Background: Earthing (grounding) refers to the practice of electrically connecting the human body to the Earth's surface to induce physiological changes. While earthing research has mainly focused on underlying mechanisms such as inflammation, autonomic nervous system regulation, and bioelectrical balance, information regarding its relevance to specific clinical medical specialties remains limited. Purpose: This study aimed to identify the distribution of clinical application targets (CATs) mentioned in earthing literature across medical specialties and discuss its clinical implications. Methods: Literature published from 2000 to 2025 was searched using the academic database Dimensions with the keywords 'earthing' and 'clinical application'. CATs were extracted from the searched articles using a generative AI model. The extracted CATs were assigned to 26 hospital medical specialties defined under the Enforcement Rule of the Medical Service Act based on five predefined rules (R1–R5). Results: A total of 516 earthing articles were identified, from which 48 unique CATs were extracted. The majority of the CATs (79.17%) were addressed in only a single article. The most frequently investigated CATs were inflammation/immune response (10 articles), pain (8 articles), sleep disturbance/sleep quality (6 articles), blood flow/blood viscosity (5 articles), and stress (3 articles). Family Medicine and Internal Medicine were associated with the highest number of CATs (14 CATs each), followed by Laboratory Medicine, Preventive Medicine, and Rehabilitation Medicine (8 CATs each). Regarding the proportion of related earthing articles by medical specialty, Family Medicine accounted for the highest share (53.85%), followed by Internal Medicine (48.72%), Laboratory Medicine (38.46%), Rehabilitation Medicine (35.90%), Neurology and Psychiatry (28.21% each), Neurosurgery (23.08%), and Preventive Medicine and Anesthesiology/Pain Medicine (20.51% each). These findings indicate that the clinical application targets of earthing research are highly relevant to primary comprehensive care, internal diseases, physiological marker evaluation, and functional recovery. Conclusions: This study confirmed that the clinical application targets of earthing research are mostly concentrated on systemic and functional health issues, demonstrating high relevance to comprehensive health management and non-surgical clinical domains. Since this study did not directly evaluate the clinical efficacy of earthing, follow-up studies verifying clinical effectiveness focusing on frequently researched CATs are needed to enhance its practical applicability in clinical settings. The results of this study can serve as an academic foundation for understanding the clinical utility of earthing in future medical practice.
