연구목적: 본 연구는 트레드밀 점증운동부하검사에서 산출된 총 운동부하량(Total Workload)의 심폐체력 평가 지표로서의 타당성을 검증하고 디지털 헬스케어 분야에서의 활용 가능성을 탐색하고자 수행되었다. 연구방법: 건강한 성인 41명을 대상으로 트레드밀 점증운동부하검사를 실시하였다. Total Workload와 직접 측정된 VO₂max 간의 관계를 상관분석, 회귀분석 및 사분위군 분석을 통해 검증하였다. 연구결과: Total Workload는 VO₂max와 유의한 정적 상관관계를 나타냈다(r=.764, p<.001). 회귀분석 결과 Total Workload는 VO₂max 변량의 58.3%를 설명하였으며(R²=.583, p<.001), VO₂max는 Total Workload 사분위군(Q1~Q4)에 따라 단계적으로 증가하였다. 집단 간 차이는 통계적으로 유의하였으며(F=33.00, p<.001, η²=.728), Total Workload가 심폐체력 수준과 유의한 관련성을 보이는 예비적 근거를 확인하였다. 결론: Total Workload는 심폐체력 평가를 위한 실용적 지표로 활용 가능하며, 향후 디지털 헬스케어 플랫폼 및 운동관리 시스템 개발에적용될 수 있을 것으로 기대된다.
Purpose: This study aimed to verify the validity of treadmill-based Total Workload as an indicator of cardiorespiratory fitness and to explore its potential applications in digital healthcare. Methods: Forty-one healthy adults completed a graded treadmill exercise test. The relationships between Total Workload and directly measured VO₂max were examined using Pearson correlation analysis, simple linear regression, and quartile-based group comparisons. Results: Total Workload was significantly correlated with VO₂max (r=.764, p<.001). Regression analysis showed that Total Workload explained 58.3% of the variance in VO₂max (R²=.583, p<.001). VO₂max increased progressively across Total Workload quartiles (Q1–Q4), and significant differences were observed among all groups (F=33.00, p<.001, η²=.728). These findings provide preliminary evidence that Total Workload is significantly associated with cardiorespiratory fitness and may have potential utility for differentiating fitness levels. Conclusion: Total Workload may serve as a practical indicator for assessing cardiorespiratory fitness and could be applied to future digital healthcare platforms and exercise management systems.

본 연구는 트레드밀 기반 점증운동부하검사에서 산출된 총 운동부하량 (total workload)과 최대산소섭취량 (maximal oxygen uptake, VO 2 max) 간의 관련성을 분석하고, 성별에 따른 심폐지구력 반응의 차이를 해부생물인류학적 관점에서 규명하고자 수행되었다. 심폐지구력은 심혈관계와 호흡계의 산소 공급 능력 및 골격근의 산소 이용 능력을 반영하는 대표적인 생리학적 지표이며, 최대산소섭취량은 이를 평가하는 가장 신뢰도 높은 지표로 알려져 있다. 연구 대상은 건강한 성인 남성 8명과 여성 8명으로 구성되었으며, 모든 대상자는 Modified Bruce protocol을 적용한 트레드밀 기반 점증운동부하검사를 수행하였다. 검사 과정에서 속도, 경사도, 운동시간 및 심박수를 측정하였으며, 호흡가스 분석을 통해 최대산소섭취량을 직접 측정하였다. 또한 체중, 운동속도, 경사도 및 운동시간을 기반으로 총 운동부하량을 산출하였 다. 자료 분석은 독립표본 t-검정, Pearson 상관분석, 공분산분석(ANCOVA), Cohen의 d를 이용한 효과크기 분석을 실시 하였으며, 추가적으로 Spearman 상관분석과 민감도 분석을 수행하여 결과의 안정성을 검증하였다. 연구 결과, 남성은 여성보다 최대산소섭취량(46.69±4.89 vs. 35.89±2.28 mL/kg/min)과 METs에서 유의하게 높은 값을 나타냈다(p<.001). 운동지속시간과 총 운동부하량 또한 남성에서 높은 경향을 보였으나 통계적으로 유의하지는 않았다(p = .058, p = .077). 효과크기 분석에서는 VO 2 max (d = 2.80)와 METs (d = 3.20)가 매우 큰 효과크기를 나타냈으며, 총 운동부하량(d = 0.92) 역시 큰 효과크기를 보였다. 또한 총 운동부하량과 최대산소섭취량 간에는 높은 정적 상관관계(r = .86, p<.001)가 확인 되었으며, Spearman 상관분석 및 이상치 제거 후 분석에서도 유사한 결과가 유지되었다. 이러한 결과는 성별에 따른 형태 학적 특성과 기능적 운동 반응 간의 연관성을 보여주는 자료로서 의미를 가진다. 특히 골격근량, 심장 크기, 혈액의 산소 운반능력과 같은 구조적 특성이 심폐지구력 차이에 영향을 미칠 가능성을 시사하며, 인간 집단 내 존재하는 성적 이형성 (sexual dimorphism)의 기능적 표현 양상을 이해하는 데 기초자료를 제공한다. 또한 총 운동부하량은 최대산소섭취량과 밀접한 관련성을 나타내어 심폐지구력 평가를 위한 잠재적 지표로 활용될 가능성을 보여주었다. 따라서 본 연구는 성별에 따른 형태학적 다양성과 심폐지구력 반응 간의 관계를 이해하기 위한 기초자료를 제공하며, 향후 총 운동부하량 기반 평가 방법의 활용 가능성을 탐색하기 위한 후속 연구의 근거를 제시한다.
This study aimed to investigate sex differences in treadmill-based total workload, maximal oxygen uptake (VO 2 max), and cardiorespiratory fitness responses, and to evaluate the association between total workload and VO 2 max from an anatomical and physiological perspective. Sixteen healthy adults (8 males and 8 females) participated in graded exercise testing using the Modified Bruce protocol. Exercise duration, speed, incline, heart rate, and directly measured VO 2 max were obtained during treadmill testing. Total workload was calculated by integrating body weight, exercise speed, incline, and exercise duration. Independent t-tests, Pearson correlation analysis, ANCOVA, and Cohen’s d effect size analysis were performed. Additional Spearman correlation and sensitivity analyses were conducted to evaluate the robustness of the findings. Males demonstrated significantly higher VO 2 max and METs than females (p<.001). Mean VO 2 max was 46.69±4.89 mL/kg/min in males and 35.89±2.28 mL/kg/min in females. Exercise duration and total workload were also higher in males; however, these differences did not reach statistical significance (p = .058 and p = .077, respectively). Effect size analysis revealed very large effects for VO 2 max (d = 2.80) and METs (d = 3.20), and a large effect for total workload (d = 0.92). A strong positive correlation was observed between total workload and VO 2 max (r = .86, p<.001), which remained consistent across Spearman correlation and sensitivity analyses. The present findings suggest that sexrelated differences in cardiorespiratory fitness may be associated with underlying morphological and physiological characteristics, including skeletal muscle mass, cardiac size, and oxygen transport capacity. In addition, total workload demonstrated a strong association with VO 2 max, indicating its potential usefulness as a workloadbased indicator of exercise performance. Although this exploratory pilot study was limited by a small sample size, the findings provide preliminary evidence supporting the relationship between morphological characteristics and functional exercise responses and may serve as a foundation for future investigations of workload-based cardiorespiratory fitness assessment.
