장애인의 이차장애 발생경험 및 대응에 관한 연구 KCI 등재

장애인구의 고령화나 일차장애의 특성 및 사회환경적인 장애억압과 스트레스로 인해 장애인의 이차적 건강상태는 더욱 취약하다. 이에 본 연구에서는 장애인의 이차장애 경험이 어떠하며, 이차장애 발생시 어떻게 대응하였는지 살펴보고자 하였다. 총 13인의 이차장애를 가지고 있는 중증장애인을 초점집단면접하였으며, 현상학적 연구방법을 적용하여 분석하였다. 분석결과, 장애인들은 노화와 일차장애의 경험의 변주, 일차장애 관리문제와 생활사고, 장애와 건강에 대한 노동시장의 몰이해, 이차장애 위험에 대한 이해 및 예방부족 속에서 이차장애 발생을 경험하고 있었다. 이에 대한 대응으로, 장애인은 몸과 일, 몸과 학업사이 에서 양자택일을 하거나, 개인차원에서 이차장애 악화를 예방하고자 애쓰거나, 병원 출입을 계속해나가거나 건강을 최우선으로 하는 삶을 선택하는 등 삶의 조율을 끊임없이 해나가고 있었다. 이차장애를 경험하면서 장애인들은 생의 위협을 경험하고, 삶을 끝까지 견디고 싸워내야 하는 것으로 인식하기도 하였고, 장애인과 비장애인의 다름을 인정하고, 이차장애를 겪어내야 하는 삶에 초연해지기도 하였으며, 사회제도적 지원을 요청하기도 하였다. 본 연구 결과를 바탕으로 의료적 차원의 방안, 노동시장에 서의 제도적 지원 방안, 사회복지 실천상의 지원방안에 대하여 논의하였다.
The secondary health status of the disabled is more vulnerable due to the aging of the disabled, the nature of primary disabilities, and social and environmental disability suppression and stress. Therefore, this study aimed to examine the experiences of secondary disabilities of the disabled and how they responded when secondary disorders occurred. A total of 13 people with severe disabilities with secondary disabilities were interviewed in a focus group and analyzed by applying phenomenological research methods. As a result of the analysis, people with disabilities were experiencing secondary disorders due to variations in the experiences of aging and primary disorders, management problems and life accidents in the primary disorder, misunderstanding of the labor market for disorders and health, misunderstanding of risks of secondary disorders and lack of prevention. In response, people with disabilities live alternatively between body and work, body and academic, try to prevent worsening of secondary disabilities at the individual level, continue to enter the hospital, or choose a life that puts health first. While experiencing secondary disabilities, people with disabilities experienced the threat of life, recognized life as having to endure and fought to the end, acknowledged the difference between the disabled and non-disabled people, and standed aloof from their ongoing secondary disability. It also requested institutional support. Based on the results of this study, medical-level measures, institutional support measures in the labor market, and social welfare practices were discussed.
Problems of Clinical Nurse Performance Appraisal System: A Qualitative Study

Purpose: The purpose of this study is to explore problems of clinical nurse performance appraisal system. Methods: This study employed a descriptive qualitative approach. The participants were purposively selected from clinical nurses working across all of the hospital units in a large metropolitan teaching hospital in Tehran, Iran in 2012. Data were collected using five focus group interviews, which were audio taped. The number of participants in each group ranged from 7 to 10. The semi-structured interviews were guided by a set of nondirective questions, and continued until the data reached saturation. Data were analyzed using framework analysis. Results: Four major themes regarding the problems of clinical nurse performance appraisal system emerged from the analysis of textual data. These themes were contextual problems, problems related to performance appraisal structure, problems related to performance appraisal process and those related to performance appraisal results. Conclusion: The findings of this study reveal that the nurse performance appraisal system confronts with various problems. Some of these problems are related to organizational context while the others concerned structure, process and results of the performance appraisal system. In order to achieve high quality of patient care as the final goal of performance appraisal, changing and revision of this system is necessary.

