Objective: This study is a follow-up study to the standardization of a clinical terminology system for Occupational Therapy (OT), and aims at developing the standard terminology for OT documentations focusing on Korean-type medical institutions, by establishing terminologies regarding the classification of OT documentations and detailed treatment acts, determining its applicability through consultations with the experts, and verifying its validity. Methods: For medical institutions where occupational therapists work, based on the current results of occupational therapeutic actions in a clinical setting by case, standardization was developed by reflecting on the International Classification of Functioning, Disability and Health (ICF) classification system and the Occupational Therapy Practice Framework (OTPF). This study was conducted in five stages, a seminar with the consultants, consultations with experts, reviewing the focus group, and a validity investigation. For verifying the validity, the content validity index was calculated. Results: Nine broad classifications of OT documentations focusing on Korean-type medical institutions consist of sensitive therapy, perceptive therapy, cognitive therapy, neuromuscular skeletal system therapy, dysphagia rehabilitation, activities and participation-mobility, activities and participation-ADL, activities and participation-IADL, and activities and participation-other occupational therapies. As a result of the content validity verification, the entire content validity index on clinical OT terminologies was higher than 8.9. Conclusion: It is expected that standardization will be available for the documentation of OT focusing on activities and participation using standard terminologies for OT documentations focusing on Korean-type medical institutions and for applying it as evidence to develop and extend OT insurance costs using statistical data on frequency of usage of standard terminologies about OT documentations.

목적 : 본 연구는 작업치료 임상용어체계 표준화를 위한 후속 연구로 작업치료 기록의 분류 및 세부치료 행위에 대해 용어를 확립한 후 전문가 자문을 통해 적용 가능성을 파악하고 타당성을 검증하여 한국형 의료기관 중심 작업치료기록 표준용어를 개발하고자 실시하였다. 연구방법 : 작업치료사들이 근무하는 의료 기관을 대상으로 지역별, 병원종별, 사례별 임상에서의 작업치료행위 기록현황 결과를 기반으로 국제 기능, 장애 및 건강에 대한 분류(International Classification of Functioning, Disability and Health; ICF)와 작업치료 실행체계(Occupational Therapy Practice Framework; OTPF)를 근거로 5단계의 전문가 검토를 실시하였고 타당도 검증은 내용타당도 지수를 산출하였다. 결과 : 한국형 의료기관 중심 작업치료기록 9개의 대분류는 감각치료, 지각치료, 인지치료, 신경근·골격계치료, 연하치료, 활동 및 참여–이동 및 움직임, 활동 및 참여–기본적 일상생활활동, 활동 및 참여–수단적 일상생활활동, 활동 및 참여–그 밖의 작업치료로 구성되었다. 내용타당도 검증결과 임상 작업치료 용어에 대한 전체 내용타당도 지수는 .89로 높게 나타났다. 결론 : 향후 한국형 의료기관 중심 작업치료기록 표준용어를 활용하여 활동 및 참여 중심의 작업치료 문서화가 가능해지고, 작업치료기록 표준용어의 사용빈도 통계 자료를 활용하여 작업치료 보험수가 개발 및 확대를 위한 근거 자료로 활용될 것을 기대하는 바이다.
