This study examined middle school students’ and caregivers’ perceptions and needs regarding school-based sex education to provide a foundation for developing effective and learner-centered sex education programs. An online self-report survey was conducted with 200 middle school students and 200 caregivers. Multiple-response items were analyzed using frequency analysis and chi-square tests, and weighted scores were calculated to identify priorities in sex education needs. All middle school students reported having received sex education at school; however, their satisfaction was relatively low, largely due to repetitive and familiar content. Students expressed a preference for experiential teaching methods and identified safe and healthy sexual relationships as the highest priority for sex education (weighted score=201) followed by puberty and related physical and psychological changes (weighted score=150). Among caregivers, 73.0% reported having received sex education themselves, yet many were unaware of the sex education their children received at school. Caregivers generally recognized the need for school-based sex education and expected it to help their children develop knowledge about their bodies and the ability to protect themselves. Based on these findings, implications for developing learner-centered, experiential sex education and strengthening school-caregiver collaboration were discussed. This study is limited by its reliance on online self-reported data and the cross-sectional nature of the study, which limits the generalizability and causal interpretation of the findings.