Food and beverages are integral elements of cultural heritage, reflecting societies’ histories, identities, and values. However, globalization and changing lifestyles have diminished the diversity of traditional cuisines, posing a threat to their preservation. Showcasing these culinary traditions, particularly in tourism destinations, can contribute to raising awareness and safeguarding cultural heritage. This study investigates the traditional wedding foods and table customs of Fethiye, which are at risk of being forgotten, and explores their potential as tourism assets. A qualitative research design was applied, combining semi-structured interviews with local experts and direct observations at wedding ceremonies. The findings reveal that Fethiye’s gastronomic traditions hold significant potential for sustainable tourism development. The study concludes by offering practical recommendations and highlighting the importance of community participation in preserving culinary heritage.
Culinary practices of the Bidayuhs in Sarawak, Malaysia: a qualitative study

The Bidayuh cuisines and culinary practices epitomize an inherent part of the cultural profile of the Sarawak community. This indigenous group (Bidayuh) is the second-largest group contributing to the Sarawak’s distinctive identity after the Iban. The distribution of the Bidayuh community is predominantly across the districts of Lundu, Bau, Kuching and Serian in Sarawak. There are concerns that the Bidayuh traditional culinary culture will gradually beset by modernization. Many prefer convenience over the hassle of cooking using the traditional manner, hence diluting the traditional culinary knowledge among the younger generations. Therefore, ethnomethodology, a qualitative research approach, was utilized to explore the traditional culinary practices and the connotation of food among the Bidayuh indigenous group in Sarawak, Malaysia. The data were collected from nine informants [Kampong Gumbang in Bau District (n?=?5) and Kampung Mongkos in Serian District (n?=?4)] who had voluntarily participated in this study. Through the ethnomethodological lens, this study documented eight traditional Bidayuh recipes and the collective development of the gastronomic culture of this community that are still remembered and practiced today.
Nurses' Perceptions of Factors Influencing Elder Self-neglect: A Qualitative Study

Purpose: Elder self-neglect is a global public health issue and should be taken seriously at large. Nurses, usually working directly with elderly patients, have a better understanding of what factors may cause elder self-neglect. In this qualitative study, we explored the influencing factors of elder self-neglect from the perception of nurses in the context of Chinese culture. Methods: Face-to-face, in-depth interviews were conducted from November 2018 to December 2018. Purposive sampling was used. Twenty one participants recruited from eight geriatric wards of a general hospital located in Wuhan were interviewed. A content analysis of qualitative nature was performed to analyze the data. Results: Our conceptual model illustrated the findings based on the three themes of the conflict between personal recognition and social judgment, the choice between current needs and individual beliefs, as well as the compromise between insufficient abilities and limited resources. Conclusion: Nurses together with family members and social workers can help older adults improve their awareness of self-neglect to bridge the gap with social judgment, learn to focus on their own needs, as well as seek as much support as possible. Nurses should also respect the autonomy and selfdetermination of elder self-neglecters because self-neglect is related to older adults' values. Furthermore, larger studies are needed to quantitatively test and refine the model.
Food taboos and suggestions among Madurese pregnant women: a qualitative study

Background: Cultural factors influence food consumption, particularly among pregnant women. Beliefs in certain food-associated myths and taboos still exist in Madura Island. Therefore, this study aims to explore the food taboos and food suggestions among Madurese pregnant women. Methods: This qualitative study was carried out in Sumenep Regency, Madura Island, Indonesia. Data were collected from pregnant women, traditional birth attendants, and community leaders through indepth interviews and focus group discussion. Results: Foods that were mostly considered taboos for pregnant women were squid, shrimp, pineapple, kedondong, cabbage, cold water, and instant noodles. Food suggestions for pregnant women were rice, corn rice, skipjack tuna, tilapia fish, milkfish, egg, moringa leaves, apples, and coconut water. Conclusion: Many food taboos and food suggestions were applied for the pregnant women in Madura. Indigenous knowledge about the food suggestions can be good information in supporting nutrition education for the Madurese pregnant women.
Sickness Experiences of Korean Registered Nurses at Work: A Qualitative Study on Presenteeism

