The Korean Military Health system operated by the Ministry of National Defense provides medical care to 600 thousands active duty Korean Military personnel. Its primary mission is to maintain the health of military personnel, so they can carry out their military missions; and to deliver health care during wartime, often described as the medical readiness mission. Unfortunately, this system is not currently accomplishing either of its two primary goals sufficiently. Patient satisfaction and the quality of medical care are consistently lower than civilian benchmarks, and the military medical workforce is not optimized the patients they would serve in combat. For those reasons, this study aimed at providing a knowledge base for military health system and evaluating problems about healthcare system and healthcare delivery system, recognizing the current military health system needs a reform on an extensive scale and scope. Based on the evaluation, this paper will suggest the direction of reform which can significantly improve the strengthen health care and the quality of medical care for Military personnel. It is clear that the direction of reform is not saving money in the Ministry of National Defense’s budget but delivering military health care in an efficient and cost-effective manner. Finally, it mentions the responsibility of the military healthcare system failure and the equality of the rights and duties in Military personnel.

군 의료체계(MHS)는 국방부 책임 하에 국방의무를 수행하는 60만 장병들에게 의료 서비스를 제공하고 있다. 군 의료의 주요 임무는 장병들의 건강을 관리하여 전투력을 유지하고, 전시에 완벽한 의료서비스 제공을 위한 준비 임무(medical readiness)이다. 불행히도 현재 군 의료는 두 가지 목적 중 어느 하나도 충분히 성취하지 못하고 있어 군 내·외로 부터 심각하게 변화를 요구받고 있다. 병사들의 의료 만족도와 의료의 질은 민간의료 보다 매우 낮은 수준이고, 군 의료 인력은 평시 작전 상황과 전시 작전 상황에 발생할 수 있는 부상병에게 최적화되어 있지도 않다. 이러한 이유로 본 연구는 우리군의 의료보장체계와 의료전달체계의 개혁이 필요하다는 인식을 바탕으로 연구를 진행하였다. 우선적으로 군 의료보장체계와 의료전달체계의 지식과 정보를 제공하고, 체계의 문제점을 탐색하기 위해 군 의료보장체계와 의료전달체계의 각각의 분석요소를 설정하여 문제점을 평가하였다. 본 논문은 평가결과에 근거하여 의료보장성 강화와 의료전달체계의 개선 방향을 제시 하였다. 이러한 개선 방향은 국방예산의 절감을 위한 효율성 측면이 아닌, 군 의료 목적에 근거한 비용-효과를 극대화 할 수 있는 현실적 개선 방향이다. 마지막으로 군 의료의 실패 책임과 장병들의 권리와 의무에 대한 형평성을 언급하였다.
