The purpose of this study was to examine the effect of therapeutic instrument music playing on hand grasp strength and dexterity in stroke patients. The subjects of this study were three outpatients(two men and a woman) who were being treated in the department of rehabilitation medicine of **University Hospital due to hemiplegia caused by stroke. Aiming at them, music treatment was conducted for 4 weeks from September 22 to October 22, 2005, 20 sessions in total(five times a week, 30 minutes per session). The researcher applied multiple baseline design between the subjects for this study and dynamometer, box, and block test were used to measure hand grasp strength and dexterity. After the each session was completed, the affected and unaffected results were recorded with two other observers. As a result of this study, an improvement showed in hand grasp strength and dexterity of affected and unaffected of the three subjects as therapeutic instrument music playing intervention was developed. Specially, it can be noted that three subjects showed high scores when preferred songs and instruments were used and improvisation method showed effects. Also, disease duration and number of sessions showed a close relationship and the shorter disease duration was, the more effects showed in the early stage of the sessions.

이 연구의 목적은 치료적 악기연주가 뇌졸중 환자의 손 장악력과 기민성에 미치는 효과를 알아보기 위한 것이었다. 연구의 대상은 경기도에 소재한 한 대학부속병원 재활의학과에서 뇌졸중으로 인한 편마비로 진단받고 외래로 치료를 받고 있는 3명의 환자(남자2명, 여자1명)였으며, 2005년 9월 22일부터 10월 22일까지 4주간에 걸쳐 총 20회(주 5회, 30분간) 음악치료를 실시하였다. 또한, 연구자는 연구를 위하여 대상자간 중다기초선설계를 적용하였으며(multiple baseline design), 손 장악력과 기민성을 측정하기 위하여 악력계와 상자와 나무토막 검사를 사용하였고, 매회 세션이 끝난 후 건측과 환측의 결과를 다른 두 명의 관찰자와 함께 기록하였다. 연구결과, 치료적 악기연주를 사용한 중재가 진행될수록 세 명의 대상자의 건측과 환측의 손 장악력과 기민성에서 향상을 보인 것으로 나타났다. 특히 3명의 대상자가 선호하는 노래나 악기를 사용하였을 때 높은 점수를 나타내었고, 즉흥연주방법이 효과적인 것으로 나타났다. 또한 대상자의 유병기간과 세션회기와 밀접한 관계가 있었는데, 유병기간이 짧을수록 세션회기 전반부에 효과가 나타났다.