Purpose: Presenteeism is a relatively new concept in nursing describing the condition within which registered nurses (RNs) come to work while they are sick. The purpose of this study was to explore and describe presenteeism experiences among RNs in South Korea. Methods: In this constructivist grounded theory study, a focus group interview (FGI) technique was utilized for data collection. A total of 20 RNs at one hospital in Chuncheon city joined in three different FGIs. Semistructured questions were asked in reference to their sickness experience in the workplace. Data analysis was conducted according to the constructivist grounded theory methodology. Results: All participants had experiences of presenteeism. The overriding theme was “having no caring for nurses leads to losing one's nursing mind”. The participants reported that due to either their personal preference or peer pressure they showed up to work, but they felt sad and their pride was hurt by the fact that there was no caring for them from other nurses. This emotional exhaustion often led to the loss of compassion and the resignation of nursing staff. Conclusions: Care for nurses in the workplace is necessary for RNs to make their presenteeism experience positive and even effective.
Perspectives on registered nurse retention in nursing homes in Korea: A qualitative study

Purpose: This study aimed to understand the factors influencing registered nurses’ (RNs) intentions to remain employed in their current nursing homes (NHs). Methods: Data were collected through individual in-depth interviews with 10 RNs in five Korean NHs from May 1 to May 31, 2024, and analyzed using thematic analysis. Results: RNs in NHs reported that their intention to stay was influenced by “positive interactions with residents,” “cooperative relationships among staff,” and “stability from long-term employment.” Conversely, their intention to leave was influenced by “work burden from insufficient RNs,” “role conflict,” and “low salaries.” Conclusion: This study indicates that significant reforms in staffing regulations and compensation systems are required to retain RNs. Improving working conditions is crucial for healthcare systems to provide effective long-term care to older adults. These changes are essential to maintain the quality of long-term care services.
Exploring the Lives of Korean College Students Who Attempted Suicide: A Qualitative Study

Purpose: This study was conducted to obtain in-depth understanding of the life experiences of college students who attempted suicide. Methods: This study employed qualitative methods involving in-depth interviews, where researchers and participants met one-on-one to capture personal experiences and perspectives. Data were analyzed using Colaizzi's phenomenological approach, providing a systematic interpretation aligned with the lived experiences and challenges shared by the participants. Results: The following six themes reflected participants' experiences: “A family that has left behind deficiencies and wounds that are not easy to overcome”, “Lingering struggles even as college students”, “Seeking self-worth through relationships”, “Achievement at all costs: The search for self-worth in a life obsessed with success”, “The irony of those aspiring to live: suicide” and “The fight still continues”Conclusion: The results underscore the necessity of broadening our interpretive framework to better understand and address the concealed complexities of mental health in this demographic. Mental health professionals are urged to consider these nuanced dynamics when developing targeted interventions to support students through their unique challenges
Experiences of Cancer Survivors Returning to Work in Korean Society: A Qualitative Study

Purpose: This study aimed to comprehensively understand and investigate the various challenges that Korean cancer survivors experience when returning to work (RTW). Methods: This study employed qualitative content analysis based on in-depth interviews to comprehensively understand and investigate the various issues faced by Korean cancer survivors when RTW. Results: The cancer survivors’ RTW experiences were categorized into five themes: "Difficulties in work life due to social awareness of cancer," "Difficult and complex administrative procedures for RTW," "Facilitators for RTW," "Barriers to RTW," and "Demands for a successful RTW," with a total of 11 sub-themes and 35 codes. Conclusion: Cancer survivors should strive to accurately recognize their individual physical changes to ensure a successful return to work. Moreover, accommodating cancer survivors in the workplace by providing flexible work schedules or offering sick leave, while considering their health status, can help promote successful RTW outcomes.
The Experience of Social Alienation in Elderly Lung Cancer Patients: A Qualitative Study

