수술실 간호사가 인지하는 간호사 및 의사와의 협력이 간호업무성과에 미치는 영향

Purpose: The aim of this study was to identify effects of perceived nurse-nurse collaboration and nurse-physician collaboration on nursing performance in perioperative nurses. Methods: A cross-sectional survey was used and data were collected in September 2016. Participants were 186 perioperative nurses from three advanced general hospitals and nine general hospitals. Data were analyzed using descriptive statistics, independent t-test, One-way ANOVA, $Scheff{\acute{e}}$ test, Pearson correlation coefficients and hierarchical multiple regression with the SPSS/WIN 23.0 program. Results: The mean scores were for nurse-nurse collaboration, $2.92{\pm}0.28$ out of 4, for nurse-physician collaboration, $3.29{\pm}0.65$ out of 5, and for nursing performance, $3.85{\pm}0.47$ out of 5. There were statistically significant positive correlations among nurse-nurse collaboration, nurse-physician collaboration, and nursing performance. Hierarchical multiple regression analysis revealed that nurse-nurse collaboration explained an additional 29%p of nursing performance. Shared processes, conflict management, and professionalism of nurse-nurse collaboration were statistically significant predictors of nursing performance. The nurse-physician collaboration explained an additional 3%p of nursing performance. Sharing of patient information was a statistically significant predictor of nursing performance. Conclusion: Findings indicate that intervention programs that integrate and strengthen shared processes, conflict management, professionalism, and sharing of patient information are useful to enhance nursing performance.

Purpose: The study was done to examine whether consciousness of calling and life satisfaction have a mediating effect in the relationship between nursing students' purpose of life and their quality of life. Methods: The participants were 171 nursing students studying in two colleges of nursing located in Chungcheong and Gyeonggi Province. Measurements included the purpose in life questionnaire, Korea-version WHOQOL-BREF questionnaire, brief calling scale and satisfaction with life scale. Data were analyzed using descriptive analysis and multiple regression techniques with SPSS 24.0. Mediation analysis was performed according to the Baron and Kenny method and Sobel test. Results: There were significant correlations among purpose in life, consciousness of calling, satisfaction with life and quality of life. Consciousness of calling and satisfaction with life showed perfect mediating effects in the relationship between purpose in life and quality of life. Conclusion: In this study, nursing students' purpose in life had significant influences on nursing students' quality of life via calling consciousness and satisfaction with life. Therefore, to enhance quality of life for nursing students, it is necessary to build effective strategies and education programs to enhance nursing students' purpose in life, consciousness of calling and satisfaction with life.

병원간호사의 직무착근도와 간호전문직관이 재직의도에 미치는 영향

Purpose: This study was performed to investigate the effects of job embeddedness and nursing professionalism on intent to stay in hospital nurses. Methods: The study was a cross-sectional survey. Participants were 242 nurses working at four general hospitals in P city of Gyeongbuk Province, Korea. Data were collected from August 29 to September 10, 2016. Structured questionnaires were used and analyzed with the SPSS/PC program. Results: The scores were, for job embeddedness, 3.19 (out of 5.0), nursing professionalism, 3.50 (out of 5.0), and intent to stay, 5.30 (out of 8.0). The three variables were all different according to participants' general characteristics. Factors influencing intent to stay were organization fitness (one of 4 sub-factors of job embeddedness), social recognition (one of 5 sub-factors of nursing professionalism) and age (one of participants' general characteristics). These three variables explained 34% of intent to stay. Conclusion: To increase competent career nurses' intent to stay, nursing and hospital managers must develop and implement programs that help improve nurses' organization fitness to adjust well in their organization and healthcare institutions and nursing organizations must continue efforts to raise social recognition of nurses and to improve nurses' working conditions.

