND Occupational therapy AND Cognitive rehabilitation”, “뇌졸중 그리고 작업치료 그리고 인지재활”을 사용하였다. 포함기준과 배제기 준을 적용하여 총 17개의 연구가 분석 대상으로 사용되었다. 결과 : 결과는 연구의 질적 분석 및 계량적 메타분석, 인지재활 접근법의 분류, 국내 임상 실용성을 위한 FAME 등급 분류, 중재 후 측정도구로 분석하였다. 교정적 접근법, 적응적 접근법과 역동적 상호작용 접 근법은 중간 효과크기로 나타났고, 기능적 접근법은 작은 효과크기로 나타났다. 결론 : 본 연구는 국내외에서 작업치료사에 의해 실시되고 있는 뇌졸중 환자에게 적용된 인지재활 접근법 에 대해 분석함으로써 국내 임상에 즉시 적용 가능한 접근법을 제시하고자 하였다. 앞으로는 많은 접근 법이 보완되고 다양한 접근법이 국내 임상에 활용되어야 할 것이다.