Purpose: The aim of this study was to understand the experience of social alienation in elderly lung cancer patients, to explore its causes, and to propose targeted intervention strategies. Methods: From July to August 2023, 16 elderly lung cancer patients attending the respiratory department of a tertiary hospital in Changsha City, Hunan Province, were selected for semi-structured interviews using a purposive sampling method. The Colaizzi 7-step method was used to analyze the data. Results: A total of four themes were distilled: the experience of social alienation in elderly lung cancer patients (pessimistic feelings, inferiority complex, and heavy psychological burden), subjective causes (disease-included shame, avoidant social behavior, and stigmatized labels), objective causes (isolated social states, and reduced amount of socialization), and rehabilitation support. Conclusion: The causes of social alienation in elderly lung cancer patients include multiple aspects of personal, family, and social support, and symptom management and psychological guidance should be strengthened for this population to construct a hospital-society-family triple-linkage care program to help patients recover.
Lived Experiences of Pregnant Women With Cancer in South Korea: A Qualitative Study

Purpose: The incidence of cancer during pregnancy is increasing, presenting several challenges to the treatment of cancer in pregnant women. However, research focusing on the lived experiences of pregnant women with cancer in South Korea is limited. This study aimed to explore and describe the day-to-day lived experiences of women diagnosed with or treated for cancer during pregnancy and their husbands. Methods: The study employed a qualitative descriptive design and utilized purposive sampling to recruit participants. The participants comprised six women living in Korea diagnosed with cancer during pregnancy and one husband of a female participant. In-depth semi-structured interviews were conducted, audiotaped, and transcribed. Five of the participants agreed to a second interview, resulting in a total of 12 individual interviews. A thematic analysis was then performed. The participants' ages ranged from 31 to 40 years, and their diagnoses during pregnancy were either breast or thyroid cancer. Results: Four main themes were identified: (1) Participants faced various heart-breaking difficulties maintaining their pregnancies throughout cancer treatment; (2) Pregnant women with cancer experienced complex but responsible feelings toward their children; (3) Patients with cancer also fulfilled their roles as parents even with their own diseases; and (4) Family support had a significant impact on the pregnant women to overcome the path. Conclusions: These findings provide a comprehensive understanding of the lived experiences of being diagnosed with cancer during pregnancy. A recommended strategy is to develop a nursing education program for pregnant women with cancer to provide necessary information and support, and to help them cope positively with their situation.
Exploring Nurses' Perceptions of Nursing Home Care in South Korea: A Qualitative Study

Purpose The purpose of this study was to describe nurses' perceptions of nursing services provided to nursing home residents, facilitators and challenges of providing care for these residents, and the need to improve the quality of care in the nursing home setting. Methods Data were collected through semi-structured, audio-recorded interviews with 19 nurses from six nursing homes in South Korea from February to June 2015. A thematic analysis was used to analyze the data. Results Five themes emerged: resident- and family-centered care as quality care, the importance of registered nurses in providing quality care, the importance of collaborating with care workers and resident's families, needs for nursing-home facility-level support for quality care, and needs for national policy for quality care. Conclusion To improve the quality of care in nursing homes requires organizational and policy endeavors to increase the number of nurses per shift, provide systematic education for care workers, and develop strategies for collaboration with care workers and families. Nursing-home facility-level support and national policy are also required to meet the complex healthcare needs of nursing home residents with multiple chronic conditions.
Nurses' Perceptions Regarding Disclosure of Patient Safety Incidents in Korea: A Qualitative Study

Purpose: The purpose of this study was to determine nurses' perceptions of the disclosure of patient safety incidents (DPSI), which is known to be effective in reducing medical litigation and improving the credibility of medical professionals. Methods: Three focus group discussions were conducted with 20 nurses using semistructured guidelines. Transcribed content including a record of the progress of the focus group discussions and researchers' notes were analyzed using directed content analysis. Results: Most participants thought that DPSI is necessary because of its effectiveness and for ethical justification. However, participants held varied opinions regarding the primary responsibility of DPSI. Participants agreed on the necessity of explaining the incident and expressing sympathy, apologizing, and promising appropriate compensation that are chief components of DPSI. However, they were concerned that it implies a definitive medical error. A closed organizational culture, fear of deteriorating relationships with patients, and concerns about additional work burdens were suggested as barriers to DPSI. However, the establishment of DPSI guidelines and improving the hospital organization culture were raised as facilitators of DPSI. Conclusion: Most nurse participants acknowledged the need for DPSI. To promote DPSI, it is necessary to develop guidelines for DPSI and provide the appropriate training. Improving the hospital organization culture is also critical to facilitate DPSI.

