The purpose of this study was to develop an integrated service model of health and welfare in an attempt to seek ways of revitalizing in-home community health plans. An integrated service model of health and welfare should be designed to ensure a horizontal relationship among all the parties concerned and to provide service through professional teamwork. To guarantee the horizontal cooperative relationship among the parties, they should set up a working-level task force for the purposes of information sharing, service planning and service coordinating. And in order to step up the revitalization of in-home health plans by linking health and welfare service, a health and welfare council should be made up of local public health centers, the social welfare departments of community centers and welfare social agencies to offer inclusive service in a manner to be suitable for the given local circumstances, and the development of integrated health- welfare community service model is urgently required.

본 연구에서는 보건 및 복지 연계서비스 모형을 개발하고, 이를 통해 지역사회의 방문보건사업의 활성화 방안을 제시하고자 하였다. 첫째, 보건 및 복지 연계서비스 모형은 전문성을 가진 팀워크 형태의 서비스 제공과 수평적인 기관 체계로 이루어져야 한다. 둘째, 기관간의 수평적인 연계는 상호기관간의 대상자 정보교환 및 의뢰, 서비스 사정, 계획 및 조정을 위한 실무자회의가 구성되어야 한다. 셋째, 보건·복지 서비스 연계를 통한 방문보건사업이 활성화되기 위해서는 각 지역사회의 보건소, 주민센터 사회복지과, 그리고 사회복지기관이 참여하는 보건복지연계 협의체를 구성하여 각 지역의 실정에 맞는 연계서비스가 이루어져야 한다. 넷째, 지역별 보건복지 연계 서비스 모델구축이 시급하다. 따라서 지역사회에 방문보건사업의 활성화를 위한 심층적 연구가 지속되어져야 할 것이다.