병원 간호사의 감정노동 측정도구 개발

Purpose: The study was done to analyze attributes of emotional labor experienced by clinical nurses working in domestic hospitals and to develop tools for measurement. Methods: The concept of nurse emotional labor was verified through concept analysis based on Hybrid model. The preliminary Nurse Emotional Labor Scale were examined for content validity, reliability and validity. The scale was verified with 500 nurses working in general hospitals located in Kangwon-do. Results: Attributes of the nurse emotional labor concept were derived from three types: surface type, internalize type and control behaviors and identified as having six constitutive factors: conscious surface behaviors, casual behaviors, empathy effort, repress, solve and endure. The results of the construct validity test of the tool showed 8 factors. The Nurse Emotional Labor Measurement Tool had a significant correlation (r=.35, p<.001) with the Scale of Morris & Felman in result of criterion-related validity. And the internal consistency reliability Cronbach's ${\alpha}$ coefficient was .93. Conclusion: The development process showed that to accurately describe emotional labor of nurses, it is necessary to consider not only quantitative aspects but also qualitative aspects and content aspects.

일개공공병원에 근무하는 간호사의 육아휴직 인식과 이용용이성이 직무만족에 미치는 영향

Purpose: The aim of this study was to identify whether awareness and perceived ease in using parental leave influence job satisfaction of nurses in a public hospital. Methods: The participants were 425 nurses working in a public hospital. A questionnaire was used to collect data from October 12 to October 25, 2016. The influence of the factors on job satisfaction of nurses was analyzed by multiple hierarchical regression. Results: Nurses taking leave accounted for 46.8% (75 nurses) of 160 married nurses. They worried about adaptation after return to work (30.7%) and complained about disadvantages related to using parental leave (45.3%). The percentage of nurses on leave replaced by new full time nurses was 70.7%. Nurses' awareness (${\beta}=.16$) of perceived ease of using parental leave (${\beta}=.32$) influenced the nurses' job satisfaction ($R^2=.21$). Conclusion: To improve job satisfaction for nurses, it is necessary to increase awareness and perceived ease of using parental leave. Public hospitals should replace nurses on leave with temporary nurses and help returning nurses adapt to their jobs as they return to work. Nursing managers should create a healthy climate in which it is easy for nurses to take parental leave.

일부 지역대학 간호졸업생의 지역병원 선택 실태 및 영향요인

Purpose: The aim of this study was to identify factors associated with new graduate nurses' choices of local hospitals. Methods: Participants were 526 new graduate nurses from one of five universities located in Busan, Ulsan or Gyeongnam. The new graduates completed a self-report questionnaire survey during February and March, 2016. The questionnaire included questions about choices of local hospitals, satisfaction with nursing major, nursing professionalism and anticipated nursing practice environment. Multiple logistic regression analysis was conducted to identify factors associated with the new graduate nurses' choices of local hospitals. Results: Of the participants, 86.3% had their first jobs in their nursing school region (local hospitals). The choice of local hospitals among new graduate nurses was related to region of origin, type of nursing school, grade point average, and anticipated nursing practice environment. Conclusion: Local hospitals need to enhance the nurse practice environment and provide institutional benefits to new graduate nurses who choose local hospitals. For quality nursing care, reducing the geographical gap and improving policy is necessary.

Purpose: This study was done to identify the relationship between perception of the importance and job performance of patient safety management. Methods: This descriptive study was performed using self-report questionnaires. Participants were 200 nurses who worked at 4 tertiary university hospitals where data were collected for 4 weeks in June 2016. Results: The scores for perception and performance were 4.28 and 4.37 points. A positive correlation was found between perception of the importance of patient safety management and job performance (r=.74, p<.001). In regression analysis, the perception of the importance of patient safety management (${\beta}=.74$, p<.001) was a significant factor in the performance of patient safety management. The regression model was statistically significant (F=134.43, p<.001) and the explanatory power of the model was 58%. Conclusion: The findings from this study indicate a need to develop strategies to improve perception of the importance of patient safety management. Also, this data should be used as a basis to develop education programs to improve awareness of the importance of patient safety management.

통제위에 따른 간호대학생의 셀프리더십 영향요인

Purpose: The purpose was to identify factors which affect nursing students' self-leadership according to their locus of control. Methods: This study was a descriptive research. Participants were 600 D city junior and senior nursing students who had completed clinical training. Results: Results showed the following characteristics of students who had high self-leadership: For students with external locus of control the following correlations with self-leadership were found:, general creativity (r=.46), social support (r=.48), academic self-efficacy (r=.29), and communication competence (r=.49). For students with internal locus of control, self-leadership was correlated with general creativity (r=.46), social support (r=.41), academic self-efficacy(r=.36), and communication competence (r=.48). For the nursing students with external locus of control, age (${\beta}=-.24$, p=.010), satisfaction with campus life (${\beta}=.16$, p=.027), general creativity (${\beta}=-.20$, p=.017), and social support (${\beta}=-.20$, p=.028) had significant effects on their self-leadership: explanatory power of 44% (F=6.53, p<.001). For students with internal locus of control, self-leadership was effected by general creativity (${\beta}=.20$, p=.011) and social support (${\beta}=.19$, p=.012): explanatory power was 42% (F=5.96, p<.001). Conclusion: Applying these findings to the curriculum of nursing students would improve their self-leadership and increase the efficiency of organizational culture and contribute to goal achievement in nursing organizations.