Purpose: In order to bridge the gap between rapid technological changes and the healthcare environment,and to alleviate the burden of nursing tasks, the need for change management must be recognized. Understanding nurses' perceptions of generative artificial intelligence (AI) is essential for thesuccessful implementation of nursing change management. However, existing studies are mostlyexploratory in nature, focusing on the utility and applications of generative AI technology. There is a lackof research on its implications for the nursing environment and nursing management. The purpose ofthis study is to explore in depth the perceptions of nurses at cancer specialty hospitals regarding theapplication of generative AI, as well as the environmental and contextual factors involved. Methods: This study conducted in-depth interviews with nurses at National Cancer Center, the first cancerspecialty hospital in South Korea, from July to August 2024. The semistructured questionnaire was developedbased on clinical nursing practice guidelines, and participants were selected considering the various characteristicsand situations of nursing tasks. Results: First, nurses at the cancer specialty hospital recognized change management factors in theareas of ‘basic nursing,’ ‘therapeutic intervention,’ ‘nursing during examinations,’ ‘infection control,’ and‘nursing administration.’ Second, limitations of change management such as ‘decline in nursing proactivity,’‘difficulties in building rapport with patients,’ ‘difficulties in providing personalized nursingcare,’ ‘patient information breach’ and ‘unclear accountability’ were identified. Third, preparationstrategies for change management such as ‘nurse competency improvement,’ ‘legal protection,’‘establishing standardized processes,’ and ‘awareness improvement’ were identified. Conclusion: Based on the research findings, academic and practical implications for the application ofgenerative AI in nursing and strategies to enhance nurses’ readiness for its implementation are presented.

Purpose: This study aimed to explore the roles, facilitators, barriers, and future directions of rapid response teams (RRTs) in Korean hospitals from the perspectives of both RRT and ward nurses. Methods: Focus group interviews were conducted with 10 RRT nurses and 10 ward nurses across three hospitals that employed RRTs in Korea from August 2021 to February 2022. The interviews were recorded, transcribed, and analyzed using qualitative content analysis to identify themes relevant to RRT operations. Results: The analysis yielded 10 subtopics and 4 main themes: the exploration of RRT's essential roles, the facilitators and barriers impacting RRT operations, and the construction of a blueprint for future systems. Notable barriers included unclear job assignments without legal safeguards, conflict arising from hierarchical structures, and insufficient organizational support. The following facilitators were identified: transformed perceptions through collaborative efforts, organizational recognition and support, and self-reinforcement by demonstrating expertise. Conclusion: This study highlights the challenges and opportunities associated with implementing RRTs in Korean hospitals, including the need for clear role definitions, effective interprofessional collaboration, and organizational support. Based on these findings, future efforts should focus on establishing legal frameworks that define the scope of practice for RRT nurses.
Experiences of Growing Up with Mothers with Schizophrenia in South Korea: A Qualitative Study

Purpose: The premenstrual symptoms (PMSs) are a common health issue affecting women of reproductive age and are associated with various physiological, psychological, and situational factors. Newly graduated nurses, when suddenly exposed to excessive workloads and shift work, may experience disturbances such as sleep disorders, fatigue, depression, and stress. These factors are related to PMS, and their occurrence varies. Therefore, this study aimed to understand the changes in PMS and its risk factors among newly graduated nurses across shift work periods, and investigate the longitudinal relationships between PMS and its risk factors based on the theory of unpleasant symptoms. Methods: This study constituted a secondary data analysis and employed a prospective longitudinal design. Newly graduated nurses were assessed three times: before starting shift work, six months after initiating shift work, and 18 months thereafter. Physiological (age, body mass index, alcohol consumption, dietary behavior), psychological (sleep quality, fatigue, depression, stress, life satisfaction), and situational (social support) factors were included as PMS-related factors. Results: PMS worsened six months after the initiation of shift work compared with those before, and this aggravation persisted even after 18 months of shift work (p < .001). Age (p = .043), sleep quality (p = .004), and fatigue (p < .001) were associated with PMS. An interaction between time and depression that affected PMS was observed (p = .021). Conclusion: Newly graduated nurses need to pay attention to the exacerbation of PMS after initiating shift work. Interventions to improve sleep quality and reduce fatigue are crucial for novice nurses, irrespective of shiftwork duration. Additionally, it is important to assess depressive symptoms before starting shift work and implement early interventions. Furthermore, the need for interventions addressing depression escalates with the increase in shiftwork duration.
Isolation and Recovery Experiences of COVID-19 Patients in South Korea: A Qualitative Study

