Native logging by cloud service providers operates at the network and infrastructure layers and cannot record three of the five access-record items required by ISMS-P criterion 2.9.4: the account identifier, the data subject, and the performed action. The application can log these itself, but that log is produced by the audited application and can be forged once it is compromised. This paper proposes MECA (Medical eBPF Compliance Architecture), a kernel-layer audit tier that neither trusts nor modifies the application, combining L4 and L7 hooks, correlation by process identifier, and in-kernel masking. An AWS measurement captured all five items, including the three that native logging misses. The baseline overhead was -2.82% in throughput and +7.55 ms in latency. Access records survived deletion and forgery of application logs. Rather than asserting the general performance of kernel-layer auditing, this study demonstrates the visibility gap observed under a typical medical-cloud configuration and the effectiveness of an application-independent audit tier that mitigates it.