Purpose: This study was conducted to expand nurse's experience of harmful work environments Methods: Data were collected through in-depth dividual interviews and analyzed using Colaizzi phenomenological analysis methodology. Results: Harmful work environments experienced by nurses were categorized as followed: 'Neglect concerning infections', 'Being thrown into a violent working environment', 'The hard times of being alone', 'Struggling with the harmful work environments'. Conclusion: The results of this study provide a deep understanding of the harmful work environments. It is necessary to create safe work environments and it is important to improve awareness in the system of organizations as well as individuals. Also, it is necessary to develop tools to measure work environment including risk factors.

Mediating Effect of Social Capital between Transformational Leadership Behavior and Organizational Citizenship Behavior in Hospital Nurses

Purpose: This study was conducted to examine the current status of transformational leadership behavior (TLB) and organizational citizenship behavior (OCB), and to investigate the mediating effect of social capital on the relationship between TLB and OCB in hospital nurses. Methods: A cross-sectional descriptive study design was used with a convenience sample of 219 nurses from two university-affiliated teaching hospitals in South Korea. Results: The survey instruments measured TLB, OCB, and social capital. Data were analyzed using t-tests, ANOVA, $Scheff{\acute{e}}^{\prime}s$ test, Baron and Kenny's regression method. The average level of TLB was 3.13 points, for OCB 3.64 points, and for social capital 3.24 points in social capital. Positive correlations were found between TLB, OCB, and social capital. Hospital nurses' social capital showed a significant mediating effect on the relationship between TLB and OCB. Conclusion: Findings indicate that TLB is a contributing factor to better OCB, and TLB can lead to improved OCB through social capital. Interventions to improve social capital of nurses in health-care organizations have important implications for OCB improvement.

낙상 환자 관리 시뮬레이션 교육 프로그램 개발 및 평가

Purpose: This study was carried out to develop simulation scenarios for the management patient falls and to evaluate the effects of using the scenarios with student nurses. Methods: The research design was a quasi-experimental study using a methodology study. Study participants were 30 students who were in 4th year of nursing at one College of Nursing. Results: When comparing knowledge of falls before and after the simulation program, it was found that knowledge increased by 4.90 (from 24.60 pre-test to 29.50 post-test). For clinical performance of fall management, the score for assessment was 10.17 out of 16, for intervention 5.97 out of 10, and for evaluation 7.33 out of 8. The average score for reporting a fall to the doctor was 19.87 out of 30 based on SBAR. Prior to the implementation of the simulation program, the confidence of reporting to the physician was less than 5 in all four areas, but self-confidence improved by more than 6 points in all four areas after the program was implemented. Conclusion: Findings indicate that results of fall management simulation practice can contribute to nursing students' knowledge of falls, as well as to nursing interventions and post-treatment following a patient fall.

DACUM 기법을 활용한 상급종합병원 간호사의 직무기술서 개발: 일반병동과 중환자실을 중심으로

Purpose: This study was done to clarify nurses' duties, tasks and task elements and to develop a job description for clinical nurses on general and intensive care units in tertiary-level hospitals, and dramatically reflect changing medical trends in Korea. Methods: The job description was developed based on the Developing a Curriculum Method (DACUM). The questionnaire included frequency, importance, and difficulty of duties, tasks and task elements, measured on a 4-point scale. Results were analyzed using SPSS version 21.0. Data were collected from September 4 to 7, 2017, and analyzed using descriptive statistics and a scattered graph. Results: The job description consisted of 10 duties, 38 tasks, and 51 elements. Of the 10 duties, the highest duty in order of importance was 'Direct nursing care' followed by 'Infection control'. The highest duties according to frequency and difficulty were 'Document and notify' and 'Research and quality improvement'. 'Safety management' and 'Infection control' were considered as relatively simple duties, however, these two duties were still included as important jobs for clinical nurses. Conclusion: The job descriptions for clinical nurses developed from this study contain nursing ethics and safety as well as infection control, to faithfully reflect clinical nurses' jobs.