Purpose: COVID-19 is both a national crisis and a cause of individual trauma. This trauma is experienced during the period of acute illness and for an unpredictable duration after recovery. This study explored experiences of COVID-19 patients in hospital isolation, providing a detailed account of their experiences to furnish basic data necessary for establishing strategies to address the psychosocial problems that may arise from the hospital isolation process and recovery period. Methods: We conducted comprehensive interviews and qualitative content analysis. We held in-depth interviews with five COVID-19 patients from May 16 to August 22, 2021, and also analyzed data from interviews with five patients conducted as part of another study on COVID-19 patients' perceptions. Thus, 10 participants' interviews were analyzed in this study. Results: From the qualitative data, 34 codes, 9 subcategories, and 4 categories were identified. The categories included "Prioritization of epidemic prevention over treatment," "Victims who have been labeled as perpetrators," "Gratitude felt in difficult moments," and "Demand for respect from the system and society." After analyzing these categories, we derived the overarching theme of "It's okay, but it's not okay." Conclusion: During isolation treatment, it is crucial to uphold human rights as much as possible by prioritizing patient care, providing a comfortable environment, and offering clear guidance on their progress. Communication between patients and caregivers should be facilitated to provide psychological support. Furthermore, national policies such as economic compensation and strategies for post-traumatic growth should be implemented to address grievances and facilitate recovery.

Purpose: This study aimed to explore nursing students' experiences of neonatal nursing simulations using peer tutoring. Methods: In this qualitative content analysis study, data were collected using a narrative survey and focus group interviews with 27 third-year nursing students and six fourth-year nursing students from April to May 2022. Content analysis of the collected data was conducted. Results: Four categories?"stabilizing emotionally through each other", "advancing together", "difficulties in relationships", and "hoping to continue"?and nine sub-categories were extracted. The sub-categories "reduced burden" and "gaining confidence" were grouped into the first category, "stabilizing emotionally through each other". The sub-categories "being motivated to learn," "increased learning ability", and "preparation as a process" were grouped under "advancing together", and "attitudes affecting study environment" and "depending on help" were grouped into the third category of "difficulties in relationships". The fourth category of "hoping to continue" had "wanting to supplement for development" and "wanting to participate in different roles" as sub-categories. Conclusion: Based on the results of this study, we expect pediatric nursing practicum education to improve through the active use of neonatal nursing simulation education incorporating peer tutoring.
The Work Experience of Newly Recruited Male Nurses during COVID-19: A Qualitative Study

Purpose: This study was to investigate the work experience of newly recruited male nurses during the COVID-19 pandemic. Methods: With a phenomenological approach, this qualitative study was adopted semistructured interviews by phone or video calls. A total of 9 male nurses newly recruited for the COVID-19 wards in Chinese hospitals were interviewed for this study. And Colaizzi's method was applied for evaluation in the data analysis. Results: Based on our findings, three themes were extracted. First, the newly recruited male nurses showed negative emotions at the beginning of COVID-19 epidemic, which was caused by changes in working conditions and content, but also prompted the nurses to change the way of coping with the crisis. Second, they gradually mastered the working skills and psychological training to cope with COVID- 19 and developed a positive attitude toward life and a high sense of professional responsibility. Finally, we learned about their needs to respond to public health emergencies such as the COVID-19 pandemic. Conclusion: COVID-19 is a disaster for all of humanity. The newly recruited male nurses are an important force in emergency rescue. Although they suffered from short-term negative emotions, they quickly adapted to the crisis. In order to better prepare for future emergencies, the disaster response capacity of newly recruited male nurses needs to be further improved. In addition, newly recruited male nurses have a strong demand for timely and personalized career development guidance.