Purpose: The purpose of this qualitative study was to evaluate nursing students' experience of incivility during clinical practice. Methods: Data were collected from 11 in-depth interviews. The main question was "Could you describe your experience of incivility during clinical practice?" The qualitative data from the field notes and transcriptions of the interviews were analyzed using Colaizzi's method. Results: The experiential themes of incivility to nursing students during clinical practice were 'exposure to verbal abuse', 'receiving unjust reproaches', 'treated as an insignificant person', 'excluded from nursing as a bothersome person', and imidated in an unfamiliar atmosphere'. Conclusion: Nursing students experienced incivility in many ways and from a variety of sources, such as nursing staff, patients, patients' families, and other employees during clinical practice as part of the 3rd and 4th year curriculum. Therefore, it is necessary to provide a safer environment for clinical practice. Also finding solutions to these incivility problems should lead to improvement in students' clinical practice.

간호사가 지각하는 수간호사의 진성리더십과 조직시민행동의 관계에서 개인-조직 가치일치의 매개효과

Purpose: The purpose of this study was to investigate the mediating effect of person-organization value congruence between the authentic leadership of head nurses and the organizational citizenship behavior perceived by hospital nurses. Methods: The participants were 211 nurses working in 3 general hospitals located in C-city, in South Korea. Data were analyzed using descriptive statistics, ANOVA, $Scheff{\acute{e}}$ test, Pearson correlations and structural equation model (SEM) with the PASW 24 and AMOS 24 program. Results: Authentic leadership of head nurses had a statistically significant positive correlation with person-organization value congruence (r=.34, p<.001). Also, authentic leadership of head nurses (r=.50, p<.001), and person-organization value congruence (r=.62, p<.001) showed a statistically significant positive correlation with organizational citizenship behavior. There was a partial mediating effect of person-organization value congruence between authentic leadership of head nurse and organizational citizenship behavior perceived by hospital nurses ($x^2=205.78$, p<.001). Conclusion: The results suggest that nurse managers need to improve the person-organization value congruence and authentic leadership of head nurses to reinforce organizational citizenship behavior of nurses in human resource management.

병원 진료에 대한 환자들의 참여 경험

Purpose: The aim of this study was to explore patients' experience of participation in their hospital care. Methods: Face-to-face interviews using a semi-structured interview guide were conducted with a total of 21 patients in a tertiary hospital in Seoul, South Korea. Collected data were analyzed using a qualitative content analysis. Results: Two categories of patients' experience of participation in hospital care emerged: 1) Reactive participatory activity, 2) active participatory activity. Major participatory activities included 'complying with medical instructions', 'listening', 'responding' and 'questioning'. Healthcare professionals' attentive attitudes and explanation, and availability of care equipment were facilitating factors affecting patient participation in their care, whereas limited accessibility to and poor communication with healthcare professionals, inconvenient patient room, and patient factors were barriers to their participation. Conclusion: The findings in this study reveal that patients tend to participate in their care in a reactive way and that healthcare professionals have a significant role in facilitating patients' participation. Furthermore, the care environment and patient factors need to be considered to promote patient participation in hospital care.

간호대학생의 리더십 생활기술과 성취동기가 셀프리더십에 미치는 영향

Purpose: Nursing students are expected to show self-leadership. For this study, leadership life skills and achievement motivation were examined for their influence on self-leadership in student nurses. Methods: A descriptive study was conducted using a self-report questionnaire completed by 239 nursing students. Data were analyzed using t-test, one way ANOVA, $Scheff{\acute{e}}$ test, Pearson correlation coefficients, and stepwise multiple regression analysis with SPSS 20.0 program. Results: The mean score for self-leadership was 3.49, for leadership life skills, 3.65, and for achievement motivation, 3.42. There were significant differences in self-leadership according to task orientation (${\beta}=.25$, p<.001), responsibility (${\beta}=.24$, p<.001), challenge spirit (${\beta}=.22$ p<.001), future orientation (${\beta}=.16$, p=.001), leadership life skills (${\beta}=.08$, p=.009), and interpersonal relationships (${\beta}=-.09$, p<.001). The explanation power of the regression model was 51.1% and it was statistically significant (F=41.87, p<.001). Conclusion: The results of this study show that factors influencing self-leadership are leadership life skills and achievement motivation. Therefore, it is necessary to develop intervention programs to improve self-leadership in nursing students for enhancement of task orientation, responsibility, challenge spirit, future orientation, and leadership life skills.

Purpose: The purpose of this study was to elucidate nursing performance to superior's ethical leadership as perceived by nurses and the mediating effect of faith in supervisor. Methods: For this study a descriptive survey design was used. Participants were 258 nurses who worked in general hospital and completed the self-report questionnaire. Data were collected from May 17 to May 31, 2017 and were analyzed using independent t-test, ANOVA, $Scheff{\acute{e}}$ test, and Path analysis with SPSS 22.0 and AMOS 21.0. Results: Superior's ethical leadership as perceived by the nurses and faith in the supervisor were significant variables predicting the level of nursing performance and accounted for 48.1% of the variance. Faith in the supervisor as a mediator was found to have a partial mediating effect. Conclusion: In order to improve the nurses' performance, it is important to establish a work environment so that hospital nurses can perform their work with faith in their supervisors, as well as having superiors who are ethical leaders.

중소병원 간호사의 이직의도 영향요인

Purpose: The purpose of this study was to explore the predictors of turnover intention among nurses working in small and medium-sized hospitals in South Korea. Methods: Data were obtained from a survey conducted in 2016 on welfare policy and system improvement. The participants were nurses of the Korean Nurses Association who worked in small and medium-sized hospitals. The responses of 2,011 nurses were analyzed using hierarchical multiple regression analysis, which was performed to explore the predictors of nurses' turnover intentions. Results: The findings of this study showed that the predictors significantly reducing nurses' desire to leave their hospitals were a 40-hour workweek, satisfaction with the salary, and six out of the 40 fringe benefits provided by their hospitals: the provision of uniforms and work shoes, paid menstrual leave, operational costs for the department, job-related educational costs, and in-house nurse training program. Conclusion: These findings suggest that hospitals should provide proper work hours and reasonable rewards for nurses' work in order to reduce turnover intention among their nurses.

Purpose: The purpose of this study was to identify the effects of social support on the relationship between practice stress, emotional labor, and burnout among nursing students. Methods: The sample for this study consisted of 199 students from four universities located in Korea, and had a practicum experience at least one semester before the study began. Data were analyzed using descriptive statistics, t-test, ANOVA, $Scheff{\acute{e}}$ test, Pearson correlation and hierarchical multiple regression with SPSS/WIN 23.0. Results: It was found that: (a) burnout had a positive correlation with both practice stress and emotional labor, whereas a negative correlation was observed with social support; (b) social support mediated the relationship between practice stress and burnout, and (c) social support mediated the relationship between emotional labor and burnout. Conclusion: Results of the study suggest that social support decreases the negative effects of practice stress and emotional labor on burnout. Therefore, we need to find ways to strengthen social support for students.

Nurses' Perception of Patient Safety Culture and Safety Control in Patient Safety Management Activities

Purpose: There have been global initiatives and efforts over the last decade to manage patient safety. Thus aims of this study were to examine university hospital nurses' perceptions of patient safety culture and levels of safety control, and to identify factors that affect patient safety management activities. Methods: Participants were 222 nurses who had worked as nurses for more than one year and who conducted patient safety management activities at a university hospital. Data were collected using structured questionnaires and were analyzed using descriptive statistics, t-test, one-way ANOVA, Pearson correlation coefficient, and multiple regression analysis with SPSS/WIN 20.0 computer program. Results: General factors which positively impacted nurses' patient safety management were total length of work in nursing, total length of work in present hospital, and experience of a patient safety accident along with safety factors of perception of communication about accident related events and frequency of reporting accident events. These variables explained 45% of the variance in patient safety management activities. Conclusion: The findings from this study suggest solutions to promote patient safety management activities in hospitals and provide basic background for nursing education intervention strategies to promote safety control and patient safety management activities intended for nurses.
